Physically, yes: nothing dangerous happens the moment a weekly dose is late or skipped. But a planned skip is a poor trade for most people, because the usual reasons for it (storage, flights, hot weather) are already covered by the pen's design.

Mounjaro is a once-weekly injection, and your prescriber has set your dose and schedule for you. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 A holiday is exactly when that routine is under strain, so it is also when the medicine's appetite support is most useful.

If you are on holiday and a dose falls due, you can usually inject it where you are. That is the plan most prescribers prefer to a deliberate gap.

02 · What to do

Skip, shift or inject abroad: which decision applies to you?

Your situation falls into one of three groups, and the right step differs for each.

  • You are worried about carrying the pen. You do not need to skip. Pens go in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription.1 See taking Mounjaro on holiday for the practical checklist.
  • You are worried about eating and drinking more. That is a diet question, not a dosing one. Skipping may bring hunger back sooner, which works against you.
  • You have already missed or want to move a dose. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time; if more than 4 days have passed, skip it and take the next one on your normal day. Never take two doses within 3 days of each other.

03 · Being honest

What dose to restart on is a clinical decision for your prescriber, and depends on your dose, your recent tolerance and how long the gap is.

04 · How long it stays

How long Mounjaro stays in your system

Mounjaro does not switch off the day a dose is late. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

That slow clearance is why a single skipped week is not an emergency. It is also why the effect fades gradually rather than abruptly. Levels fall over the gap, and the appetite effect that depends on them eases with them.

The mechanism explains what you may notice. Tirzepatide activates the GIP and GLP-1 receptors, which help regulate appetite and slow how quickly the stomach empties.1 Most people feel fuller sooner and less hungry between meals. With less medicine on board, that fullness can ease, and hunger or cravings may return before you have injected again.

05 · Keeping pens usable abroad

Keeping your pen usable abroad

The storage rules are the reason most people do not need a break. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which suits short trips, after which it should be discarded.1 Keep the cap on to protect it from light.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing. A cool bag with the prescription label, or a copy of your prescription, is the sensible set-up. For hot climates, use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1

Unopened pens belong in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use.1 If in doubt, contact the pharmacy before injecting.

06 · Appetite and weight on a break

Appetite and weight if you do take a break

A short break can mean a hungrier week, and that can show on the scales.

Weight loss on Mounjaro is gradual by design, and trials measured results at 72 weeks, so think in months, not days.2 Judge progress by trend rather than individual weigh-ins. Early loss also includes water weight, so a rise after a holiday may be partly fluid and partly food.

Plateaus of a few weeks are normal and expected as the body adapts. If you stall after a trip, the licensed responses are to review food intake honestly, protect protein and activity, check sleep and alcohol, and ask your prescriber whether a dose step-up is appropriate. A plateau is not a reason to double doses or add unlicensed products.2

Staying on track matters because results in trials came with continued treatment. In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, and individual results vary.2

07 · Changing the dosing gap

Changing your injection day and longer breaks

Mounjaro is licensed as a once-weekly injection. You can change your injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.

If your holiday leaves you needing a different timing, for example because a long flight lands on your injection day, tell your prescriber in advance. They can advise on the safest day to inject. Never take two doses within 3 days of each other.

A longer break, such as a couple of weeks, is a different decision from a single skipped dose. The medicine takes roughly four to five weeks to clear, so the effect may fade while you are away. Your response is reviewed by your prescriber anyway. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.2

08 · Stopping and rebound

Stopping, withdrawal and rebound weight gain

Stopping Mounjaro is a medical decision, and a holiday on its own is not a reason to do it.

Mounjaro helps you feel fuller and eat less. When you stop, that support goes, so appetite can return. If eating returns to what it was before treatment, weight can climb back. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. That is why the diet and activity habits you build during treatment matter, since they are the part you keep.

If you feel unwell after stopping or skipping doses, speak to your prescriber, and report any suspected side effect through the Yellow Card scheme.19 If you have severe, persistent stomach pain, get medical help straight away.

There is also a broader point about expectations. Weight management on Mounjaro is health management, not just appearance: in trials, weight loss came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures.2 Those are trial averages, not promises, but they are a reason to protect continuity.

09 · Planning the trip

A practical plan for your holiday

Plan around your normal injection day rather than around the destination. Work out whether a dose falls before, during or after the trip, and decide in advance where you will inject it.

  • Before you go: check your supply covers the whole trip with a margin, and carry the prescription label or a copy.
  • Packing: pens, needles and a sharps solution in hand luggage; an insulated pouch for hot places.
  • On arrival: if you are staying somewhere with a fridge, keep unopened pens there. An in-use pen can stay out of the fridge not above 30°C for up to 30 days.1
  • Eating: expect to be full sooner than usual and plan meals around that. Alcohol and rich food can make nausea more noticeable.
  • If something goes wrong: a lost, frozen or heat-damaged pen is a call to the pharmacy, not a reason to guess.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
