If you skip a dose of Mounjaro, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual injection day. If more than 4 days have passed, skip it entirely and take your next dose on your normal day. Never double up to make up for it. One missed dose is not dangerous, and the medicine clears slowly.

Why the weekly timing matters
Mounjaro is a once-weekly injection designed to keep a steady level of medicine in your body. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1
That routine is the reason the missed-dose rule exists. Injecting soon after you remember keeps you close to your schedule. Never take two doses within 3 days of each other.
There is also no need to worry about time of day. There is no "best" time of day — Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1
04 · What you may notice
What you may notice after a skipped dose
Most people notice little in the immediate days after one skipped dose, because the medicine leaves the body slowly. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
That slow clearance is why a single skipped injection does not suddenly switch off the medicine.
Mounjaro slows how quickly the stomach empties and helps regulate appetite.1
Weight can also shift for ordinary reasons. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 One skipped dose is a small blip, so judge progress by trend rather than a single reading.
05 · Side effects on return
Side effects when you restart
After a short gap, most people simply carry on at their usual dose. Your prescriber will guide the right pace, and higher is not automatically better.1
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If symptoms feel unusual, severe or new, speak to your prescriber, and report suspected side effects through the MHRA Yellow Card scheme.19
Everyone starts Mounjaro on 2.5mg once a week, and the dose can then step up in stages up to a maximum of 15mg weekly.1 Whether you restart at your previous strength or a lower one after a long break is a prescriber decision. It is not something to work out for yourself from a forum or a chart.
06 · Changing your day
Changing your injection day instead of skipping
If your regular day no longer works, move the injection rather than skipping it. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1
This is helpful for holidays, shift changes or a busy weekend. Pick the new day, inject, and then keep to it every week. Many people choose a weekend morning so early side effects fall on days off.1
Do not use a day change as a way to stretch doses or to bring two injections close together. Never take two doses within 3 days of each other. If you are unsure, ask the pharmacy before you inject.
07 · Pens and storage
Pens, storage and a skipped dose
A skipped dose does not change how you look after the pen. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.1
This matters if you have been away and missed an injection. Check how long the pen has been in use before you inject. Each KwikPen contains four weekly doses.

What not to do after missing a dose

Altering doses outside the licence means unknown dosing accuracy and no safety data.1

How to avoid missing doses
Most missed doses come down to routine. A few simple habits make them rare.
- Pick a day and time you will remember, and keep it the same every week.
- Set a repeating phone reminder, or log each injection in a tracker such as our free GLPTrackr companion app.
- Keep the pen in a place you see regularly, while still following the fridge storage guidance.
- Take the pen and a copy of your prescription with you when you travel.
- Tell your prescriber early if side effects tempt you to skip.
You can fly with Mounjaro: carry pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription.1
Weight loss on Mounjaro is gradual by design, and trials measured results at 72 weeks. Think in months, not days, and judge progress by trend, not individual weigh-ins.2 A single missed dose, handled correctly, is a small bump rather than a setback.

Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
