What you are paying for with a weight loss injection

When people ask how much weight loss injections cost, they usually mean Mounjaro, the once-weekly injection whose active ingredient is tirzepatide. It is made by Eli Lilly and licensed in the UK for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

That structure explains why prices are quoted per pen. Each KwikPen contains four weekly doses, so one pen is one month of treatment at a single dose.¹³ If you read a forum thread that quotes a single figure without a dose, treat it with caution.

Why the dose you are on drives the cost

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least four weeks the dose increases to 5mg, and it can then step up by 2.5mg at minimum four-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹

For your budget, that creates three phases. In the first month you pay the lowest price, for the 2.5mg pen. Then the dose climbs and each pen costs more, so your costs rise during the first few months. After that you reach a maintenance dose and the monthly price levels out at that dose's price. Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace.

05 · NHS versus private

NHS versus private: what each route costs

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially people with a BMI of 40 or above and four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

For cost, this matters in a simple way. If you qualify and your local NHS service can offer it, the medicine itself costs you the standard NHS prescription charge, which is £9.90 per item in England. That is far below private prices. The catch is access: the NHS decides who is seen first, and many people with a BMI under the early thresholds are not yet eligible. It is worth asking your GP what is available locally.⁵

Private treatment is faster to start and is open to anyone who meets the licensed criteria after assessment, but you pay the monthly pen price. It is a trade-off between time and money, and neither route is wrong. Some people use private treatment while they wait for NHS access. Check with your GP if you plan to move between routes, so your records and dose history are clear.

Wegovy follows its own pattern. NHS access to Wegovy runs through specialist weight-management services, so ask your GP. Privately, your first Wegovy pen costs £99.99 as a first-order price.

Do not assume a lower price means a lower-quality route, or a higher one means better care. What to compare is what each route includes: an assessment by a prescriber, a licensed medicine from a verified supply chain, a clear dose plan and follow-up. 06 · Eligibility and who should not

Who can have it, and who should not take it

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹

That is the answer to "can I get it?", and it is also an important point about cost. Paying for an injection you are not eligible for is not a bargain, and any provider that supplies without assessing you should be avoided. A prescriber will assess your BMI, medical history and current medicines before deciding whether Mounjaro is suitable for you. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

The honest answer is that this decision belongs to the prescriber, who looks at your full history. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Stop it at least one month before trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.¹³ Tell your prescriber about every medicine you take, any conditions you have, and anything that changes while you are on treatment.

Two practical points are worth knowing. If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so say so in your consultation. And Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.⁷ The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ Neither is a reason to avoid treatment on its own, but both are reasons to be open about your history.

07 · What results to expect

What results to expect for the money

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.²

To make that concrete, for someone starting at 100 kg, 20.9% is about 21 kg and 15% is about 15 kg. Those are straight arithmetic from the trial averages, not predictions for you. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.² Trial averages do not predict your own result.

People often ask how much weight you can lose in 3 months. What the evidence supports is a gradual pattern. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Early experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.

How does this relate to cost? It means that the first month, at £169.99 standard for the starter pen, is mostly about adjustment, and that your spend rises as the dose goes up and results build. If you step up after 4 weeks on each dose, your first four pens cost £879.96 at standard prices before discounts, so it is sensible to plan beyond the first pen rather than judging value after one.

08 · Cheap pens and false economy

Cheap pens, fakes and false economies

The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.¹²

A genuine UK pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been kept out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.¹³

09 · Using and storing your pen

Using and storing your pen so none is wasted

Using your pen correctly protects your investment, because a pen that is stored badly or dosed wrongly cannot be refunded once dispensed. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.¹

There is no best time of day. Mounjaro works the same morning or evening, and what matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose and take the next one on your normal day. Never take two doses within 3 days of each other. You can change your injection day as long as at least 3 days (72 hours) have passed since your last dose.¹

Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the cap on to protect it from light, and never use a pen that has been frozen.¹

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. For hot climates use an insulated pouch. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.¹³

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
