02 · What to decide

The two decisions most people face

The first decision is whether you carry your pen or pause treatment. Carrying it is straightforward for almost everyone, and it keeps your routine intact. Pausing is possible, but it is a clinical choice that belongs with your prescriber.

The second decision is which pen goes with you. If you are mid-pen, take that pen. What to pack for your Mounjaro pen

Pack your pen in your hand luggage, inside a cool bag, with the prescription label visible. Hold luggage can fall below freezing, and a frozen pen must never be used.1

  • The pen in its original box, with the pharmacy label attached, so the batch number and expiry date are easy to show.
  • Your prescription label or a copy of your prescription, in case anyone asks what the pen is.
  • A cool bag or insulated pouch for hot destinations. It is a small accessory purchase and worth having.
  • Needles and a sharps bin. Needles, a sharps bin and injection guidance are included with every order, so check you have enough for the length of your trip before you leave.

Keep the pen somewhere you can see it. Do not leave it in a parked car, on a sunny windowsill, or in a beach bag in direct sun. Heat is the realistic risk on holiday, and the limit is not above 30°C.

04 · Flying and security

You can fly with Mounjaro, and hand luggage is where the pen belongs.1 Airlines and airports set their own procedures, so check the current rules on your airline's and airport's websites before you travel.

The practical approach is to be open and prepared. Tell security staff you are carrying a prescription injectable pen. Keep the pharmacy label on the box, and have your prescription or a copy of it to hand. If you are asked to take the pen out of its bag, that is normal and it takes moments.

Do not put the pen in the hold, even in a padded case. Hold luggage can fall below freezing, and a pen that has frozen must not be used.1 If a pen is damaged or you are unsure after a long journey, contact the pharmacy before injecting.

05 · Heat and storage

Keeping the pen safe in the heat

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them.1 Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, which is useful for short trips, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen.13

For a typical holiday, that means your in-use pen can live in your room or bag. Use the hotel fridge if there is one, but do not let the pen sit against the cooling element where it could freeze. If a hotel fridge is not an option, an insulated pouch is the answer for hot climates.

Unopened pens

Unopened pens should be kept in the fridge, and they keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and they are the wrong guide for a UK pen.1

Before you inject abroad

Look at the pen first. A genuine pen's solution is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use.1 If anything seems wrong, contact the pharmacy before injecting.

06 · Pausing for a holiday

Can you take a break from Mounjaro for a holiday?

You can ask your prescriber about a break, but it is a clinical decision and not a casual one. If you are tempted, speak to the prescribing team first.

It helps to understand how the medicine behaves. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 A missed week therefore does not mean the medicine disappears overnight.

A holiday is a period of changed routine, so planning meals and activity matters whether or not you inject. Appetite can return as the medicine clears, and weight changes after stopping are a topic for your prescriber to discuss with you, based on your own history.

07 · Injection day abroad

Injection day, time zones and meals abroad

Plan where your injection day falls before you leave. If it lands mid-flight or during a long travel day, ask your prescriber how best to handle it rather than improvising.

On the practical side, pick a calm moment and a clean surface for the injection, follow the instructions that come with the pen, and dispose of the needle in your sharps bin. Never share a pen.

Holiday eating can be a challenge on this treatment. Mounjaro helps regulate appetite and slows how quickly the stomach empties, so most people feel fuller sooner and less hungry between meals.1 Rich food, large portions and alcohol can make you feel unwell, so smaller, slower meals tend to work better. If you notice stomach upset, eat smaller meals slowly and stop at the first feeling of fullness. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention. Severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis), pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems), or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing need immediate medical help.

Stay hydrated, especially in the heat. If you experience any new symptom you are unsure about, tell your prescriber and report it through the Yellow Card scheme.19

08 · Insurance and rules

Do you have to declare Mounjaro on holiday insurance? That depends on your insurer and how they define the conditions you are treated for. Ask your insurer directly, and tell them honestly about your health and your medication. Getting this wrong can affect a claim.

Destination rules matter too. Check destination rules for travelling with prescription injectables before you fly.1 Some countries have specific requirements for medicines and needles, and they change. The embassy or official government travel advice for your destination is the right place to look, not a forum.

Before you go, also check that your supply will last. If a repeat order falls due while you are away, order early. You can stop, but speak to your prescriber first. After a final dose the medicine takes roughly four to five weeks to clear, there is no way to speed this up, and nothing needs doing while it clears. Treatment works alongside diet and activity, so plan those too, and ask the prescribing team what to expect.
Is it normal to gain weight after stopping Mounjaro?
Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
What should I avoid while taking Mounjaro on holiday?
Avoid freezing the pen, leaving it in direct sun or a hot car, and packing it in the hold.
