How to compare weight loss injection prices fairly

Compare the same dose, the same unit and the same point in your journey. A first-order price is not comparable with a repeat-order price, and one pen is four weeks of treatment, not one dose. What weight loss injections are and how they work

Weight loss injections are prescription medicines that you inject under the skin to help manage body weight, alongside a reduced-calorie diet and more physical activity. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.¹ It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.¹ In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).

Each KwikPen contains four weekly doses, which is why one pen is priced as one month of treatment. The medicine is injected once a week on the same day, at any time of day, with or without food.¹ Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose and follow the pen's instructions for the injection itself.

Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.¹

Dose ladder: how the price changes as your dose rises

Everyone starts Mounjaro on 2.5mg once a week, and the price you pay follows the dose you are prescribed. The starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹ Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.¹

07 · First year cost planning

What a first year of weight loss injections costs

A first year of Mounjaro is 13 pens, and at standard prices it costs £2,399.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,279.88 if you step up through every dose to 15mg. Those totals include the dose steps you pass through on the way to your maintenance dose. They are standard prices before any discount: a new customer's first order is £40 less and every later order is £25 less.

If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £879.96 at standard prices. The figure assumes you move up at the shortest interval the licence allows, which is at least 4 weeks on each dose.¹ If your prescriber keeps you on a dose for longer, your own total will differ.

For a first year, the totals are £2,399.88 if you stay on 5mg after your 2.5mg starting pen, and up to £3,279.88 if you step up through every dose to 15mg (13 pens each). For a maintenance dose between those two, use that dose's monthly price rather than a year figure, because the path to it depends on your prescriber's plan.

Why plan a whole year? Because treatment is gradual. Trials measured results at 72 weeks, so think in months rather than days.¹ A prescriber reviews how you are responding, and if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you (NICE TA1026).⁴

Ways to spread the cost include Klarna and pay-monthly options, and there is no subscription lock-in, so you only order when you and your prescriber agree the next pen is right. 08 · NHS versus private

NHS or private: who can get weight loss injections and how

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 and above with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Through the NHS you pay only the standard prescription charge of £9.90 per item in England where charges apply, but you must meet the clinical criteria for your area and stage of rollout. A private prescription follows the licence rather than the NHS rollout criteria. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.¹ It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable.

Neither route is a shortcut around assessment. A prescriber looks at your BMI, medical history and current medicines in both. If the prescriber decides the treatment is not suitable, we refund the order automatically in full.

09 · Other injections compared

Wegovy, liraglutide and other weight loss injections

Mounjaro is not the only weight loss injection, but each medicine has its own dosing, schedule and cost structure, so compare them like with like. Wegovy (semaglutide) is also a once-weekly injection. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly; the dose steps up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17.¹⁸ The 2.4mg dose is the licensed maintenance dose for weight management.

Liraglutide, sold as Saxenda and in products such as Erly, is supplied at a concentration of 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. Daily dosing is a practical difference to weigh up alongside price. For any other injection, your prescriber can explain the dosing and what to expect.

Price comparisons only make sense when the basis matches. For first-order prices, your first 2.5mg Mounjaro pen costs £129.99 with code FIRST40, while the first Wegovy pen costs £99.99 at first order. Never set a standard price against a first-order price, because the first-order figure already has a discount in it.

What about tablets? Some GLP-1 medicines are taken by mouth rather than injected. Wegovy tablets (oral semaglutide) are taken on an empty stomach after fasting for at least 8 hours, swallowed whole with a sip of water (up to half a glass, about 120ml), and then you wait at least 30 minutes before eating, drinking or taking other tablets.²⁰ Foundayo (orforglipron) can be taken any time of day, with or without food and without water restrictions, swallowed whole.²¹ See current tablet prices on the relevant product pages, and do not compare their trial results with injection results as if they were head to head.

Which medicine is right is a prescriber's decision, based on your medical history, current medicines and how you have tolerated treatment before. The consultation is where those factors are weighed up properly.

10 · What results to expect

What results to expect from weight loss injections

Weight loss on Mounjaro is gradual by design, and trial averages do not predict your own result. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

As a worked example: for someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. That is straight arithmetic on the trial averages, not a forecast for you.

The same trial reported that at 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.² These are proportions from a trial population, not promises about any one person.

The first weeks on 2.5mg are an adjustment phase, and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Think in months, not days, and judge progress by trend rather than individual weigh-ins.¹ Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.

Your prescriber reviews your response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you (NICE TA1026).⁴

If you stop, planning it with your prescriber is advised. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average.¹⁴

11 · Side effects and safety

Side effects, safety and what to watch for

Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects, up to 1 in 10 people, include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.¹³ Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Serious side effects are uncommon but need urgent attention. Get medical help straight away for:

• severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis);
• pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems);
• signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.¹³ You can report any suspected side effect through the MHRA Yellow Card scheme.¹⁹

Contraception needs specific attention. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹⁷ This applies to the pill only; implants, coils and injections are unaffected.

Who should not take it? You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive; stop at least one month before trying to conceive.¹ Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.

12 · Buying safely and storage

Buying weight loss injections safely and storing your pen

Price is the first thing most people compare, but the source matters more, because fake weight loss pens are in circulation. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation, so be wary of anyone offering to supply without a consultation or prescription.¹²

When your pen arrives, check it. A genuine pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been in use for more than 30 days. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.

Storage is simple. Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days, which is useful for short trips, and must then be thrown away. Do not use a pen that has been above 30°C or frozen. Keep the pen cap on to protect it from light.¹³ Unopened pens stay in the fridge until first use and keep until the printed expiry date. If an unopened pen has been out of the fridge, check with the pharmacy before using it.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days at a temperature not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch.
