03 · How the ladder works

How the dose ladder works

Everyone starts Mounjaro on 2.5mg once a week, and the licence builds in a gentle climb from there. The starting dose is about letting your body adjust, not about weight loss itself. If you want to see how that first rung works, read about how long you stay on 2.5mg.

The medicine comes as a pre-filled multi-dose pen (KwikPen) in strengths of 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg, and each pen contains four weekly doses.1

The minimum interval of at least 4 weeks on each dose gives your body time to adjust before the dose changes. It is not a schedule you are expected to hit automatically, and a prescriber who keeps you on 5mg longer is following the licence, not falling behind it.

Because the dose ladder is a menu rather than a conveyor belt, two people on 5mg for the same number of weeks can have completely different plans. One may be happily losing weight and staying put. The other may have stalled and be discussing a step up. Both are normal.

04 · Is 5mg working?

How to tell if 5mg is working for you

5mg is working if your weight and appetite are moving in the right direction over months, not days. Mounjaro is gradual by design: the first weeks on 2.5mg are an adjustment phase and losses are usually modest, and most measurable progress builds from 5mg onward, month by month.1

It helps to know what the trial evidence actually says about this dose. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Staying on 5mg is a reasonable choice and not a compromise. Do not read the dose figures side by side as a promise that moving up will give you a specific extra amount. Trial averages do not predict your own result.

Judge by trend, not single weigh-ins

05 · Plateaus on 5mg

Plateaus on 5mg: what to do

A plateau on 5mg is common and is not, on its own, a reason to move up. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 The aim is to separate a short pause from a genuine stall.

Within the licence, the responses are sensible and in a particular order:

  • Review your food intake honestly, because portions drift back up without anyone noticing.
  • Protect protein and activity, which support the changes you are making.
  • Check the basics, including sleep and alcohol.
  • Discuss with your prescriber whether a dose step up is appropriate if you are not yet at maximum.

That order matters. Jumping straight to a higher dose treats the symptom without checking the cause.

One firm rule: a plateau is not a reason to double doses or add unlicensed products.1 If you are tempted, speak to your prescriber instead.

06 · Treatment review

When your prescriber will review 5mg

Your prescriber reviews your response, and there is one formal checkpoint worth knowing. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

The review is tied to the dose that suits you, which could well be 5mg. If 5mg is the most you tolerate comfortably, that is the dose against which progress is judged, and you are not expected to push into side effects to reach a higher number.

Moving up the ladder is a tool for people who need it, not a goal in itself. Higher is not automatically better, and many people settle at a maintenance dose of 5mg, 10mg or 15mg.1 A person who is steadily losing weight on 5mg with mild side effects may have little reason to change dose.

On the other side, the trial averages do rise with dose, and some people genuinely need more to keep their appetite controlled. The sensible stance is neither "always stay" nor "always climb". It is to ask what the current dose is doing for you and whether the trade-offs of a change are worth it.

Beyond the scales, trial participants on tirzepatide saw average improvements in waist size, blood pressure, cholesterol and blood-sugar measures.2 These are trial-average secondary outcomes, not promises, but they are a reminder that the goal is health, not simply the highest dose or the lowest number on the scale.

08 · No dose stretching

Do not stretch, split or space out doses

If you want to stay on 5mg, stay on it properly: the licensed dose, once a week.

The reasons are practical. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

Tell your prescriber what is happening rather than adjusting on your own.

Keep the routine steady too. Inject on the same day each week, at any time of day, with or without food, and rotate the injection site each time.1 There is no best time of day, so pick a day you will remember. Pens stay in the fridge at 2–8°C until first use, and once in use a pen can be kept out of the fridge not above 30°C for up to 30 days before it should be discarded.1

09 · Stopping or changing

Coming off 5mg or changing plan

You can discuss coming off at any time, and it is worth doing that with your prescriber rather than alone. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

11 · NHS and private access

NHS and private access to 5mg

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
