A quick overview
What to know
before you start.
-
There is no set end date: your maintenance dose continues while it works, is tolerated and your prescriber supports it.
-
Many people settle at 5mg, 10mg or 15mg; higher is not automatically better.
-
Stopping should be planned with your prescriber, because weight can return after treatment ends.
-
The regulated option is a lower weekly dose or a planned exit.
-
Our prescribing team carries out ongoing monthly reviews, so the decision is never left to you alone.
In this guide
01 · Quick answer
How long do you stay on a Mounjaro maintenance dose?
You stay on your maintenance dose for as long as treatment is helping and is safe for you, and there is no licensed cut-off date. Mounjaro is a once-weekly medicine for long-term weight management, so for many people maintenance runs for months and often years. The licence sets a ladder for getting to your dose. It does not set a date for leaving it.1
That does not mean you are committed for life. Some people eventually stop, some step down to a lower licensed weekly dose, and some continue indefinitely. What matters is that the choice is made deliberately, with your prescriber, rather than drifting into it or stopping suddenly because a pen has run out.
If you want the dose-by-dose picture first, our guide on how long you stay on each dose walks through the whole ladder, and the maintenance dose frequency guide covers the weekly rhythm.
02 · Your decision
The decisions you are really facing
Where you are in treatment shapes the answer, and each situation calls for a different decision.
| Your situation | What usually makes sense | Who decides |
|---|---|---|
| You have reached a dose that works and you tolerate it | Stay on that dose and keep reviewing it. There is no need to keep stepping up. | You and your prescriber at each review |
| You have reached your goal and want to stop | Plan the exit with your prescriber, with diet and activity habits already in place. | You, with prescriber guidance |
| Cost is the pressure | Ask about moving to a lower licensed weekly dose. Do not stretch pens or inject fortnightly. | Your prescriber |
| Side effects are troubling you | Your prescriber can hold a dose longer or step down within the licence. | Your prescriber |
| You have lost very little at the highest tolerated dose | If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you. | Your prescriber |
Whichever row fits you, the principle is the same: changes happen within the licence and are reviewed by a clinician. Our Mounjaro duration FAQ answers the shorter questions people ask along the way.
03 · Cost
What long-term maintenance costs
Each pen holds four weekly doses, so staying on a maintenance dose means a new pen after at least four weeks, and cost is a fair part of the decision. At standard prices, 5mg is £218.99 per month, 10mg is £290.99 and 15mg is £329.99, with £279.99 at 7.5mg and £319.99 at 12.5mg. At standard prices, a first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg.
New customers save £40 on their first order with code FIRST40, and an automatic £25 reorder discount applies to every future order (code DOSE25). Every later order is £25 off the standard price. Pens are in stock across all six doses. Our maintenance dose cost guide goes further into planning.
If you are ready to start or continue, you can see the current Mounjaro prices and begin a free consultation, reviewed by a GPhC-registered prescriber. Our wider weight loss clinic page explains how ongoing care works.
04 · What maintenance means
What a maintenance dose actually is
A maintenance dose is the weekly Mounjaro strength you settle on after titration, and it is the dose you continue with rather than keep increasing. Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1
From there the dose steps up gradually. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.1
This is why the duration question has no single answer. Titration is a stepwise process, and the pace is individual. Maintenance is open-ended by design. You reach it when going higher no longer adds enough benefit, or adds side effects you would rather avoid, and from that point the question becomes how long the treatment keeps earning its place.
Maintenance is not a failure to progress
Some people feel that staying on 5mg or 7.5mg means they have not tried hard enough, and that the top dose is the real goal. It is not. The licence offers a range because people respond differently. A dose that holds your appetite steady, produces manageable side effects and keeps your weight moving in the right direction is a successful dose, whatever number is on the pen.
Our guide to the average maintenance dose explains which strengths people most often land on, and how to think about your own.
One pen, four weekly doses
Each KwikPen contains four weekly doses, so staying on maintenance means a repeat supply after at least four weeks. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Keeping that routine steady is a large part of making maintenance work.1
If you need to move your injection day, you can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.13 Beyond that, the day matters less than consistency.
Where the dose ends up is personal
Because the licence describes a ladder rather than a target, two people on the same plan can finish at different strengths and both be doing exactly what the treatment intends. Appetite, tolerability, other medicines, the pace of loss and what you want from treatment all feed into where you land. Your prescriber will talk through that at each monthly review rather than pushing you toward the top of the range.
05 · How reviews decide
How your prescriber decides how long to continue
Your prescriber decides how long you continue by reviewing your response, tolerability and goals at regular intervals, not by counting down to a date. At EverydayMeds, ongoing monthly clinical reviews, dose-titration and follow-up assessments are carried out by the prescribing team, and repeat orders re-check suitability. There is no auto-shipping, so every supply is a fresh decision.
Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4
Note what that rule is and is not. It is a prompt for a conversation, not an automatic cut-off, and it applies at your highest tolerated dose, which may not be 15mg. If you are responding, the review is simply a confirmation that continuing makes sense.
What a review usually looks at
- The trend in your weight over weeks, not a single weigh-in.
- Appetite and how well you are managing food portions.
- Side effects, and whether they have settled or are interfering with daily life.
- Other health measures your clinician is tracking alongside weight.
- Your own goals, including whether you want to hold, lose more or plan an exit.
Why trend beats the scale
Weight loss on Mounjaro is gradual by design, and trials measured results at 72 weeks. Think in months, not days, and judge progress by trend, not individual weigh-ins.4 That matters for the maintenance question too: a stable weight on a stable dose is often exactly the outcome you wanted, even though it looks like nothing is happening.
Questions worth bringing to a review
Come prepared with specific questions. Is my current dose still doing something useful? Would a lower licensed dose hold my result? What would a planned stop look like, and what habits should be settled beforehand? Are there any new symptoms I should mention? A review that starts from your questions usually produces a clearer plan than one that simply asks whether you want another pen.
Treatment on the NHS
NHS prescribing follows its own pathway and review schedule, set by NICE and the services that deliver it. NHS England publishes rollout guidance for weight management injections, and your GP or weight management service will explain how duration is handled in that route.6 The principle that treatment continues only while it is helping and tolerated applies across both routes.
06 · When you stop
What happens if you stop Mounjaro
If you stop Mounjaro, weight can return, because the medicine manages weight rather than curing the biology behind it. The clearest evidence comes from a trial built to test exactly this. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14
Those are trial averages and your own result may differ. But they explain why prescribers treat stopping as a planned step. Mounjaro manages weight; it does not cure the biology behind it. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.14
What the medicine does while you are on it
Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 When the medicine leaves, that help leaves with it, which is why appetite can feel noticeably different after stopping.
How long it takes to leave your system
Tirzepatide stays in the body for a good while. After a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 So you will not feel an abrupt change the week after your last injection; the shift is gradual, and that is the period to protect with your habits.
Building a plan before you stop
A sensible exit usually has these parts, all agreed with your prescriber:
- A decision about whether to step down to a lower licensed weekly dose first, rather than stopping from a high one.
- Eating patterns you can sustain, with protein protected and portions monitored honestly.
- Regular activity that does not depend on the medicine.
- A weigh-in routine, so drift is noticed early and discussed rather than ignored.
- A clear route back to your prescriber if weight starts creeping up.
Restarting is possible, but it is a clinical decision, not an automatic one. If you are weighing the long game, our guide on maintenance dose prescriptions in the UK explains how continuing supply works.
Stopping is a legitimate choice
Nothing here says you must stay on Mounjaro forever. It says the decision deserves the same care as starting. Some people stop successfully, and a planned approach gives you the best chance. The point is to go in with your eyes open about what the evidence shows, rather than assuming the weight will stay off by itself.
07 · Stretching and shortcuts
Can you stretch or space out your maintenance dose?
No: spacing out maintenance doses is not a licensed way to stay on Mounjaro. Mounjaro is licensed as a once-weekly medicine; its blood levels are designed around a 7-day cycle.
The idea is understandable. Once you have reached a dose that works, it is tempting to ask whether you can take less to make pens last or to reduce side effects.
What to do instead if cost or side effects are the issue
If a dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Both options keep you inside the licence, keep your dosing accurate and keep a clinician aware of what you are doing.
Moving down is not a step backwards.
Why self-adjusting is risky
- You cannot tell what blood level you are actually achieving.
- Side effects may return in waves, because levels rise and fall.
- There is no safety data for the pattern you are inventing.
- Your prescriber cannot give good advice if they do not know what you are taking.
If you have already been adjusting doses yourself, tell your prescriber plainly. They would rather know than guess.
08 · Plateaus on maintenance
Plateaus: do they mean it is time to change?
A plateau on maintenance usually does not mean it is time to stop, and it is not a reason to change your dose on your own. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1
Within the licence, the responses are: review food intake honestly (portions drift back up), protect protein and activity, check the basics (sleep, alcohol), and discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.1
Plateau versus maintenance
It helps to separate two things that look alike on the scale. A plateau is a pause while you still want to lose more. Maintenance is a deliberate hold at a weight you are happy with. Both show a flat line, but they call for different responses. If you still have weight to lose, a plateau is worth discussing at your next review. If you are at your goal, a flat line is the point.
