The free online consultation is reviewed by a GPhC-registered prescriber, not a scoring script. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. 05 · Types of injection

Which weight loss injections are priced monthly?

Several injectable medicines are used for weight management in the UK, and they are priced on different cycles. Mounjaro is a once-weekly injection containing tirzepatide, licensed in the UK for weight management in adults and for type 2 diabetes.¹ It comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. One pen contains four weekly doses, which is why pens are priced per month.

Wegovy (semaglutide) is also a once-weekly injection. Its pens run 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17. Liraglutide, sold as Saxenda and in products such as Erly, is a daily injection supplied at 6mg per millilitre, so its cost is built differently from the weekly pens. Why does this matter for cost? A daily injection and a weekly injection cannot be compared by the price on the box alone. A month of Mounjaro is four doses in one pen. A month of a daily product is a longer run of doses. When you compare quotes, check what period of treatment the price covers and whether the dose you will really need is the one being advertised.

Mechanism: why tirzepatide stands apart

Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin.¹ Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide, the active ingredient in Wegovy and Ozempic.

Is one medicine better value?

SURMOUNT-5 is the first large head-to-head trial directly comparing tirzepatide with semaglutide in people with obesity.¹⁵ There is no single best injection on cost: the right medicine is the one a prescriber judges suitable for your health, BMI and goals, at a dose you tolerate. 06 · Dose and price

Why the dose sets your monthly bill

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.¹ After at least four weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of four-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.

That gives a clear pattern for budgeting. The 2.5mg pen costs £169.99, the 5mg pen £189.99, the 7.5mg pen £244.99, the 10mg pen £274.99, the 12.5mg pen £289.99 and the 15mg pen £299.99. These are standard prices; your first order is £40 off, and every later order £25 off the standard price.

A pharmacy advertising a very low starting figure may be showing only the 2.5mg pen, which most people use for a single month.

Plan for the dose you will probably need

Because the starting pen is an adjustment dose, the 2.5mg price is a poor guide to long-term spend. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, so these are the prices to plan around. If you are already on a higher dose with another provider, you do not restart at 2.5mg just because you switch; your prescriber reviews your history and decides the right dose. Higher is not automatically better

Treatment response is reviewed too: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.¹

What a year of weight loss injections costs

A first year of Mounjaro costs £2,399.88 at standard prices if you settle at 5mg, and up to £3,279.88 if you settle at 15mg. A year is 52 weeks, which means 13 pens. If you step up after 4 weeks on each of 2.5mg, 5mg, 7.5mg and 10mg, your first four pens cost £879.96. These totals are before discounts: a new customer's first order is £40 less, and every later order is £25 less.

Why plan by the year rather than the month? A person who steps through to 10mg will pay more per month in the second half of the year than in the first few weeks. Thinking in a year helps you decide whether the spend is sustainable before you begin.

What the treatment is for, and what you may expect

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.¹ In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages, and individual results vary.

As a worked example only: for someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg. Trial averages do not predict your own result, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Think in months, not days

Trials measured results at 72 weeks. Judge progress by trend rather than single weigh-ins, since early loss includes water weight.¹ The year-cost figures above are therefore a planning tool, not a promise of any result.

What happens if you stop?

If you decide to stop, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹ Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

08 · NHS and free options

Are weight loss injections free on the NHS?

Weight loss injections can be provided through the NHS, but access is being phased in over several years, so they are not free for everyone who wants them. Mounjaro is available on the NHS. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

What an NHS prescription costs

In England, an NHS prescription charge of £9.90 per item applies unless you are exempt. Whether a weight loss injection is prescribed to you on the NHS depends on local commissioning and whether you fall into a current rollout group, so your GP is the person to ask.⁵

When private treatment makes sense

Private treatment suits people who meet the licensed criteria but are not yet in an NHS group, or who prefer to start now. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹ A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.

Moving between NHS and private

Nothing stops you using private treatment while you wait to learn whether you qualify for NHS support. Tell any clinician involved about all medicines you take, so your records stay accurate. If you later become eligible for NHS treatment, speak to your GP about how to transition.

Other "free" routes

Free lifestyle programmes through the NHS or local services can run alongside any injection, and Mounjaro is meant to be used with diet and activity changes in any case. How to reduce the monthly cost safely

Do not stretch pens

Mounjaro is licensed as a once-weekly medicine; its blood levels are designed around a 7-day cycle. Talk to your clinical team rather than self-adjusting.

Microdosing is not a saving

Altering doses outside the licence means unknown dosing accuracy and no safety data.

Cheap supplements are not equivalent

Low-cost supplements marketed as natural alternatives are available cheaply without assessment and come with no clinical oversight; they are not medicines and have no comparable trial evidence. They are not a like-for-like swap for a licensed prescription medicine.

Practical ways to keep spending steady

• Order on time so you avoid gaps and restarts at a lower dose.
• Use monthly reviews to confirm the dose you are on is still doing its job.

Missing or changing doses carries its own rules: if you miss a dose, take it as soon as you can within four days (96 hours) of the scheduled time; if more than four days have passed, skip it and take the next on your normal day, and never take two doses within three days of each other.¹

10 · Safety and counterfeits

Why a very low price can be a danger sign

An unusually low price for a weight loss injection can signal a counterfeit, and the MHRA has seized thousands of fake weight-loss pens in the UK. It has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.

Checking a pen is genuine and still good

A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.

Warning signs when you shop

• A seller offering to supply without a consultation or prescription.
• Sales through social media, messaging apps or salons.
• Prices that sit far below every verified pharmacy.
• Pens with no batch number or expiry date, or with damaged packaging.

Report problems

If you have a side effect or suspect a pen is faulty or falsified, report it through the MHRA Yellow Card scheme, and speak to your prescriber about any new symptom. Saving on the monthly cost is never worth a hospital visit, and the cost of a counterfeit is usually higher than it looks.

11 · Storage and duration

How long does a weight loss injection last?

A Mounjaro injection works across a week, and one pen lasts four weeks. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Each KwikPen contains four weekly doses.¹ Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system.

This is useful for cost: a month's supply is a single pen, so you know exactly when to reorder. There is no best time of day to inject. Mounjaro works the same morning or evening. What matters is the same day each week, and many people choose a weekend morning so early side effects fall on days off. You can change your injection day if at least three days (72 hours) have passed since your last dose.

Using the pen

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window. Follow the pen's instructions for how to deliver the injection.

Storing pens so none is wasted

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen. Waste from poor storage is an avoidable cost.

Travelling

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. For hot climates use an insulated pouch, and check destination rules for travelling with prescription injectables before you fly. Unopened pens should stay in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.

What the first weeks feel like

Some people notice appetite change and early loss in week one; others feel little until a higher dose. Both are normal. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹

13 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
