Which is cheaper, Mounjaro or Wegovy?

The honest answer depends on the dose, because both medicines get dearer as the dose rises. At the starting pen, you compare £129.99 for a first Mounjaro 2.5mg pen (standard price £169.99) with £99.99 for a first Wegovy pen. Both are first-order prices with a new-customer discount already inside them, so you can read them directly against each other.

Beyond the first pen the picture changes. Mounjaro has six strengths, from £169.99 at 2.5mg to £299.99 at 15mg per month. Wegovy has five, ending at the 2.4mg maintenance dose. Cost per kilogram lost is a fairer yardstick than cost per pen. In the SURMOUNT-5 head-to-head trial, tirzepatide produced a higher average loss than semaglutide 2.4mg.¹⁵ A medicine that costs more per month can still be the better-value option for someone whose body responds well to it. Equally, a cheaper pen you tolerate and keep taking is worth more than one you stop.

04 · How they differ

What Mounjaro and Wegovy are, and how they differ

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ Wegovy is semaglutide, also a once-weekly injection, supplied as 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg pens.¹⁸

The key difference is the mechanism. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide.¹

Wegovy's dose ladder is also worth understanding because it shapes cost. Titration steps up after at least four weeks through 0.25mg, 0.5mg, 1mg and 1.7mg, reaching the 2.4mg maintenance dose from week 17. That 2.4mg dose is the licensed maintenance dose for weight management.¹⁸ You therefore pay for four pens at rising strengths before you reach the dose the trials tested.

Mounjaro's ladder has six steps, and your prescriber decides how far up you go. Not everyone needs 15mg.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. Nothing needs doing while it clears, and there is no way to speed it up.¹ Your prescriber will explain what to expect if you ever pause or change treatment.

05 · Trial results

Head-to-head trial results: what you get for the money

In the first large head-to-head trial, SURMOUNT-5, adults on tirzepatide lost on average 20.2% of body weight over 72 weeks, versus 13.7% on semaglutide 2.4mg (Wegovy).¹⁵ Both are effective, licensed treatments. Which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone. Individual results vary.

The detail helps when weighing price against outcome. In SURMOUNT-5, 751 adults took one of the two medicines for 72 weeks. At least 10% of body weight was lost by 81.6% on tirzepatide versus 60.5% on semaglutide. At least 15% was lost by 64.6% versus 40.1%, at least 20% by 48.4% versus 27.3%, and at least 25% by 31.6% versus 16.1%. Average loss was 22.8 kg versus 15.0 kg.¹⁵

As a worked example, for someone starting at 100 kg, 20.2% is about 20 kg and 13.7% is about 14 kg. These are trial averages, and your own result will differ.

Each medicine also has its own pivotal trial. In SURMOUNT-1, which enrolled 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. At 72 weeks, 85% on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5%.²

In STEP 1, adults on semaglutide 2.4mg lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo. Of those on semaglutide, 86.4% lost at least 5%, 69.1% at least 10% and 50.5% at least 15%.⁹ We list these separately because they came from different trials; only SURMOUNT-5 supports a direct comparison.

The takeaway for cost is simple. Choosing on monthly price alone can miss the picture, because a lower-priced pen is only good value if you tolerate it and keep taking it. Treatment works alongside a reduced-calorie diet and increased physical activity, whichever medicine you choose.

06 · Side effects

Side effects and why people stop

Side effects are one of the main reasons people stop either medicine, so they affect real-world value as much as the price. In STEP 1, gastrointestinal side effects were reported by 74.2% of people on semaglutide 2.4mg, compared with 47.9% on placebo. Of those on semaglutide, 7.0% stopped treatment because of side effects, compared with 3.1% on placebo.⁹

For Mounjaro, the most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.

Cost is the other reason people stop, and it is usually the one that can be planned for. A monthly price that feels fine at 2.5mg can feel heavy at higher strengths. Working out your likely maintenance dose before you start avoids the surprise.

Do not respond to cost pressure by changing how you use the pen. Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle. Talk to your clinical team rather than self-adjusting.

