Work out how long ago your scheduled injection day was, then follow the licensed rule. The rule is the same whichever dose you are on, from 2.5mg to 15mg.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.113 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

In practice that gives you a simple routine:

  • Check your calendar or tracker. Count the days since the day you should have injected.
  • Inside the window: inject as soon as you can, at any time of day, with or without food. Mounjaro works the same morning or evening.1
  • Outside the window: do not inject. Wait for your normal day and take your usual dose.
  • Either way: carry on with your regular weekly day afterwards. Do not shift the whole schedule unless you have a reason to change your injection day.
  • Never double up. A larger or repeated dose does not make up for the one you missed.

A late dose is one you take inside the licensed window. A skipped dose is one you leave out because too much time has passed. Both are normal ways of handling a missed injection, and neither is a failure of treatment.

05 · What happens in your body

What happens in your body when you miss an injection

Missing a single injection does not undo your progress. Mounjaro leaves the body gradually, not all at once. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

Mounjaro activates the GIP and GLP-1 receptors, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 When a dose is late or skipped, the level of medicine in your body gradually falls.

If you feel unwell, or your symptoms worry you, speak to your prescriber or pharmacist for advice.

Weight loss on Mounjaro is gradual by design. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.1 One late injection will not show up as a lasting change in the trend, though results vary between people. 06 · Never double up

Why you should never double up or microdose

Do not inject extra to catch up, and do not split or spread a dose. Neither approach is licensed. A catch-up dose breaks the spacing rule: never take two doses within 3 days of each other.1

Altering doses outside the licence means unknown dosing accuracy and no safety data.

07 · Changing your injection day

Can you take Mounjaro early or change your injection day?

Yes, you can change your weekly injection day, but only with enough space between doses. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Put another way, there must be at least 3 days between two doses.

This answers the common worry about jabbing early. Taking an injection earlier than planned is only fine when it respects that minimum gap. Anything that puts two doses closer than that breaks the rule.

Changing the day can be a sensible reaction to missed injections. If your chosen day keeps clashing with work, travel or family routines, move to a day you can keep. There is no best time of day: Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember (many people choose a weekend morning so early side effects fall on days off) and stick to it.1

Once you have changed the day, treat the new day as your fixed schedule going forward. Set a recurring phone reminder, and note the change in your tracker so you do not accidentally inject on both days.

08 · Side effects after a gap

Side effects after a late or restarted dose

Mounjaro can cause digestive side effects, whether a dose is on time or late. Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common — affecting more than 1 in 10 people — are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13 Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.13

Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13 If your gap was long enough that your prescriber restarts you on a lower dose, expect the same pattern as when you began.

Mild nausea, altered appetite and tiredness in the days after an injection are the commonest early effects and usually settle.1 Speak to your prescriber or pharmacist if they continue or you cannot cope with them.

When to seek urgent help

Serious side effects are uncommon but need urgent attention. Get medical help straight away for:

  • severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis);
  • pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems);
  • signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.113 You can also report any suspected side effect through the MHRA Yellow Card scheme.19

09 · Contraception and gaps

Contraception, dose changes and missed injections

If you use the oral contraceptive pill, a gap in treatment can matter when your dose changes. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7

The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 This applies to the pill only — implants, coils and injections are unaffected.1

If a long gap means your prescriber restarts you at a different dose, ask them about your contraception.

10 · Pens, storage and travel

Pens, storage and travel: other reasons injections get missed

Practical problems cause many missed doses, usually storage, travel or a pen that has run out. Knowing the basics prevents most of them.

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.1 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

Each KwikPen contains four weekly doses.1

Travelling

You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription.1 In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.1

How to stop missing Mounjaro injections

A fixed day, a reminder and a reorder habit prevent nearly every missed dose. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1

These habits help:

  • Choose a day that fits your life. Many people pick a weekend morning, because early side effects then fall on days off.
  • Set a repeating alarm. Make it recur weekly on your phone and name it clearly.
  • Track each dose. A diary or the free GLPTrackr app tells you at a glance whether you injected.
  • Reorder early. Do not wait until the last dose. Repeat orders re-check suitability, and there is no auto-shipping, so you control the timing.
  • Keep the pen visible. Store it in the fridge where you will see it, with the cap on.
  • Plan for trips. Pack pens in hand luggage and check the in-use limit before you travel.
  • Rotate sites. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection.

12 · Your dose after a gap

Which dose to take after a gap, and when to ask your prescriber

For a single missed injection, you carry on at the same dose. Everyone starts Mounjaro on 2.5mg once a week. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

That wording matters after a gap, because the steps depend on how you respond, not on a fixed calendar. If you have skipped a dose during titration, your next increase may simply come later. Your prescriber guides the right pace.

Contact your prescriber before injecting if any of these apply:

  • You have missed several injections in a row.
  • You stopped treatment for a while and want to restart.
  • You were unwell, vomiting or unable to eat or drink for a while.
  • You are unsure which dose is in your pen.
  • Your side effects were hard to manage before the gap.

Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better.1 Your prescriber can decide what suits you after a break.

Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
