04 · Why 2.5mg is a starter dose
Why Mounjaro starts at 2.5mg
Mounjaro starts at 2.5mg because the body needs time to get used to the medicine, not because the lowest strength is expected to do the heavy lifting. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.
To see why a gentle start makes sense, it helps to understand what the medicine does. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1
In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 The licensed dose ladder already starts low (2.5mg) for tolerability.1
It also explains why the starting strength should not be judged by the scales. The first weeks on 2.5mg are an adjustment phase and losses are usually modest.1 If you judge the medicine only on what happens at its lowest dose, you are judging a warm-up.
Another thing worth knowing is that the starting dose is the same for everyone. Your height, starting weight or how motivated you are does not change the first rung. That is deliberate: tolerability, not ambition, sets the pace. Someone who is keen to progress quickly still starts at 2.5mg, and someone who is cautious still moves on once the minimum interval has passed and things feel settled.
The injection itself is simple. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 That routine is worth building properly during the starting weeks, because you will repeat it at every later dose.
So the starter dose does a specific job: it lets your body adjust, and it gives you and your prescriber early information about how you respond. Once that job is done, the licence points you upward, one step at a time.
05 · When a longer stay makes sense
When your prescriber might hold you on 2.5mg
A prescriber may keep you on 2.5mg for longer when side effects are still getting in the way of daily life. The ladder is built for flexibility: the 5mg step is allowed after a minimum interval, not demanded at that moment, and the dose depends on how you respond and tolerate it.1
The commonest early effects are well described. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If they settle, there is usually little reason to wait. If they are still strong as the first pen finishes, tell your prescriber, who can decide whether to hold the dose a little longer.
What a hold looks like in practice is simple. You carry on with your usual weekly dose at 2.5mg under the prescriber's direction, you keep reporting how you feel, and you step up when you both agree it is time. The licence also allows a step down if that is what tolerability calls for.1
It is important to separate a prescriber-led hold from doing something yourself. Holding means the prescriber has looked at your response and chosen to wait. It does not mean skipping doses to feel better, halving the dose in the pen, or spreading a pen across a longer period than designed.

Side effects you did not expect deserve attention rather than patience alone. 06 · Weight loss on 2.5mg
What weight loss to expect on 2.5mg
Weight loss on 2.5mg is usually modest, and that is normal. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1
Experiences differ a great deal, and both ends are normal. Some people notice appetite change and early loss in the first week, others feel little until a higher dose.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. If your scales barely move in the starting month, that is not a verdict on the treatment.
The headline trial numbers describe a different situation from the starting weeks. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Notice that none of those figures belong to the 2.5mg starter dose; they describe the doses you step up to.
For the early months, a more useful benchmark is this: A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure — the dose you respond to may still be ahead of you.
Judge progress sensibly. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.1 Weekly weighing can be useful if it helps you, but daily swings of water and food in the gut can easily mask a real trend, so many people prefer a regular, consistent weigh-in and a monthly look at the direction of travel.
Also remember that there is more to the picture than the scales. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are a reason to track how you feel and how your clothes fit as well as the number.
Finally, your response is reviewed. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That review point sits on the highest tolerated dose, which again shows why the starting strength is a staging post and not the place where results are judged.
07 · Staying put and microdosing
Staying on 2.5mg for longer, or stretching it
Staying on 2.5mg because a prescriber has chosen to hold you there is fine; stretching or splitting doses yourself is not. The distinction decides whether you are inside the licence or outside it, and it is worth being plain about.
The safety position is clear. Altering doses outside the licence means unknown dosing accuracy and no safety data.1
People who want to stay low are usually motivated by fear of side effects or a feeling that a small dose is "enough". Each has a legitimate answer inside the licence. For side effects, ask for a hold or a step down. For the feeling that a small dose is enough, review your actual progress: if you are happy with the trend, say so, and your prescriber can advise whether the dose stays where it is.
Note that the licence describes where many people end up. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 That sentence matters in both directions. It tells you that 2.5mg is not the listed maintenance rung, and it also tells you that you do not have to chase the top dose.

One more caution: the same applies to other products. A plateau is not a reason to double doses or add unlicensed products.1
08 · Injection routine and storage
Getting the routine right on your first pen
The starting weeks are the best time to build a routine you can repeat for as long as you are on treatment. Get the basics right now and every later dose becomes automatic.
Pick a day and stick to it. There is no "best" time of day — Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember (many people choose a weekend morning so early side effects fall on days off) and stick to it.1
Choose your site and rotate it. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.13 Needles, a sharps bin and injection guidance come with every order from us.
Know what to do if you miss a dose. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 This matters in the starting weeks because a missed or mistimed injection can feel like a reason to extend the dose; in fact the missed-dose rule tells you exactly how to carry on.
You can also move your day. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That is useful if a holiday or shift pattern changes, and it avoids the temptation to skip.
Store the pen properly. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.13
Travelling during your first pen is manageable. You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch.13 If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.13

09 · Plateaus and moving on
If progress stalls or you are ready to move up
A stall at the starting dose is common and does not mean the medicine has failed. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1
Within the licence, the responses are practical and unglamorous. Review food intake honestly, because portions drift back up without anyone noticing. Protect protein and activity. Check the basics such as sleep and alcohol. Then discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.1
If you are ready to move up, here is what to expect. The next strength is 5mg, the first licensed rung beyond the starting dose, and your prescriber will confirm the timing. From there the dose can step up by 2.5mg at a minimum of 4-week intervals, depending on how you respond and tolerate it.
Expect the next phase to feel different. Appetite effects often become clearer at higher strengths, and some of the early side effects can return briefly after a step up before settling again. Keep the same injection day, keep your notes, and tell your prescriber how it feels. If a step feels too big, ask for a hold; the licence allows it.
Plan your food around the medicine rather than hoping the injection will do everything. Trial results describe treatment alongside diet and activity, and nothing in the licence changes that. Smaller portions, regular protein and movement you enjoy are not extras; they are part of the treatment plan your prescriber is assuming.
Costs are worth planning too, since a longer titration means more pens at lower strengths before you reach a maintenance dose. 10 · Stopping, goals and maintenance
Reaching a goal weight, stopping and maintenance
There is no body-mass-index number at which Mounjaro automatically stops, and the decision to reduce or stop belongs with you and your prescriber.
What "maintenance" means in practice is the dose you settle at once you have responded and tolerated treatment well. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1 So maintenance is not a separate drug or a secret schedule. It is simply the strength that suits you.
If you reach your goal weight, talk to your prescriber before changing anything.
If you do stop, know how the medicine leaves the body. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 During that period appetite effects can gradually fade, so it is wise to have a food and activity plan ready before your final dose, not after.
Staying healthy at lower weights is also part of the conversation. Body mass index is a screening tool, not a stop signal, and the thresholds in our eligibility information (27, 30, 35 and 40 kg/m²) describe who may start treatment, not when to finish. If you are worried about losing too much weight, or you feel unwell, speak to your prescriber promptly.
Finally, remember why prescribers frame this as long-term health. The trial-average improvements in waist size, blood pressure, cholesterol and blood-sugar measures are part of the reason weight management is treated as health management.2 Decisions about when to reduce or stop should weigh those benefits, not just a number on the scales. 11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
12 · NHS, private and ordering
NHS or private: where the 2.5mg pen comes from
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
Either way, the dose ladder is the same. Whether your prescriber is an NHS clinician or a private one, the pen strengths and the step-up rules come from the same licence. If you are on the NHS and want to know your timing, ask your clinician at the next review.
