Can you miss a week on Mounjaro?
Yes, it can happen, and one missed week is not usually a crisis. But skipping a week on purpose is not recommended. Mounjaro (tirzepatide) is a once-weekly injectable prescription medicine1, and the weekly rhythm is what the dosing schedule is built on.
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.

What to do if you have already missed a week
Start by reading the missed-dose section of your leaflet. If more than 4 days have passed, skip the missed dose and take the next one on your normal day.

04 · Why the weekly schedule matters
Why the weekly schedule matters
The schedule exists because of how the medicine behaves in the body. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
That has two practical consequences. First, a single missed week does not mean the medicine vanishes. Levels fall gradually, not abruptly. Second, a weekly pen only works as intended when the doses arrive at regular intervals, which is why repeated gaps matter more than one slip.
The pen itself comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Your prescriber chooses the strength. You should not change it yourself because you missed a dose.

05 · Appetite and weight after a gap
What happens to appetite and weight after a missed week
You may notice appetite returning a little as the medicine level drifts down. This is a reasonable expectation rather than a certainty, because tirzepatide helps regulate appetite and slows how quickly the stomach empties.1 People differ widely, and some notice very little.
Weight is the same story. Weight loss on Mounjaro is gradual by design, and trials measured results over 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.2 A single missed week will rarely show up as a meaningful change on the scales. Water weight moves around day to day, and early loss includes water weight, so the trend over weeks matters more than any single weigh-in.
What a gap does change is momentum. If you were already finding a stretch hard, a pause can make it feel harder to get going again. That is a good reason to be honest with yourself and your prescriber rather than hide the lapse.
Do not respond by cutting food drastically. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Return to your usual routine, keep protein and activity steady, and let the weekly schedule do its job.
06 · Side effects when you restart
Side effects and restarting after a break
After a gap, the first injection back can feel like a mini restart. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If you notice them again after a missed week, that is consistent with the early-treatment pattern, not necessarily a new problem.
Which strength to restart on after a longer break is a prescriber decision. That depends on your current dose, how long you have been away and how you tolerated treatment. Your prescriber will decide, and that is why we say to ask rather than guess. A short gap and a gap of many weeks are very different conversations.
If you ever feel unwell in a way that worries you, speak to your prescriber. For severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or swelling of the face or throat and difficulty breathing, get medical help straight away. You can also report suspected side effects through the MHRA Yellow Card scheme.19

07 · Planned breaks and holidays
Holidays, travel and planned breaks
Planning to skip Mounjaro for a trip is a common thought, and the better route is to plan around the trip rather than through it. Your injection day does not have to be a fixed enemy of your calendar. Speak to your prescriber before you go, and ask how to handle the injection while you are away.
Travel preparation is mostly practical. Keep pens stored as the leaflet instructs: store in the fridge until first use, and unopened pens keep until the printed expiry date.13 Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Pack your sharps and your needles in hand luggage with your pharmacy label.

08 · Every other week and stretching doses
Can you take Mounjaro every other week?
No, not as a licensed schedule. Mounjaro is licensed as a once-weekly injection1, and we do not support stretching the interval, taking half-measures or spacing doses to make a pen last longer.
If cost or side effects are behind the thought, the right answer is a conversation. That is part of ongoing monthly reviews.
A missed week is a lapse. A deliberate pattern of skipping is a change in treatment, and any change should be made with your prescriber, not around them.
09 · Withdrawal and stopping
Does Mounjaro cause withdrawal symptoms?
Withdrawal symptoms are not listed as a side effect in the Mounjaro patient leaflet; what does change when treatment stops is the medicine's effect. As the last dose clears, which takes roughly four to five weeks, its influence on appetite and stomach emptying fades.1
If you notice anything new after a gap, discuss it with your prescriber, and report suspected side effects via Yellow Card.19 Do not assume a symptom is caused by the missed dose, and do not assume it is not.
People who stop often find hunger returns as the medicine wears off. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
10 · Staying on track
How to avoid missing a week
Most missed doses come from routine, not intent. A few habits help. Pick a fixed injection day that suits your week, not your best week. Set a recurring phone reminder with a backup. Keep the pen where you will see it, within the storage instructions.
Order before you run low. Same-day dispatch applies when you order by 3pm, and free next-day tracked delivery means you are rarely stuck. Repeat orders re-check suitability, and there is no auto-shipping, so you decide when to reorder.
Use ongoing reviews well. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Missed doses are useful information in that review, so be candid.
If you are on a plateau, a missed week is not the cause by default. Plateaus of a few weeks are normal and expected as the body adapts to a lower weight, and a plateau is not a reason to double doses or add unlicensed products.1
11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
12 · NHS and private routes
If you get Mounjaro through the NHS
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.46
Whether you are treated on the NHS or privately, the missed-dose advice is the same: use the leaflet and ask your prescriber. NHS patients should contact their GP or weight-management service. Private patients can contact their prescribing team.
