Royal Mail normally leaves a card or sends a tracking notification explaining whether the parcel is at your local delivery office or can be redelivered. Is the pen still usable, and will you miss an injection?
Usually yes, provided the pen has not frozen and the pharmacy is happy with how long it was out of the fridge. Unopened pens keep in the fridge until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1
If the delay leaves you without a pen on injection day, the rule is simple. Take the dose as soon as you can within 4 days (96 hours) of the scheduled time. 04 · Why the fridge matters
Why a missed delivery matters for a fridge medicine
A missed delivery matters more for Mounjaro than for most parcels because the pen is stored cold. The rule from the product information is to keep pens in a fridge at 2–8°C and never freeze them.1 A parcel left at a depot, on a doorstep or in a warm hallway is outside that setting for as long as it sits there.
That does not mean every delay ruins a pen. It means the delay should be as short as you can make it, and that you should know what to check when the parcel arrives. The main things are the temperature of the pen, whether anything has frozen, and the expiry date on the box.
The pen is a pre-filled multi-dose KwikPen containing four weekly doses, so a single damaged pen affects a whole month of treatment.1 That is one reason pharmacies use insulated packaging for the journey. The packaging is designed for the delivery window, not as a substitute for the fridge.
Once you have the parcel, open it promptly rather than leaving it on the side. Take the pen out of the outer box only when you are ready to inject, keep the cap on to protect it from light, and put it in the fridge straight away.1 Check the pen for anything that looks wrong, and keep the packaging until you have confirmed everything is as expected.
If anything about the pen worries you, the pharmacy that supplied it is the right first call. It holds the dispensing record and can tell you whether a replacement is appropriate. Do not use the pen while you wait for an answer. A short wait costs you nothing, while an injection from a pen that may be damaged is a risk you cannot undo.
It also helps to know your own routine. If you are often out during the day, arrange delivery for a time or place where someone can take the parcel straight into the cool. Delivery to a work address, a trusted neighbour or a safe place all reduce the time the pen spends unrefrigerated.
Finally, remember that the fridge rule applies to unopened pens. Once a pen is in use, the licence allows it to be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 That flexibility is for the pen you are currently using, not for new pens waiting in a parcel.
05 · Royal Mail missed-item steps
The usual steps after a missed Royal Mail delivery
The first step is to read the card or notification, because it tells you what Royal Mail will do with this particular item. Follow the instructions you are given.
Typically, when nobody is in, the delivery worker leaves a card or sends a message with a tracking reference. The parcel may then be held at your local delivery office, and the notice usually offers redelivery or collection. Redelivery can often be arranged online with the tracking reference, sometimes to a different day, a neighbour or a safe place.
If you choose collection, take the card with you, and check the opening hours of the delivery office before you go. Offices are not open all day, and a wasted trip means another night in a warm van or sorting area rather than your fridge.
Choose the option that gets the pen into your hands soonest. For a fridge medicine, a fast redelivery to a place where someone will be home beats a slower option that is more convenient on paper. If you work during the day, consider redelivery to a neighbour who will be home, or collection at the first opportunity.
Do not leave it for a week and hope. Parcels held at an office are not held indefinitely, and a parcel that goes back to the sender means a delay that could cost you an injection. If the notice mentions a deadline, treat it as firm.
Tell the pharmacy what has happened as soon as you know. They can see the tracking, confirm what service was used, and tell you whether they would advise using the pen once it reaches you. Pharmacies handle this regularly, and asking costs nothing.
When you do collect it, check the box for damage before you leave, and check the pen itself at home. If the box is crushed, wet or visibly opened, take photos and send them to the pharmacy with your order details. That makes any replacement conversation quicker.

What to do if your Mounjaro arrives warm
If the pen feels warm, do not use it until you have spoken to the pharmacy. Unopened pens should be kept in a fridge at 2–8°C and never frozen.1
First, put the pen in the fridge so it stops warming further. Second, note roughly how long the delivery was delayed and what the weather was like. Third, contact the pharmacy with that information and your order details. The pharmacist can then tell you whether to use the pen or whether a replacement is needed.

