For most people, very little happens straight away, because these medicines do not switch off the moment a dose is late. Mounjaro, for example, is a weekly injection that works by activating the GIP and GLP-1 receptors, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1
What you may notice is a gradual change over the following days. Hunger between meals can feel stronger and snacking may become harder to resist. If you take a GLP-1 medicine for diabetes, your blood sugar readings may also run higher than usual, which is worth mentioning to your prescriber or diabetes team.
The real risk of a missed dose is not physical danger. It is that a late injection turns into a skipped week, then a skipped month, and the progress you built slips away quietly. That is why a clear rule matters more than worry.

Why one miss is manageable
A single late or missed GLP-1 dose is manageable because these medicines are built around a long-acting weekly rhythm. Mounjaro is a weekly injectable prescription medicine made by Eli Lilly, supplied as a pre-filled multi-dose pen, and it is designed so that the effect carries smoothly from one injection to the next.1 The medicine does not vanish overnight when a dose is late.
The mechanism explains why. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 When the amount of medicine in your body falls, those effects soften gradually rather than stopping abruptly.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 That is a statement about stopping altogether, but it shows the same principle on a smaller scale.
What you might notice
Because the effect fades gradually, the changes after a missed dose tend to be subtle at first. Many people describe appetite creeping back, snacks becoming harder to resist, and portion sizes feeling less satisfying. These are the same appetite signals the medicine was dampening, so it is understandable that they return a little as levels fall.
None of this means the treatment has stopped working. Getting back to your normal day, inside the rules, restores the pattern. If you are on treatment for diabetes, remember that blood sugar control is part of the picture, so keep monitoring as your diabetes team has advised.

A missed dose does not undo everything you have achieved so far. Weight change on this medicine is gradual by design, and results build month by month, so the trend over time matters far more than any single week.1 A late injection is a bump in that trend, not a reset. Early loss also includes water weight, so a short-term change on the scales after a miss says little about the longer picture.
It also does not, on its own, call for any change to your plan. The Mounjaro missed-dose rule is short, and it is worth knowing by heart so you can act without searching. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.113
Notice that the rule has three working parts. There is a window in which you may still take the late dose. There is a cut-off after which you skip it. And there is a safety limit that protects you from stacking doses too close together. The third part is the one people most often forget when they are anxious about having missed one.
Working through it in practice
Start by finding the day and time the injection was due. Count forward from that moment. If you are still inside the window, inject as soon as you can, using the correct dose shown in the pen window, and then carry on with your usual day. You do not need to change your routine permanently because of one late injection.
If the window has passed, do nothing for that dose. Skipping is a legitimate, planned outcome of the rule, not a failure. Wait for your next normal injection day, take that dose as usual and carry on. Do not try to compensate with a larger dose, and do not bring the next dose forward.
Why doubling up is the real danger
The rule explicitly says never to take two doses within 3 days of each other. That limit matters because taking doses too close together is not safe. Mild nausea, altered appetite and tiredness are the commonest early effects of the medicine and usually settle, but there is no reason to invite more of them.1
When the rule does not give you the answer
The rule covers a single missed weekly dose. Changing your injection day
If your current injection day keeps getting missed, you can move it. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That makes changing day a safe, planned fix for a schedule that does not fit your life, rather than a workaround.
People often miss doses for predictable reasons. The day lands on a busy work evening, a regular commitment, a weekend away or a shift pattern. If you can see a pattern in your misses, the simplest solution is usually to move the day rather than keep fighting it. The only constraint is leaving enough gap since your last injection.
How to choose a better day
There is no best time of day to inject. Mounjaro works the same in the morning or evening, with or without food. What matters is the same day each week, so pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off, though that is a preference rather than a requirement.1
Think about what will actually trigger the habit. Some people attach the injection to a weekly event such as a shop, a bin day or a regular call. Others set a recurring phone alarm and keep the pen in a visible place in the fridge. Moving the day safely
When you do move the day, count from your last injection, not from the day it was due. If that last injection has already passed the required gap, you can inject on the new day. If not, wait until the gap has been reached. The same principle sits behind the missed-dose rule: doses must never be bunched too close together.

The most reliable way to avoid missed doses is not to remember harder but to build the injection into something that already happens every week. A steady day, a fixed place for the pen and an alarm together remove most of the decision-making. That consistency is more valuable than a perfect day, because the medicine's benefit comes from the regular weekly pattern rather than from any one injection.
07 · After a longer gap
After a longer gap
After a gap longer than a single missed dose, do not restart on your own: speak to your prescriber first, because the right dose to resume on is a clinical decision.
Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
That ladder exists because tolerability matters. If you have been away from the medicine for a long stretch, your prescriber will weigh up how long it has been, what dose you were on, how you felt on it and what your goals are. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better, so the sensible dose on restarting may not be the one you left on.1
Why you should not guess
It is tempting to assume you can pick up exactly where you left off. That assumption can lead to taking a dose your body is no longer accustomed to. The honest advice is to ask your prescriber.