Early expectations
Slow early progress does not predict failure. Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. A sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation, and the dose you respond to may still be ahead of you.2
The headline trial figures, in context
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
For someone starting at 100 kg, 20.9% is about 21 kg. Treat that as a worked example of arithmetic, not a forecast for you.
Checking the basics first
Before blaming the dose, run through the simple things honestly. Have portions crept up? Is protein still prioritised? Has activity slipped? Is sleep suffering, or alcohol creeping in? These everyday factors often explain a stall better than the medicine does, and fixing them is free. If they check out and you are still below the maximum dose, raise a possible step-up with your prescriber instead of deciding alone. For higher-dose questions, see how long you can stay on 15mg.
09 · Why people stay long term
Why many people stay on Mounjaro long term
Many people stay on Mounjaro long term because the benefits are tied to taking it, and because weight management is a long-running health goal rather than a short project. Mounjaro is a prescription medicine used alongside a reduced-calorie diet and increased physical activity, and the results you hold depend on all three continuing.
Weight is only part of the picture
In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises — but they are why prescribers talk about weight management as health management rather than appearance.
That framing changes the duration question. If the goal were a number on the scales, stopping at the number would make sense. If the goal is a healthier long-term profile, you may want to keep the treatment while it supports that, in the same way people stay on other long-term medicines that manage a condition.
Efficacy and response
At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Again, these are trial figures and individual results vary. They show that benefit builds over a long period, which is part of why treatment is not designed as a few weeks of intervention.
Who might reasonably stop
Some people are good candidates for a planned exit: those who have reached their goal, built firm habits and want to test life without the medicine, under supervision. Others prefer to hold a lower dose. Others still keep going at a steady dose because it works and they tolerate it. All three are legitimate, and none is a moral choice.
The role of a clinical relationship
What matters is not picking the right answer at the start, but keeping a clinician involved as your situation changes. Weight, health, life events and finances all shift over the years. A regular review gives you a place to reassess calmly, so decisions about staying, stepping down or stopping are made with information rather than panic. For the practical day-to-day of long-term use, see the monthly cost guide for maintenance.
10 · NHS and private routes
NHS and private routes to staying on treatment
Both NHS and private routes can supply Mounjaro for weight management, but access and pace differ. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
What this means for duration
If you are treated privately, your duration is decided between you and your prescriber, with cost as a practical factor. If you are treated on the NHS, the service sets how long it supports you and how it reviews you. Either way, the clinical principle is the same, and you should never be left to stop or adjust alone.
Private supply with clinical oversight
We are a GPhC-registered UK online pharmacy (registration 9012878). Mounjaro is supplied through official LillyDirect supply, direct from Eli Lilly's UK distribution, and every order starts with a free online consultation reviewed by a GPhC-registered prescriber. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
Aftercare includes ongoing monthly reviews, dose-titration and follow-up assessments, and side-effect support. Repeat orders re-check suitability, so there is no auto-shipping and no subscription lock-in. Delivery is free next-day tracked, in temperature-controlled packaging, with same-day dispatch when ordered by 3pm.
Planning cost across the long run
If you expect to stay on maintenance for a long period, plan around the monthly price of your dose. Standard prices are £218.99 at 5mg, £290.99 at 10mg and £329.99 at 15mg. New customers save £40 on their first order with code FIRST40, and an automatic £25 reorder discount applies to every future order (code DOSE25). Klarna and pay-monthly options are available.
If cost becomes the obstacle, raise it early with your prescriber. A lower licensed dose may be an option, and that is a better conversation than quietly rationing pens. Our 15mg maintenance buying guide covers higher-dose supply specifically, and the diabetes maintenance dose page covers that licensed use.
11 · Staying consistent
Staying consistent on maintenance: missed doses, travel and storage
Staying consistent on maintenance comes down to a few practical rules, and the licence is clear on each. The most common slip-up is a missed dose, so start there.
If you miss a dose
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.13 One missed injection does not undo your progress, and you should not try to make up for it by doubling.
Choosing your injection time
There is no "best" time of day — Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember (many people choose a weekend morning so early side effects fall on days off) and stick to it.13
Where to inject
Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.13 Over a long maintenance period, rotating sites becomes a habit worth protecting.
Storage over a long run of pens
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13
Travelling while on maintenance
You can fly with Mounjaro. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.13
Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit — figures circulating online often come from other countries' labels.13
Reporting anything new
If a new symptom appears during long-term use, discuss it with your prescriber, and you can report suspected side effects through the MHRA Yellow Card scheme.19 Our support team is available Monday–Friday 9am–5pm on 0333 207 0413 or at support@everydaymeds.co.uk, and the free GLPTrackr app can help you keep a record of doses and weight over time.
12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
How ordering works
How ordering works at EverydayMeds