Warm is different from frozen. Never use a pen that has been frozen.1 Cold-weather deliveries can carry this risk, particularly if a parcel sits in an unheated porch, outbuilding or letterbox. If you find a pen with ice crystals, or you think it may have frozen, do not inject. Contact the pharmacy for advice.
Do not try to test the pen by injecting a small amount, and do not split doses to make a doubtful pen last. Altering doses outside the licence means unknown dosing accuracy and no safety data.1 If the pen is not right, the answer is a replacement, not a workaround.
Check the label and the pen's appearance too. The expiry date should be printed on the box, and the pen should look as described in the patient information leaflet.13 If anything differs, say so when you call.
Finally, keep the packaging. Photographs of the box, the pen and any damage make it easier for the pharmacy to decide quickly, and quick decisions matter when you are counting days to your next injection.
If the pharmacy advises using the pen, store it in the fridge as normal and inject on your usual day. If they advise against it, ask what happens next with the replacement.
07 · Time in packaging
How long can Mounjaro stay in delivery packaging?
The product information sets no number of hours for a pen in a delivery box. What it does say is that unopened pens should be kept in the fridge until first use and keep until the printed expiry date.1 Packaging is there to protect the pen during transit, and it should come off the clock as soon as the parcel is in your hands.
That is why you should open the delivery on the day it arrives. Take the pens out and put them in the fridge. Do not leave the box in a car, a porch, a warm kitchen or beside a radiator, even if it feels cool for now.
A pen that is in use can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 That figure concerns the pen you are injecting from, such as when you travel. It does not tell you how long a new, unopened pen can sit in a courier's van or a depot.
If you want to know where the line falls for your pen after a delay, the right route is the pharmacy, with the patient information leaflet as backup.13 They can look at your delivery times and tell you what is reasonable.
Practical tips follow from this. Avoid ordering for a day when nobody will be home. Pick a delivery address with someone available. If a delivery is delayed, ask the courier or pharmacy for an updated time, and clear space in the fridge beforehand so the pens go straight in.
Travelling is a separate matter. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements, and for hot climates an insulated pouch is the sensible choice.1 Our storage rules for unopened pens are stricter than for the pen in your bag, so keep spare pens in the fridge at home.
08 · Missing your injection
What a late delivery means for your weekly injection
A late parcel only matters for your schedule if it leaves you without a pen on injection day, and the missed-dose rule then tells you what to do. Take the dose as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day.1
Never take two doses within 3 days of each other.1 This is the rule people most often get wrong when they are trying to catch up, so it is worth being strict about it. Catching up by doubling does not make up for the lost dose; it simply adds risk.
An example helps. Suppose your injection day is Sunday and the parcel arrives on Tuesday. That is two days late, so you are within the 4-day window: inject when the pen arrives and the pharmacy has confirmed it is fine, then decide whether to keep Sunday as your day. If the parcel arrives on Friday, more than 4 days have passed, so you skip that dose and inject on the following Sunday.
If you would like to move your injection day, for example to match the day the pen arrives, that is possible as long as at least 3 days (72 hours) have passed since your last dose.1 Choosing a new fixed day is fine, and what matters is the same day each week from then on. Time of day does not matter: Mounjaro works the same morning or evening, with or without food.1
When you do inject, follow the pen's instructions.
After a skipped dose you may notice appetite creeping back, or mild side effects such as nausea when you restart. The most common early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.1 If you are worried about how you feel, talk to your prescriber.

09 · A week or longer without a pen
What if the delay means a week or more without Mounjaro?
If a delivery problem leaves you without a pen for a week or longer, speak to your prescriber before restarting, rather than working out your own plan. The licensed dosing rule covers a single missed dose and the 4-day (96-hour) window.
What you can do in the meantime is stay on top of the basics. Keep to your reduced-calorie diet and physical activity, since treatment works alongside them, not instead of them.1 Note down the date of your last injection, your dose, and any symptoms. That information is exactly what a prescriber needs to advise on your next step.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.1 That helps explain why a short gap does not instantly remove the effects, and why appetite changes tend to return gradually rather than overnight. It is not a reason to delay getting in touch, and it is not a way to judge how long a gap is safe.
Do not try to compensate. The licensed dose ladder starts at 2.5mg and steps up by 2.5mg at a minimum of 4-week intervals, depending on how you respond and tolerate it.1
Early contact is useful because the pharmacy can often sort a replacement quickly, which may mean the gap never becomes a week. If you are on a repeat order, tell the team about the delay when you reorder, since repeat orders re-check suitability anyway and there is no auto-shipping.

The most effective step is to choose a delivery time and address when someone will be there, and to order before you run out. If you leave ordering until your last pen is almost finished, a single missed delivery can mean a skipped dose.

11 · Lost or delayed parcels
Lost, delayed or damaged parcels: who to contact
If a parcel is lost, delayed or damaged, report it to the seller first, because the seller is your contact and handles any claim with the carrier. Its published terms and customer service are the source for those answers.
Gather your order number, the tracking reference, the date and time of the missed delivery, and any photos of damage. Send these to the pharmacy in one message.
For a medicine, the question that matters most is not compensation but replacement. A pharmacy cannot simply re-send a prescription medicine without checking what happened, so be clear and honest about the circumstances. If the parcel was delivered to a neighbour or a safe place that you did not choose, say so.
Our own position, for orders placed with us, is this. Dispensed prescription medicines cannot be returned, except if the item is faulty, damaged or wrong, in which case you should contact us within 3 days. Refunds take 5–10 working days. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
If you need to speak to someone at Royal Mail, use its published customer service channels, and have the tracking reference ready. If you cannot get through, going back to the sender often resolves things faster, since they have an existing relationship with the carrier.
When a parcel arrives late but intact, keep the evidence of the timing anyway. If the pharmacy later asks about the pen's journey, a note of when you collected it and where it had been is more useful than a rough memory.
If you suspect a parcel was opened or tampered with, do not use the contents. Report side effects or suspected problems with a medicine through the MHRA Yellow Card scheme.19
12 · Planning your supply
Planning supply so one missed parcel does not derail treatment
Plan your supply around the pen cycle, since each pen contains four weekly doses and a single dose ladder step lasts at least four weeks.1 That gives you a clear reorder point: the second week of each pen. Reordering then leaves time for a delayed parcel, a redelivery or a replacement.
It also helps to know the costs when you plan ahead. At standard prices, the first four pens (2.5mg, 5mg, 7.5mg and 10mg) cost £959.96. A first year of 13 pens costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. These totals are before discounts: a new customer's first order is £40 less, and every later order is £25 less. If you are not yet on treatment, the eligibility criteria are the same however you receive your pen. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition, alongside a reduced-calorie diet and increased physical activity.1 A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable.
Mounjaro is also available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the primary-care rollout began in June 2025 with the highest-need groups first.4 Many people who meet the licensed criteria choose regulated private treatment rather than waiting.
A word on safe sourcing, since delivery problems sometimes tempt people to look elsewhere. 13 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
