Once you have dealt with the missed dose, the useful question is why it was missed. Most missed doses come from a changed routine: a holiday, a late shift, a pen left in the wrong place, or an injection day that clashes with your week.
If your injection day is the problem, you can change it. If the problem is memory, anchor the injection to something you already do weekly. If side effects make you dread the day, tell your prescriber rather than quietly skipping, because they can hold your current dose longer before stepping up. Pick a day and time you will remember and stick to it; there is no best time of day.1

Read the first part carefully. The clock starts at the scheduled time of your injection, not at the moment you noticed. So if your injection day is a Sunday morning and you remember on a Wednesday, you are still inside the window and you inject straight away. After that you do not slide your schedule across to Wednesday; you carry on with your usual day. That detail catches people out, because it means your next injection will be nearer than a full week away, which is exactly why the spacing rule matters.
The second part is a hard stop. Once the window has passed, the missed dose is gone. You wait for your usual day and take the normal dose.
Why the rule is built this way
Mounjaro is designed to be taken at an even weekly rhythm. The active ingredient, tirzepatide, has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

In that situation the safe approach is to ask your pharmacy or prescriber how to handle it, rather than guessing. The rule is clear that two doses must never fall within 3 days of each other.
You remember after the window has closed
More than 4 days have passed, so you skip the missed dose entirely and take the next one on your normal day. This can feel like wasting a dose, but it is the correct action. The pen will still have its remaining doses for the coming weeks.
You are not sure whether you injected
This is more common than people admit, particularly when you inject at the same time as other routines. Do not inject again on a hunch. You can inject at any time of day, with or without food, so a late-evening injection in a hotel is fine. If the day genuinely cannot work, you can move your day as long as at least 3 days (72 hours) have passed since your last dose.

Why you must never take extra doses to catch up
Taking an extra dose to make up for a missed one is the single most common mistake, and the rule is explicit: never take two doses within 3 days of each other.13 It helps to understand why, because the instinct to catch up is strong, especially when you are worried about your progress.
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1
The medicine already lingers

Start from your last injection date. Count forward to confirm at least the minimum spacing has passed before you inject on the new day. Then keep the new day consistently, just as you did the old one. Moving your day earlier is where people slip up, because it shortens the gap; check the spacing rule each time, and if in doubt ask your pharmacy.
You do not need to wait for a missed dose to change day. If you know in advance that a certain weekday will be a problem, for instance a regular work trip, make the change at a convenient point rather than after the dose is already late.

Appetite and cravings
If you feel hungrier for a few days, plan for it rather than fight it. Keep to your usual meal structure, lean on protein and fibre-rich foods you already use, and avoid reacting by overeating. The medicine is used alongside a reduced-calorie diet and increased physical activity, not instead of them, so your eating habits are still doing much of the work.2 A slip in the injection does not cancel the habits you have built.
Nausea when you restart
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 Eating smaller meals and avoiding very rich or greasy food around injection day can help. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.
Blood sugar for people with diabetes
Keep monitoring as you normally do and follow the advice from your diabetes team. If you also take insulin or sulfonylureas, doses may need adjusting because of the low blood sugar risk, so tell your prescriber about any change in your pattern of injections and do not adjust other medicines yourself.
Does one missed dose undo your progress?
Almost certainly not. Weight loss on Mounjaro is gradual by design, and trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.13 A small wobble on the scale in the days after a late dose is normal noise rather than a verdict on your treatment.
For context on what the treatment can achieve over the long term, SURMOUNT-1 reported that people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages — individual results vary.2 They reflect consistent treatment over a long period, which is why getting back to routine matters more than any one injection.

08 · Pen mechanics and storage
Pen, dial and storage problems that cause missed doses
Some missed doses are really pen problems: a pen that will not dial, a pen left somewhere warm, or confusion about how much is left. Knowing the mechanics removes much of that worry.
How the pen works
Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Each KwikPen contains four weekly doses.1
That last point matters for missed doses. If you have lost track of how many injections you have taken from a pen, the pen itself will tell you: when it cannot dial a full dose, it is finished. Start a new pen for your next dose.
Where to inject
Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 If you are injecting late, the same site rules apply.
Storing your pens
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use — useful for short trips — and must then be thrown away. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13 If a pen has been in a hot car or left on a windowsill, treat it with caution and ask your pharmacy.
Travel and missed doses
Travel disrupts routine and leads to missed doses. You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.13
Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.13

Why a missed dose is not a reason to change dose
If a missed dose has left you feeling the medicine more strongly on return, the answer is a conversation, not a home adjustment. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1
If you have missed a lot of doses and are worried about restarting at your previous strength, say so. Your prescriber will advise what is appropriate for you. Do not choose a lower or higher dose yourself.
Other things to avoid
- Taking an extra dose to catch up, which risks breaking the spacing rule.
- Using a pen that has been frozen, or one stored outside its recommended conditions.

Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Missed doses, the NHS and private treatment
The missed dose rule is the same wherever your Mounjaro comes from, because it is part of the licensed product information. What differs is who you speak to when something goes wrong, and how easily you can reorder.
NHS route
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).
