The missed-dose rule exists so that nobody ends up with doses bunched together. Mounjaro is a once-weekly injection, taken on the same day each week, and the whole schedule is built on a regular gap between doses. When that gap is interrupted, the rule decides whether to inject late or to leave the dose out.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. The other half of the rule applies to you if a full week has gone: If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.13

Put simply, there is a late window. Inside it you can still take the dose. Outside it, that dose is gone. A full skipped week sits clearly outside it, so the answer to how many days you can skip is that there is a short grace period for injecting late, but beyond that you do not skip and then compensate; you simply leave the dose out.

This is a deliberately cautious rule. The final part, that you never take two doses close together, is there for your safety. We would rather you lose a week of treatment than risk bunching doses.

It also helps to know what the rule does not say. It does not tell you to inject earlier next time, and it does not suggest that the following dose should be larger. It does not give you a way to add the missed week to the end of your pen. Your prescriber is the person to ask if your situation is unusual, for example if you have missed more than one injection in a row.

Finally, keep your own record. Write down the scheduled date, the date you actually injected, and the reason for any gap. That record makes your next review quicker and gives your prescriber something concrete to work with, rather than an estimate from memory.

06 · How long it stays in your body

Why one skipped week is a dip, not a reset

A single skipped week is a small dip because tirzepatide leaves your body slowly. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears. 1

That has a practical meaning. The amount in your system does not drop to nothing after a skipped injection. It declines gradually across the following weeks, and a normal injection on your next scheduled day brings you back into the regular pattern. This is why a single gap rarely feels dramatic, and why it is reasonable not to panic if you realise you have missed a dose.

What you may notice is mostly about appetite. Mounjaro activates two natural gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals. When the medicine in your system dips, that effect can soften, so you may find portions creeping up or snacking returning. That is a signal to be mindful with food for a few days, not a sign that treatment has failed.

Where it gets less clear is when a gap runs longer than a single week. Your prescriber decides what to do after a longer break. Your prescriber is the right person to decide whether to continue at your current dose or to step back.

Titration is relevant too. The ladder starts everyone on 2.5mg. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. 1 If a gap happened during a step-up, ask your prescriber whether to move up on schedule or stay on your current dose a little longer. Higher is not automatically better, and the pace should suit you.

If you are changing your day because the skipped week has thrown your routine off, this is allowed. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Choose a day you will realistically keep, rather than moving it repeatedly.

Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.13

Storage is what makes this practical. Pens should be kept in a fridge at 2–8°C and never frozen. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.1 For hot climates, use an insulated pouch. Keep the cap on to protect the pen from light.

For unopened pens, the rule is simple: they stay in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit; figures circulating online often come from other countries' labels.13

Illness is different. If you are unwell and cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.

Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Taking Mounjaro every other week is not a licensed way to use it. People often ask whether a skipped week can become a pattern, perhaps to save money, to reduce side effects or to make a pen last longer. The licensed schedule is a weekly injection on the same day each week, and nothing in the product information supports spacing the doses further apart.1

Taking it every two weeks or "stretching" pens to save money is unlicensed, makes levels rise and fall unpredictably, and has no trial evidence for keeping weight off. 1

A single skipped week is unlikely to undo your progress, but it may slow it slightly or bring some appetite back. Weight loss on Mounjaro is gradual by design, and progress builds over months rather than days.1 One missed injection is a small interruption in a long process.

The trial evidence shows what steady use looks like. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and the treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Those trial averages do not predict your own result, and they cannot tell you the effect of a single missed week.

The first weeks matter less than people expect. The 2.5mg starting weeks are about adjustment, and some people notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 That advice applies directly after a skipped week: do not judge the gap by the scales on the following morning.
