Can you miss 2 weeks of Mounjaro?
You can end up missing two weekly doses, and it happens to plenty of people, but it is not something to plan or to hide from your prescriber. The medicine is licensed as a once-weekly injection, and the missed-dose rule is written for single misses. When a gap runs to a fortnight, the safe path is to stop guessing and follow three steps: do not catch up, do not restart at full strength on your own, and speak to the prescriber who manages your dose.
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 For a two-week gap, the first missed dose is long past its window, so it is simply skipped. That rule does not tell you which dose to resume at, so your prescriber decides.

04 · Why the gap matters
Why a two-week gap matters
A gap matters because Mounjaro is built around steady weekly dosing. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. When doses stop arriving, those effects can fade, even though the medicine does not vanish immediately.
The reason it does not vanish is the half-life. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 That helps explain why a short gap is a different thing from stopping altogether. Some medicine is still in your body, but the level is falling each day, and that falling level is why appetite can start to return.
It also explains why a gap changes the restart conversation. The starting dose of 2.5mg is about letting your body adjust, not about weight loss itself.1 If your body has had a break from the medicine, your prescriber has to weigh how much of your tolerance you may have kept against the risk of stomach side effects on a higher dose.
A fortnight is not the same as a missed single dose, and it is not the same as a full stop. It sits in between, which is why treating it as a routine slip and carrying on at your usual strength is not something we would suggest without a conversation. The conversation is quick, it is part of the aftercare our prescribing team provides, and it protects you from an avoidable rough week.

05 · The missed-dose rule
The missed-dose rule, applied to two doses
The missed-dose rule is the anchor for everything else. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
Now apply it. Suppose your injection day is a Sunday and you miss two Sundays in a row. By the time the second Sunday has passed more than 4 days, both missed doses are outside their windows. The rule says skip them. It does not say you owe the medicine two extra doses, and it does not say you should inject twice in one week to make up the numbers. The safe reading is that missed doses are lost, not banked.
Now suppose you realise on the second missed Wednesday, which is within 4 days of that second scheduled dose. The label allows you to take that dose as soon as you can. Even then, because a fortnight has passed, we would still ask you to tell your prescriber first, because the question of dose strength is separate from the question of timing. The rule governs timing only.
The phrase to hold on to is the last one: never take two doses within 3 days of each other. That is the line that protects you from doubling up. If you are tempted to take a dose early to get back on track, check the gap since your last injection first. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That same spacing is your guardrail whichever day you land on.
Where a gap came from a missed delivery or a pen running out, think about how many doses the pen holds. Each KwikPen contains four weekly doses.13 Missed doses do not stay in the pen waiting for you. The pen still keeps to its own storage limits, so check the in-use window before you reach for a half-used pen.

06 · Restarting safely
Restarting safely after a gap
Restarting safely starts with a call or message to your prescriber, not a guess about your dose. Everyone starts Mounjaro on 2.5mg once a week. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 The ladder exists for tolerability, and a gap is exactly the kind of event that can change how well you tolerate a given strength.
The right restart dose is individual and your prescriber decides it.
What your prescriber will usually want to know is simple: how many doses you missed, what dose you were on, whether you were unwell, how your appetite and side effects have behaved, and whether you still have a pen in date and in the right storage conditions. Having those facts ready makes the conversation faster. If you are an existing customer, repeat orders re-check suitability and nothing ships automatically, so the review is built into reordering.
Equally important is what not to do. Do not restart at a higher strength because you feel behind. Do not inject twice in a week to catch up. Do not use a pen that has been frozen or that has run past its in-use window.
Expect the first few days after restarting to feel like the first days on any new dose. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 If symptoms are severe or do not settle, contact your prescriber, and consider a Yellow Card report.19
Finally, remember there is no prize for speed. Higher is not automatically better, and your prescriber will guide the right pace.1 A calm restart that you tolerate is worth more than a rushed one you abandon.
07 · Appetite and weight
Appetite and weight during a gap
During a fortnight without injections, appetite can return gradually rather than all at once, because the medicine clears slowly. Since tirzepatide takes roughly 4–5 weeks to clear the system, you are not suddenly back to your pre-treatment baseline.1
Weight loss on Mounjaro is gradual by design, and trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1 A small bump after a fortnight is a data point, not a verdict.
It also helps to keep the headline trial in perspective. In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Those results came from sustained treatment, which is the reason consistency matters and why a gap is worth flagging.
Plateaus are also normal and can follow a gap. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 Within the licence, the responses are: review food intake honestly, protect protein and activity, check the basics such as sleep and alcohol, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.
During the gap itself, the most useful habits are practical. Keep meals regular, keep protein in each meal, stay active, and weigh yourself on a consistent schedule so you can see the trend rather than react to a single day. If hunger feels louder than usual, that is useful information to pass to your prescriber rather than something to push through silently.
08 · Planned breaks and travel
Holidays, illness and planned breaks
Most holidays do not require a break at all. You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription.13 In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.
That answers a common worry: you do not usually need to stop Mounjaro while on holiday. If you are away on your injection day, you can inject there, at any time of day, with or without food. If your day falls on travel day and the timing is awkward, remember you can change your weekly injection day as long as there are at least 3 days between two doses.1 Plan the shift before you leave rather than skipping and hoping.
Storage is where most holiday problems start. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.
If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.13
Illness is the other usual cause of a gap. If you have been vomiting, unable to eat, or generally unwell, tell your prescriber rather than deciding alone whether to inject. They can advise on holding a dose or stepping down within the licence, which is a normal part of managing treatment, not a failure.
If a break is deliberate, for example around surgery or a medical appointment, raise it with your prescriber early so the restart plan is agreed before the gap, not after. A planned pause with a plan is a very different thing from an accidental one. 09 · Injection day and routine
Rebuilding your injection routine
The easiest way to avoid another gap is to make the weekly injection automatic. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day: Mounjaro works the same morning or evening. What matters is the same day each week. Pick a day and time you will remember, and many people choose a weekend morning so early side effects fall on days off.
Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 If you have not injected for a while, take a moment to re-read the pen's instructions and the patient leaflet before you start again, rather than relying on memory.
Practical tools help more than willpower. Set a repeating phone alarm on your injection day. Keep the pen in a fixed fridge spot where you see it when you get the milk. Think about delivery timing as well. A common cause of a gap is running out of pen before the next order arrives. 10 · Mistakes to avoid
Mistakes to avoid after a missed fortnight
The commonest mistake is trying to make up lost ground. It feels logical to inject extra, or to jump back to a higher strength, but the label says the opposite. Never take two doses within 3 days of each other.1
Another mistake is staying silent. Many people feel embarrassed about a gap and restart without telling anyone. Your prescriber has heard it all before, and the information they need is simple. Telling them is also what lets them decide whether to continue, adjust or step down. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 A gap can affect that picture, so context matters.
For storage, use the UK figures: in-use pens up to 30 days not above 30°C, unopened pens in the fridge until the printed expiry. For anything else, ask the pharmacy.
Another mistake is judging the gap by one weigh-in. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.13 If the scale is up a little after a fortnight, resist the urge to compensate with extreme dieting or skipped meals. Steady food, protein and activity are the foundation the medicine works alongside, not instead of.

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

If a gap was caused by access rather than choice, it helps to know your options. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