07 · Switching

Switching between them, and other options

There is no licensed direct conversion between semaglutide and tirzepatide. Switching is done under prescriber supervision: typically the previous medicine is stopped, a washout interval is observed, and Mounjaro is started at a dose your prescriber judges appropriate. That is often the 2.5mg starting dose, though it is sometimes adjusted based on your history. Never run two GLP-1 medicines together.¹

If you are thinking about switching from Mounjaro to Wegovy, the prescriber decides when to switch and which dose to start on. Wegovy's licensed dose ladder starts at 0.25mg.

Mounjaro must also not be combined with unlicensed research peptides such as retatrutide, which has no UK licence, or with compounded copies. These combinations have no safety data and no regulated supply chain. Anything promising faster results outside a licensed pathway is the warning sign.¹

Ozempic is a separate point. It is semaglutide licensed for type 2 diabetes and is not a UK weight-loss medicine, so prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg). So Mounjaro versus Ozempic for weight loss is really Mounjaro versus Wegovy, and a regulated UK pharmacy will not supply Ozempic for weight loss.¹⁸

There is no tablet form of Mounjaro; tirzepatide is injection-only in the UK. Two GLP-1 weight-loss tablets are now licensed: Wegovy tablets (oral semaglutide, made by Novo Nordisk) and Foundayo (orforglipron, made by Eli Lilly). They are different medicines with their own dosing, eligibility checks and trial results.²⁰²¹

Orlistat, a tablet, blocks some dietary fat absorption and produces smaller average losses; see our orlistat price comparison. Saxenda (liraglutide) is a daily injection, unlike the weekly Mounjaro and Wegovy. "Natural GLP-1" supplements such as berberine are not medicines, are not regulated for this use and have no comparable trial evidence; they are low-cost supplements with no clinical oversight. 08 · NHS and private

NHS or private: who can get which

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

NHS access to Wegovy runs through specialist weight-management services; ask your GP. The NHS prescription charge in England is £9.90 where it applies, but the NHS route depends on meeting the local criteria and being offered a place, not on cost alone.

For private treatment, the licensed criteria are wider than the NHS's current priority groups. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹

A prescriber will assess your BMI, medical history and current medicines before deciding whether either medicine is suitable. That is why our consultation is reviewed by a GPhC-registered prescriber and why unsuitable orders are refunded automatically in full.

If you are on the NHS pathway and receiving treatment, do not mix routes without telling both teams. Taking two GLP-1 medicines together is never appropriate. If you are waiting for an NHS place, private treatment is a personal choice and the cost is entirely yours, so planning the year in advance matters.

09 · Buying safely

Buying safely: counterfeits, genuine pens and false savings

A low price is only a saving if the pen is genuine. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.¹²

A genuine pen arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been in use out of the fridge for more than 30 days. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.¹³

Be wary of savings that rely on changing how the pen is used. Each Mounjaro KwikPen is designed and licensed to deliver exactly four weekly doses. Use the pen as designed, then dispose of it in the sharps bin we include.¹

The same caution applies to "too good to be true" prices from sellers offering to supply without a consultation or prescription. Sellers using dramatic weight-loss claims are another red flag. Genuine suppliers do not promise specific results.

How to choose between Mounjaro and Wegovy

The choice comes down to five practical questions, and price is only one of them. You can work through them with your prescriber during the consultation.

• Your goal. In the SURMOUNT-5 head-to-head trial, tirzepatide averaged 20.2% weight loss versus 13.7% on semaglutide 2.4mg over 72 weeks.¹⁵ Trial averages do not predict your own result.
• Your budget. Compare the first-pen prices, £129.99 for Mounjaro and £99.99 for Wegovy, then the monthly price at the dose you are likely to stay on.
• Your tolerance. Side effects, especially digestive ones, affect whether you can stay on treatment.
• Your history. If you have taken one before, tell the prescriber what dose you reached and how it felt. There is no licensed conversion between them.
• Your timeline. Both ladders take time to climb. Wegovy reaches 2.4mg from week 17, and Mounjaro steps up as your prescriber decides.¹⁸

Neither is better for everyone. Both are effective, licensed treatments, and the better one is the one that suits your health profile, that you tolerate and that you can sustain financially.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
