03 · What is Mounjaro

What is Mounjaro and where does 5mg fit?

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ Each KwikPen contains four weekly doses, so one pen covers four weeks of treatment.¹³

That structure explains why price comparisons are quoted per pen. When you compare a 5mg price, you are comparing the cost of four weeks of treatment at one strength. It is not a price per injection or per milligram.

The 5mg strength is the second step on the ladder. Everyone starts on 2.5mg once a week, and the starting dose is about letting the body adjust, not about weight loss itself. After at least four weeks the dose increases to 5mg. It can then rise by 2.5mg at minimum four-week intervals, through 7.5mg, 10mg and 12.5mg, up to a maximum of 15mg weekly, depending on response and tolerability.¹

So 5mg is both a stepping stone and, for many people, a destination. Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber guides the pace.¹ That is why the 5mg price matters more than it first appears: it is the price some people will pay for months, not just for one pen.

The mechanism is worth a sentence because it explains what you are paying for. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹ This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).

One practical point for cost planning: tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. Nothing needs doing while it clears, and there is no way to speed it up.¹

04 · Year cost planning

What a year at 5mg costs

A first year at 5mg costs £2,399.88 at our standard prices, and up to £3,279.88 if you settle at 15mg. A year is 52 weeks, or 13 pens. Those totals are before discounts: a new customer's first order is £40 less, and every later order is £25 less than the standard price.

The year figure includes one 2.5mg starting pen and the step up, so it is not simply thirteen pens at 5mg. If you take longer on a dose, your own total will differ, because your prescriber sets the pace, not a calendar.

For maintenance doses between those two, work from the monthly price: £244.99 for 7.5mg, £274.99 for 10mg and £289.99 for 12.5mg.

This is where comparison pays off. A difference between providers of £20 to £36.03 on the same dose repeats every month you stay on treatment. Across 13 pens that adds up, so the sensible comparison is the cost over the months you expect to be treated, not only the first order. Ask each provider what a repeat order costs, whether a consultation fee returns each time and whether delivery is charged.

05 · Results at 5mg

What results to expect at 5mg

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.²

To make the percentage concrete, for someone starting at 100 kg, 15% is about 15 kg. That is straight arithmetic on the trial average, not a prediction for you. The trial also reported that at 72 weeks, 85% of participants on 5mg had lost at least 5% of their body weight.²

Higher doses showed larger averages in the same trial: 19.5% on 10mg and 20.9% on 15mg, with 89% and 91% of participants losing at least 5% respectively. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.² Those figures matter for price comparison because they show why not everyone stops at 5mg, and why a monthly 5mg price is only part of your picture. If your prescriber later advises a step up, your monthly cost changes with the dose.

The pace is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.²

Response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.²

Experiences in the early weeks differ widely. Some people notice appetite change and early loss in the first week, while others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.²

06 · Eligibility and NHS

Who can get Mounjaro, and NHS versus private

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹ A prescriber assesses your BMI, medical history and current medicines before deciding whether it suits you. Eligibility is a clinical decision, which is why every order is reviewed by a GPhC-registered prescriber rather than approved automatically.

On the NHS, Mounjaro is available, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 or above with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

On the NHS, an eligible patient pays the standard prescription charge (£9.90 in England, where it applies) rather than the private pen price. The catch is access: whether and when you qualify depends on the rollout stage in your area and on your clinical profile. If you think you may qualify, ask your GP. The price landscape changed in 2025. Eli Lilly announced a UK list-price increase for Mounjaro on 14 August 2025, effective 1 September 2025. Pharmacy sell prices vary because of confidential discounts. That is one reason the same 5mg pen can cost different amounts at different providers, and why a dated comparison matters.

Whichever route you choose, the clinical safeguards are the same: a consultation, a prescriber's decision and follow-up. A pharmacy that offers to supply without a consultation or prescription is not a pharmacy you should use, however low its price. 07 · Genuine supply

Why price alone is the wrong test: genuine supply

A very low price can signal a pen that is not genuine, so verify the seller before comparing the figure. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷

The safe rule is simple. Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.⁷ Our registration number is 9012878, and you can check it yourself.¹²

Mounjaro is manufactured by Eli Lilly. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked.¹ Our supply is official LillyDirect supply, sourced directly from Eli Lilly's UK distribution, and all six doses from 2.5mg to 15mg are stocked.

When your pen arrives, check it. A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C or out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.¹³

08 · Cutting cost safely

Ways to cut the cost of 5mg safely, and ones to avoid

The regulated ways to lower the monthly cost are limited, so start with the legitimate ones: compare like for like and use the new-customer and reorder discounts. Some shortcuts are widely discussed online, and they are unsafe. Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.

09 · Using your pen

Using your 5mg pen: injecting, missed doses and changing day

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.¹³ Each KwikPen contains four weekly doses, so a 5mg pen covers four injections.

There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.¹³

The pen is set by turning the dose knob until the dose indicator shows the correct dose in the window. Follow the pen's instructions for the injection itself, and your order includes injection guidance, needles and a sharps bin.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹³

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹³

These details connect to cost. A missed or wasted dose is money wasted, so understanding the rules protects the pen you have paid for.

Everyone's route differs. Some people stay at 5mg because they respond well and tolerate it, and that is a legitimate maintenance dose. Higher is not automatically better, and your prescriber will guide the pace at each review. Your monthly cost follows the dose you are on, so plan with your prescriber.

10 · Storage and travel

Storing and travelling with your pen

Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹³

For unopened pens, store them in the fridge until first use. They keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check the patient information leaflet and the pharmacy before using it, rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.¹³

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.¹³

Storage matters for cost because a spoiled pen is a pen you have to replace. When your delivery arrives, put it in the fridge unless you are starting it that day. A pen arrives in cold-chain packaging so that it reaches you within its safe range. If a delivery looks wrong, such as a pen that looks cloudy or discoloured, contains particles, or seems to have been frozen, do not inject it. Contact us first. Declined consultations are refunded automatically in full. Once dispensed, prescription medicines cannot be returned except where an item is faulty, damaged or wrong, in which case you should contact us within 3 days. Refunds take 5 to 10 working days.

Plan around your travel. Do not leave a pen in a car, in luggage in the hold, or beside a window. Because an in-use pen is discarded after 30 days (not above 30°C) regardless of remaining medicine, a fresh order timed to your schedule is easier than a rescue plan.

11 · Why prices vary

Why the same 5mg pen costs different amounts

The same 5mg pen can cost different amounts because pharmacies set their own sell prices against a shared list price. Pharmacy sell prices vary because of confidential discounts between suppliers and pharmacies.

In our index of the UK pharmacies we track (checked 26 October 2026), the cheapest starting 2.5mg pen is £143.96 and the dearest is £179.99. Across all doses, pens range from £143.96 to £319.99. For the same dose, the gap between providers runs from £20 to £36.03. The index is not split by 5mg, so use these figures as context and check each provider's own 5mg listing.

Beyond list price, the drivers of difference are the extras: whether there is a consultation fee, how delivery is charged, whether follow-up is included and how discounts are structured. A low first-order price followed by a higher repeat price changes the true cost, so always ask what month two and month six cost.
