What you are actually comparing: one medicine, six strengths

Every genuine Mounjaro pen is the same medicine, so a comparison site is really comparing service, price and safety rather than the product itself. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.¹

It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ That matters for comparison because there is no 'budget' version of tirzepatide. If a listing looks like Mounjaro but is not a KwikPen supplied through a regulated route, it is not a like-for-like comparison, and you should treat it with caution.

Because the pens differ only by strength, the fair way to compare is dose by dose. Higher strengths cost more, and a comparison page that shows only the starting dose tells you very little about what you will pay once you reach your maintenance strength.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.¹ In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).¹ For a comparison reader, the point is that you are choosing between genuinely different medicines, not rebranded versions of one product. That is why a price comparison should never be the only step before you start treatment.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹ This is worth knowing if you are comparing providers with the idea of switching: stopping one supplier and starting another does not reset your body's exposure overnight, and the choice of dose at your new provider should be made by a prescriber who knows your history.

The medicine is used alongside a reduced-calorie diet and increased physical activity, not instead of them. A provider that sells Mounjaro as a standalone fix is giving you a reason to look elsewhere.

Finally, remember that the pen is a once-weekly injection. Prices differ because pharmacies buy at different terms, run different service models and choose different margins. The one fixed point is the manufacturer's list price.

Pharmacy sell prices vary because of confidential discounts, so no one can tell you from the list price alone what a given pharmacy charges. The reasons a price may look low are worth understanding. A provider might show a low starting-dose price and charge more for higher strengths. A consultation or delivery fee might sit outside the figure on the product page. A discount might apply only to the first order. Or a price might simply be out of date. A like-for-like comparison asks each of these questions in turn.

06 · Checking a provider is legitimate

How to check a Mounjaro provider is legitimate before price matters

Check the provider is registered before you compare anything else, because a low price from an unregulated seller is not a saving. Every UK pharmacy that supplies prescription medicines must appear on the GPhC public register, and you can search it using the registration number.¹² Ours is GPhC 9012878, and you can look it up on the GPhC register yourself.

Mounjaro is a prescription-only medicine. A legitimate provider supplies it only after an online consultation reviewed by a GPhC-registered prescriber. Anyone offering to supply without a consultation or prescription is a warning sign, however attractive the price.

The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting side effects and suspect products.⁷ If you suspect a pen is not genuine, do not use it, and report it. Signs that should make you stop include packaging that does not match the patient information leaflet, a seller who cannot name their supply route, and social-media listings with no pharmacy behind them.

07 · Mounjaro vs Wegovy evidence

Comparing medicines: Mounjaro versus Wegovy

If you are comparing options, SURMOUNT-5 is the first large head-to-head trial between the two licensed weight-management injections. Adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).¹⁵ Both are effective, licensed treatments; which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone.

The detail is worth reading. In SURMOUNT-5, 751 adults took tirzepatide or semaglutide 2.4mg for 72 weeks. At least 10% of body weight was lost by 81.6% on tirzepatide versus 60.5% on semaglutide; at least 15% by 64.6% versus 40.1%; at least 20% by 48.4% versus 27.3%; and at least 25% by 31.6% versus 16.1%. Average loss was 22.8 kg versus 15.0 kg.¹⁵ Individual results vary, and these are trial averages, not predictions for you.

For Wegovy's own evidence, STEP 1 found that adults on semaglutide 2.4mg lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.⁹ In STEP 1, 86.4% lost at least 5% of body weight (versus 31.5% on placebo), 69.1% lost at least 10% (versus 12.0%) and 50.5% lost at least 15% (versus 4.9%).⁹ STEP 1 is a separate trial against placebo and should not be set beside SURMOUNT-5's numbers as if it were head to head.

Tolerability is part of any fair comparison. In STEP 1, gastrointestinal side effects were reported by 74.2% of people on semaglutide 2.4mg, compared with 47.9% on placebo, and 7.0% stopped treatment because of side effects, compared with 3.1% on placebo.⁹ Talk to the prescriber about how you tolerate any GLP-1 medicine.

