Should you stay longer or step up?

Step up when you have finished the starter pen, side effects are manageable and your prescriber agrees. Stay longer only when there is a clinical reason, and the reason is nearly always tolerability. A slow scale is not on its own a sign you should linger, because the starter dose is not meant to deliver much weight loss.

Ask yourself these questions before your review:

  • Have the side effects after each injection eased between doses, or are they getting worse?
  • Can you eat and drink normally on most days?
  • Did you inject on the same day each week without missed doses?

Mounjaro starts at 2.5mg because the body needs time to get used to the medicine. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

04 · Reasons to stay longer

When a longer stay on 2.5mg makes sense

A longer stay on 2.5mg makes sense when side effects are still getting in the way of normal life. The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.1 If they have not settled by the time the starter pen is finished, tell your prescriber rather than pushing on.

Within the licence, your prescriber can hold a dose longer or step down if side effects are the concern.1 That is a clinical decision made at review, usually after hearing how severe the symptoms are and when in the week they occur. It is not something to arrange yourself by skipping doses or injecting less than the pen delivers.

Reasons that do not justify a longer stay include:

  • Worry about the next dose being stronger, when you have had little trouble so far.
  • A wish to stay on a low dose, because the starter dose is not intended for maintenance.
  • Disappointment that the scales have barely moved, because that is expected at this stage.

07 · Staying safe on the pen

Injecting safely on the starter dose

Safe use on 2.5mg means following the pen's design and the label, not improvising. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Rotate the site with each dose.1 Each KwikPen contains four weekly doses.13

Missed a dose? Take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

Storage matters too. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 Keep the cap on to protect it from light.

What comes after 2.5mg

After the starter phase, the dose steps up in stages that your prescriber paces to your response.

Results are reviewed rather than assumed. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.

Weight loss is not linear. Plateaus of a few weeks are normal and expected as the body adapts to a lower weight. Within the licence, the responses are: review food intake honestly (portions drift back up), protect protein and activity, check the basics (sleep, alcohol), and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.1

Why the early weeks still matter

The starter phase lays the groundwork for the benefits seen later. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises.2

That is why prescribers talk about weight management as health management rather than appearance. It also explains why patience through 2.5mg is worth it: getting through the adjustment phase comfortably puts you in a better position to continue to the doses where those results were measured.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