Weight loss is gradual and judged by trend. Your prescriber reviews response, and if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.1 A few missed doses will not make or break that picture, but a pattern of gaps will blur it, so tell your prescriber honestly how consistent you have been.
08 · Other GLP-1 medicines
Other GLP-1 medicines
If you take a GLP-1 medicine other than Mounjaro, do not apply the Mounjaro missed-dose rule: each product has its own leaflet instructions. The name GLP-1 covers a family of medicines with different schedules, and a rule written for one weekly pen cannot safely be borrowed for another.
Wegovy, which contains semaglutide, is also a once-weekly pen. Its dose ladder runs through 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 Its missed-dose timing comes from the Wegovy leaflet. Liraglutide, supplied for example as Saxenda, is different again. It is supplied at a concentration of 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. A daily medicine has a very different rhythm, so a missed dose is handled differently, and the answer is in its own leaflet or from your prescriber.
Tablets
Some GLP-1 type medicines now come as tablets, including Wegovy tablets and Foundayo. Tablets are taken on a different schedule from weekly injections, and their instructions, including what to do if you forget, are set out in their own product information.2021 Check the leaflet in your box or ask your pharmacist.
The safe general habit
Whichever medicine you take, a few habits protect you. Keep the leaflet from your box. Know whether your dose is daily, weekly or on another pattern. Never double up to catch up. And if you are unsure, ask your prescriber or pharmacist before taking anything.
If you take insulin or a sulfonylurea alongside a GLP-1 medicine, doses of those may need adjusting because of the risk of low blood sugar, and a gap in your GLP-1 can change that balance. Talk to your diabetes team about how to manage it rather than adjusting on your own.

09 · Never double up
Never double up or stretch doses
The wrong response to a missed dose is to take extra or to change how the pen is used. The guidance is explicit that you should never take two doses within 3 days of each other, and there is no safe way to make up for lost time by injecting more at once.1
Altering doses outside the licence means unknown dosing accuracy and no safety data.1
Why these shortcuts appeal and why they backfire
After a miss, many people feel they have fallen behind and want to catch up. Others feel nervous about side effects and want to make the pen last longer. Both instincts come from a reasonable place, but both lead away from the tested design of the medicine.
If a dose feels too strong, the right answer is a conversation with your prescriber, not a home adjustment.
A word on side effects
Mild nausea, altered appetite and tiredness in the days after an injection are the commonest early effects and usually settle.1 If you restart after a gap and feel these again, that is not unusual. Anything that worries you, or any new symptom, should be discussed with your prescriber, and you can report suspected side effects through the MHRA Yellow Card scheme.19

A few practical habits make a missed dose less likely. Choose a fixed day, link it to something you already do, keep the pen where you will see it, and plan ahead for travel and deliveries.
Start with the day. There is no best time of day to inject, because Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1 Choose a time that your week naturally protects, and set a recurring alarm. If your week is unpredictable, consider moving the day, which you can do as long as at least 3 days (72 hours) have passed since your last dose.

Running out of medicine is a common cause of missed doses. Storage and travel
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1
For unopened pens that have been out of the fridge, check with the pharmacy before using one rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.
Build a backup plan
Decide in advance what you will do if you notice a miss: check the time, apply the rule, and contact your prescriber if you are unsure. Having that plan written down removes the panic. Getting back on track is mostly about calm, consistent routine rather than extra effort. Once you have applied the missed-dose rule, the goal is simply to return to your regular weekly pattern and give your body time to settle.
Weight loss on Mounjaro is gradual by design. The early weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1 A missed dose is one data point on a long line.
Keeping the bigger picture in view
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
Those averages come from people who stayed on a regular schedule, so they illustrate why consistency matters rather than predicting your own result. They are not a promise, and a late dose does not cancel them out.
Food, movement and the food-noise dip
If appetite has crept back after a miss, lean on the habits that support the medicine. Regular meals, protein with each meal, and planned snacks can help you ride out the stretch before your next dose takes effect. Increased physical activity also remains part of the plan, as the medicine is used alongside a reduced-calorie diet and increased physical activity.
Be kind to yourself about it. Appetite returning does not mean you have failed. It is a sign the medicine does what it is meant to do, and it will settle once you are back on schedule.
When to speak up
Tell your prescriber if misses are becoming regular, if you are struggling with side effects that tempt you to skip, or if your appetite is hard to manage. Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