08 · Efficacy by dose

What the trial evidence says at each dose

The main trial for Mounjaro in weight management is SURMOUNT-1, and its figures are the reference point when providers describe what the medicine does. In this trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.²

These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.² A provider that presents an average as a promise is not being straight with you.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.²

To make the percentages concrete: for someone starting at 100 kg, 20.9% is about 21 kg, and 15% is about 15 kg. That is simple arithmetic on the trial averages and not a forecast for any individual, and it says nothing about how long it takes you to get there.

Why does this belong on a comparison page? Because it shows that the dose you settle on shapes both results and cost. Higher strengths have higher pen prices, but your prescriber, not the price list, chooses your dose based on your response and tolerance. Do not choose a provider because it makes a higher dose easy to buy, and do not choose a dose to chase a trial number.

A responsible provider reviews your progress, adjusts your dose through monthly reviews and follow-up assessments, and lets you stay on a dose that works for you. That ongoing clinical oversight is part of what you should expect and compare, alongside the pen price.

09 · NHS versus private

NHS or private: how the two routes compare

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

The comparison, then, is not 'NHS versus a comparison site' but which route fits your situation and timeline. If you are in a highest-need group, speaking to your GP about NHS access is a sensible first step. If you are not, or you would rather not wait, a regulated private pharmacy is the alternative, and that is where price comparison starts to matter.

The practical differences are these. On the NHS, access follows the phased criteria, and prescribing follows NICE guidance. Privately, a GPhC-registered prescriber reviews your consultation against the licensed criteria and you pay for the pen. The NHS prescription charge in England is £9.90 per item where it applies, but whether a weight-management prescription is available to you depends on the phased rollout and your local service.

Neither route removes the need for the lifestyle side of treatment. Both expect Mounjaro to be used alongside a reduced-calorie diet and increased physical activity. And neither is a reason to accept an unregulated source: if you are not eligible yet for the NHS, an unregulated seller is not the next best thing.

If you are moving from one route to the other, tell your prescriber. Treatment history matters for choosing a safe dose, and your prescriber needs to know what you have already taken. A private pharmacy with proper aftercare will ask for this rather than skipping the question.

10 · Switching and alternatives

Switching providers, switching medicines and cheaper alternatives

Switching between providers for the same medicine is straightforward in principle, but switching between medicines is a prescriber decision. There is no licensed direct conversion between semaglutide and tirzepatide. Switching is done under prescriber supervision: typically the previous medicine is stopped, a washout interval observed, and Mounjaro started at a dose your prescriber judges appropriate, often the 2.5mg starting dose, sometimes adjusted based on your history. Never run two GLP-1 medicines together.

Mounjaro must not be combined with other GLP-1 medicines, unlicensed 'research' peptides (such as retatrutide, which has no UK licence), or compounded copies. These combinations have no safety data and no regulated supply chain. If something promises faster results outside a licensed pathway, that is the warning sign.

Ozempic is semaglutide licensed for type 2 diabetes, not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg). So 'Mounjaro vs Ozempic' for weight loss is really Mounjaro vs Wegovy, and a regulated UK pharmacy will not supply Ozempic for weight loss.

What about tablets and cheaper alternatives? There is no tablet form of Mounjaro, as tirzepatide is injection-only in the UK. Two GLP-1 weight-loss tablets are now licensed in the UK: Wegovy tablets (oral semaglutide, made by Novo Nordisk), approved by the MHRA in June 2026, and Foundayo (orforglipron, made by Eli Lilly), approved in August 2026. They are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, and both are prescription-only. Other licensed options work differently. Orlistat is a tablet that blocks some dietary fat absorption and produces smaller average losses, and Wegovy and Saxenda (liraglutide) are other licensed injectables. 'Natural GLP-1' supplements such as berberine are low-cost supplements with no clinical oversight; they are not medicines, are not regulated for this use and have no comparable trial evidence. Their low cost is a reason for caution, not a saving.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
