01 · The core answer
How long you stay on each Mounjaro dose
You stay on each dose for at least the minimum interval in the licence, and often longer.
Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
Plenty of people never go that far. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.1

02 · The decision you face
What decides whether you step up
Two things decide a step-up: how you tolerate the current dose, and how you are responding. The licence says the dose rises depending on how you respond and tolerate it, so the calendar alone never decides.1
If you feel well, your appetite is clearly changed and progress is steady, a prescriber may be happy to hold you on a dose. If side effects are lingering, your prescriber can hold you on the current dose for longer.

04 · The licensed ladder
The licensed dose ladder, rung by rung
The ladder has six strengths, and the licence defines the smallest allowed gap between them. Understanding that gap explains almost every question about timing. The ladder runs 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, and each is delivered by the same KwikPen design.1
After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 It sets a floor on time, not a ceiling. Nothing in it says you must move up at the earliest opportunity.
Each rung is a 2.5mg step. The steps are equal in size, but how your body feels about each one is not equal. Some people find the first step to 5mg the most noticeable. Others barely notice 5mg and feel the jump to 10mg. There is no way to predict this in advance.
Because the minimum interval is after at least four weeks and each pen contains four weekly doses, a typical titration lines up neatly with the pens: one strength per pen, then a new strength with the next pen. A final point on the ladder: it is a one-way route only in the sense of progression, not obligation. Your prescriber can hold you at a strength for longer, and holding at a strength is entirely normal. The ladder is a menu of strengths, not a race to the top.
Be wary of any seller, forum post or friend who suggests faster escalation. Your prescriber is the person to ask if you feel you are progressing too slowly or too quickly.
05 · The starting weeks
The 2.5mg starting weeks
The 2.5mg weeks are about adjustment, not weight loss, and that shapes how long you stay there. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1
Experiences on this dose differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1
So how long should you stay? The licence sets the earliest point you can move on: at least 4 weeks on each dose, depending on how you respond and tolerate it. If you are not, your prescriber can keep you on 2.5mg for longer. Neither is a failure. The aim is a dose you tolerate, not a record for fastest escalation.
There is a particular temptation at this stage: because the starting dose is not designed to produce large losses, some people feel it is wasted time and try to skip ahead. Skipping is not licensed. The ladder starts low for tolerability.

It also helps to build the habits that will matter later. Pick a day and time you will remember. There is no best time of day: Mounjaro works the same morning or evening, with or without food. What matters is the same day each week, and many people choose a weekend morning so early side effects fall on days off.1
Use the starting weeks to practise rotating sites and to notice how your appetite and energy change across the week. Licensed sites are the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.13 Notes from these weeks make your first review conversation far more useful, because your prescriber can see whether the next step looks comfortable or whether a longer hold would help.
06 · Holding a dose
When people stay longer on a dose
Staying longer on a dose is a normal, licensed choice, usually driven by tolerability. The licence sets only the minimum interval, so a prescriber can hold you where you are for as long as makes sense.
There are several honest reasons to hold. Side effects that have not settled after a few weeks are the most common one.
Holding is also sensible if your appetite is already well controlled at the current strength. The dose is a tool, not a target. People often assume that more medicine means more progress, but the licence is clear that higher is not automatically better.1
What holding should never look like is self-directed dose manipulation. If you want to slow down, the safe route is a conversation with your prescriber, who can hold you on your current strength within the licence.

Whatever the reason you hold, tell your prescriber at your next review. Record what you noticed, such as appetite, energy, digestion and how the days after injection felt. This gives them better information than a simple request to stay put or to move up.
07 · Maintenance dose
Reaching a maintenance dose
A maintenance dose is the strength you stay on once you and your prescriber agree it is working, and there is no single right one. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1
This is where the question of how long you stay on each dose changes character. During titration, each stage is temporary by design. At maintenance, the stay is open-ended. You remain on the strength that balances progress, side effects and day-to-day life, and your prescriber reviews it as you go.
It helps to know what the trials measured at different strengths. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
For an example of what a percentage means in kilograms: for someone starting at 100 kg, 20.9% is about 21 kg. Your own starting weight, diet, activity and response will decide your result, and trial averages do not predict it.
Beyond the scales, weight loss on tirzepatide came in trials with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises.2
Choosing a maintenance dose is a shared decision. If you feel well, your progress is steady and side effects are minimal, there may be little reason to climb further. If progress has stalled and you are tolerating the dose, your prescriber may discuss whether a step-up is appropriate while you are not yet at the maximum. One more fact worth knowing: tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That is relevant to anyone planning a change of dose or a break, because effects fade gradually rather than abruptly.
08 · Realistic pace
Realistic pace of results across the doses
Results build gradually across the ladder, so judge each dose by trend, not by the scales on a single morning. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1
Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.2 Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full averages measured at 72 weeks. A sensible three-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.
Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
Plateaus are part of the picture too. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight. Within the licence, the responses are to review food intake honestly because portions drift back up, protect protein and activity, check the basics such as sleep and alcohol, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.1
In short, the right length of time on a dose is long enough to know how it works for you. That might be the minimum interval, or it might be much longer. Keep a simple log of appetite, side effects and weight trend, and bring it to each review.
09 · Missed doses and timing
Missed doses, changed days and the ladder
A missed dose does not reset the ladder, but the timing rules matter. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.13
For titration, this means a single late or skipped injection usually does not change your plan. Tell your prescriber if several doses have been missed, so they can advise on the pace.
You can also change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.13 This is useful if your routine changes. If you are unsure whether you are due to step up after a shift, ask your prescriber rather than counting days alone.
Consistency is the real goal. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Each KwikPen contains four weekly doses.13 Because a pen contains four weekly doses, most people change strength when they start a new pen.

Do not respond to a missed dose by compensating with extra injections or by combining pens. That is outside the licence and carries unknown risk. Carry on with the dose your prescriber has set.

What the ladder costs, and the NHS alternative

If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg.

On the NHS route, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). 11 · Storage between doses
Storing pens while you move through the doses
Good storage protects each dose, and it matters more when you are holding a strength for several pens. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1
Because you may have several pens at the same strength over time, keep a clear system. Store unopened pens in the fridge until first use, where they keep until the expiry date printed on the box. Use the oldest pen first, and write the start date on the box once you begin one.
If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, as figures circulating online often come from other countries' labels.1 The UK in-use limit is up to 30 days at a temperature not above 30°C.
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. If anything about a pen looks unusual, do not inject. Contact the pharmacy, and report any suspected side effect through the MHRA Yellow Card scheme.19
12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
