How long can you stay on Mounjaro 7.5mg?
You can stay on Mounjaro 7.5mg until your prescriber decides a different dose suits you better. What the licence does set is the pace of increases: After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
That wording is a minimum, not a schedule. Many people move on after the minimum gap; others hold for several months because they are losing weight steadily and feel well. Both are reasonable, and the prescribing team reviews you as you go.
02 · Your decision
Should you hold at 7.5mg or step up?
You should hold at 7.5mg if your appetite is still well controlled, your weight is trending down and side effects are manageable. You should talk to your prescriber about stepping up if progress has stalled and you are not yet at your highest tolerated dose.
Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1 That means 7.5mg can be a pause on the way to 10mg, or the place where you find your pace before deciding. A useful way to frame it:
- Hold when the trend on the scales is still moving the right way, hunger is quiet and side effects are mild.
- Discuss a step up when appetite has crept back and the trend has flattened over several weeks despite honest food and activity habits.
- Discuss holding or stepping back when side effects are hard to live with. Your prescriber can hold a dose longer or step down within the licence if side effects are the concern.1

03 · The dose ladder
Where 7.5mg sits on the dose ladder
7.5mg is the third strength on the Mounjaro ladder, reached after you have been on 2.5mg and 5mg. Everyone starts on 2.5mg once a week, and that starting dose is about letting your body adjust, not about weight loss itself.1
From there the dose can step up by 2.5mg at a time, never faster than every four weeks, to a maximum of 15mg weekly. Mounjaro comes as a pre-filled KwikPen in six strengths, and each pen holds four weekly doses.1
Because many people settle at 5mg, 10mg or 15mg, 7.5mg is often described as a step rather than a destination. That description is useful but not a rule against staying. Your prescriber chooses the dose from your response and tolerance, as the BNF and the product information both make clear.11

04 · Reasons to stay
Good reasons to stay on 7.5mg for longer
A good reason to stay on 7.5mg is that it is working and you feel well on it. There is no clinical benefit in moving up a dose simply because the calendar allows it.
Common situations where holding makes sense:
- Steady progress. Your weight trend is moving down month by month and you are not struggling with hunger.
- Side effects settling. Each step up can bring a fresh wave of nausea or tiredness in the days after injecting. If you are only just feeling comfortable, more time on the same strength can help.
- Appetite is controlled. If you feel full sooner and less hungry between meals, the dose is doing its job.1
- You are near your goal. Someone who has only a little left to lose may not need a higher dose at all.
The trial evidence shows why higher is not the only route. In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
05 · Reasons to move on
When it makes sense to move up from 7.5mg
It makes sense to discuss moving up when your response has levelled off and you have not reached your highest tolerated dose. Progress that has flattened for a few weeks is normal; progress that has flattened for months while hunger returns is worth a conversation.
Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight. The licensed responses are to review food intake honestly, protect protein and activity, check the basics such as sleep and alcohol, and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.1
Response is also formally reviewed. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

06 · 7.5mg versus 5mg and 10mg
Does 7.5mg work better than 5mg, and how does it differ from 10mg?
7.5mg is a higher strength than 5mg and a lower one than 10mg. Whether that translates into better results for you depends on your response, because many people do very well on lower strengths.
What the evidence says: trial averages rise across the dose range, with the largest averages at 15mg, but they are averages across many people and your own result will differ.2 A person who responds well at 5mg may gain little from stepping up, while someone whose appetite returns at 5mg may notice a clear difference at 7.5mg.

07 · What to expect
What to expect on 7.5mg
On 7.5mg you can expect steady rather than dramatic progress, with the biggest influences being your food habits, activity and how well you tolerate the dose. Weight loss on Mounjaro is gradual by design; most measurable progress builds from 5mg onward, month by month, and trials measured results at 72 weeks. Think in months, not days, and judge progress by trend, not individual weigh-ins.
Three months in, most people are still on the lower rungs of the ladder, so trial-average losses at that stage are modest compared with the headline figures. A sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
Side effects tend to be most noticeable in the days after each injection or after a step up. Mild nausea, altered appetite and tiredness are the commonest early effects and usually settle. If anything worries you, or a symptom is new, contact your prescriber and report it through the Yellow Card scheme.19

08 · Do not stretch doses
Why you should not stretch or split doses to stay on 7.5mg
You should never change the gap between injections or divide a dose to make a pen last, because that is outside the licence. Mounjaro is a once-weekly injection on the same day each week, at any time of day, with or without food.1
Practical rules that stay within the licence:
- If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.
- You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.
None of these let you spread a dose over a longer period. Staying on 7.5mg means taking the full 7.5mg once a week, not less often.
09 · Stopping and the long term
Staying on treatment long term
How long you stay on Mounjaro overall is a separate question from how long you stay on 7.5mg, and it is a decision for you and your prescriber. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
Weight management is health management, not just appearance. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. 10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
