The missed dose rule, explained step by step

The missed dose rule exists so that your doses stay spaced out safely, and it is the same whether you forgot by a day or by a week. Mounjaro is a once-weekly injection, taken on the same day each week.1 The rule works out how far you are from that day.

Start by identifying the day you should have injected. Then count the days since. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.13 That means a dose missed by a day or two is usually still worth taking, and you then return to your regular injection day.

The second half is the one that applies to a full missed week. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.13 The key phrase is skip entirely. There is no half dose, no smaller catch-up dose and no extra dose later in the week.

It helps to picture the calendar. Say your injection day is Sunday and you did not inject last Sunday. By the following Sunday, the late-dose window for last week's dose closed days ago. The Sunday that has just arrived is your next scheduled dose. You inject your usual dose today, and you are back on schedule.

Why does the rule forbid two doses close together? Because tirzepatide stays in the body for a long time, a second dose soon after the first stacks on top of medicine that has not yet cleared. The spacing in the label is there to avoid that overlap.1 If you ever feel tempted to compensate for a miss, the licensed answer is always the same: do not.

The half-life is about five days, and after a final dose it takes roughly four to five weeks to clear.1 There is no way to speed this up, and nothing needs doing while it clears. To understand what you might feel, it helps to recall what the medicine does. Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1. With a lower level of medicine around, some people notice that this effect feels less pronounced for a few days. Weight loss on Mounjaro is not linear, and the pattern over months matters far more than a single week.1 Think of the missed dose as a short dip in a long process.

Why might a prescriber want to talk to you? Because a longer break can change how comfortable your next dose feels, and because the right dose depends on how you respond and tolerate it.1 Everyone starts on 2.5mg once a week, and the dose then increases to 5mg after at least four weeks and can step up by 2.5mg at a minimum of four-week intervals, up to a maximum of 15mg weekly.1 Your prescriber will guide the right pace, and they are the person to ask whether your current dose is still the right place to restart.

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If you restart and notice these again, that is not unusual; get medical help straight away rather than pushing through severe symptoms, and get medical help straight away for severe, persistent stomach pain. If you are on one of the lower doses, remember that the starting dose is about letting your body adjust, not about weight loss itself.

You may also want to move your injection day. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.13 That can help if the missed week happened because your old day clashed with work or family routines. After a gap, you can reset onto a day that suits you, provided you respect that spacing.

As for timing within the day, there is no best time. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.

Before you inject, check the pen as normal. Rotate your injection site as usual, using the stomach area, the front of the thigh, or the back of the upper arm if someone else injects.

Each KwikPen contains four weekly doses.1 If you skip one, you will have a dose left in the pen for longer than you expected, and that is where the in-use rule matters. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.13 Keeping the pen in the fridge between injections is the simplest way to stay within the storage guidance, and your pharmacy can tell you what to do with a pen that has been in use for a long time. If you are unsure about the state of a half-used pen, ask the pharmacy rather than guessing.

Unopened pens should be kept in the fridge, and they keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1 This matters if your missed week was caused by a delivery problem or a trip, and a pen sat somewhere warm or got left out of the fridge.

Mounjaro is injected once a week on the same day, at any time of day, with or without food.1 There is no best time of day, so pick one you will remember. Many people choose a weekend morning so early side effects fall on days off.

Start with the obvious tools. Set a repeating phone alarm on your injection day, and put a reminder in your calendar a day earlier to check you have a pen.
