Can you carry Mounjaro in hand luggage?
Yes. Mounjaro is a prescription injectable medicine, and travellers with prescribed medicines routinely carry them in the cabin. Hand luggage is the place we advise for your pens, because you stay in control of them from your front door to your destination.
There are three practical reasons. First, a checked bag can be delayed, rerouted or lost, and a missing pen means a missed dose far from your prescriber. Second, you cannot control conditions in the hold, while the cabin is where you can keep an eye on your bag. Third, you can show your medicine to security staff straight away if you are asked about it.
02 · Your next step
What should you do before you travel?
Check three things before you pack: your supply, your airline and your destination. Do you have enough pens to cover the whole trip, plus a sensible buffer in case your return is delayed? Does your airline have a policy on medicines, injectables or medical cool bags in the cabin? Does your destination have any rules on bringing in prescription medicines or needles?

03 · Hand luggage vs hold
Hand luggage versus checked luggage for Mounjaro
Hand luggage is the better option, and the hold is the one to avoid. Mounjaro pens that have not been used yet must be stored in the fridge until first use, and they keep until the printed expiry date.1 A checked bag gives you no way of looking after that storage, because you cannot reach it during the journey.
If you want the case against the hold, read putting Mounjaro in hold luggage and putting Mounjaro in checked luggage. In short, a bag you cannot reach is a bag you cannot protect, and a lost suitcase can mean lost medicine.
The hold is not the only risk. Bags can be delayed after a connection, left behind at the wrong airport or opened for inspection. None of that matters much for a pair of trainers. It matters a great deal for a prescribed medicine that you cannot simply replace on holiday, because Mounjaro is a prescription-only medicine supplied after a consultation.
A sensible rule is to treat your pens like your passport. They go in the bag that stays with you, and they do not leave your sight unless there is no alternative.
04 · Airport security
Going through airport security with Mounjaro
You can take Mounjaro through airport security, and the process is normally straightforward when you are prepared. Medicines and the equipment needed to take them are generally treated differently from ordinary liquids, but the detail depends on the airport and the country, so check the guidance for your departure and arrival airports before you go.
When you reach the screening point, tell the staff you are carrying a prescription injectable medicine, and take the pens out of your bag if asked. Medical items are often screened separately from the rest of your belongings. If a cooling pack is with the pens, mention it at the same time. Being open at the start usually speeds things up, and staff deal with injectable medicines every day.
Do not put the pens loose in a pocket or an unmarked pouch. A medicine with no label is hard to explain. A pen in its box with your pharmacy label is easy to explain.
Many travellers ask whether they must declare medication in hand luggage. The safest approach is to mention it proactively at security and at any border check where you are asked, and to follow the instructions of the staff. Rules differ between countries, so confirm the destination's requirements before you travel, particularly for needles and prescription injectables.
05 · Keeping pens cool
How do you keep Mounjaro cool when travelling?
Keep pens that have not been used yet at fridge temperature, 2–8°C, until you start them.1 For a journey, that means a medical cool bag or insulated case with a cooling element, packed in your hand luggage so you can reach it. Keep the pens in their original box inside the case, and avoid letting them sit directly against a frozen pack, because a pen that touches ice can be exposed to colder conditions than the fridge range intends.
Once a pen is in use, the rules change. The UK KwikPen may be kept out of the fridge for up to 30 days (not above 30°C) once you have started it.13 That gives useful flexibility on a trip, but it does not extend to unopened pens. For an unopened pen that has been out of the fridge, read the patient information leaflet and ask the pharmacy before you use it.
If you are asking how long Mounjaro can go unrefrigerated, the honest answer is that it depends on whether the pen has been started. In-use pen: up to 30 days (not above 30°C). Unopened pen: store in the fridge until first use, and check the leaflet and ask us if it has been left out.
At your destination, put the pens in the room fridge or hotel mini fridge if you have one. Check that the mini fridge is actually cold, because some are not, and keep the box where it will not be moved or tampered with. If there is no fridge, ask the hotel whether they can store a medicine for you.
Also think about the journey to the airport. A pen left in a hot car, or beside a radiator or window in direct sun, is not being stored properly. Keep the cool bag with you, in the shade.
06 · Paperwork and labels
Do you need a doctor's note to carry Mounjaro?
Requirements for a doctor's note vary between airlines and countries. What helps every time is being able to show that the medicine is yours and that it was prescribed. That starts with the original box and the pharmacy dispensing label, which carries your name and the medicine details.
It is sensible to carry a few extras, kept together in a travel folder:
- The original box for every pen, with the label visible.
- Your dispensing paperwork or order confirmation from the pharmacy.
- A short letter from your prescriber if your airline or destination asks for one.
- A list of your medicines, in case you need medical help abroad.
If your airline or destination asks for a letter, ask your prescriber well before you leave. Do not leave it to the last day, because prescribers cannot always respond immediately. Do not decant pens into a pill organiser, an unmarked bag or any other container. A pen away from its labelled box is harder to identify and may be harder to explain to staff.
07 · Needles and sharps
Travelling with needles and sharps
Pen needles are needed to inject, so pack them with your pens, in your hand luggage. Needles, a sharps bin and injection guidance are included with every order, so you will already have what you need for your normal routine.
For a trip, work out how many needles you will need for the whole stay, with a few spare in case one is damaged or dropped. Keep them in their packaging and in the same bag as the pens, so you can show that they belong to your treatment. Do not put loose needles in a pocket.
Plan for disposal as well. Used needles should go into a sharps container, never into ordinary rubbish. If you cannot take a full-size sharps bin, ask your pharmacist about a travel option, and check how to dispose of used needles safely at your destination. Bring any used needles home in a proper container if you cannot dispose of them safely where you are staying.
Do not share pens or needles with anyone, even a travelling companion on the same medicine. Your pen is for you alone.
08 · Dose day and timing
Planning your injection day around a trip
Mounjaro is a once-weekly injection, so most trips are best planned around your usual dose day. Before you travel, work out where you will be on that day and whether you will have somewhere private, clean and cool to inject. Many people simply take their injection at home before they leave or on return, but whether that suits you is a question for your prescriber.
Do not change your dose, double up or make up your own schedule to fit a holiday. Changing the timing or amount of a dose is a decision for your prescriber, who can look at your own dose, your own progress and the length of your trip. Your treatment works alongside a reduced-calorie diet and increased physical activity. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. A holiday does not cancel that plan, but it does tend to test it, so think ahead about meals and activity while you are away.
Keep an eye on how you feel in unfamiliar places. If you notice a new symptom while travelling, tell your prescriber, and you can report suspected side effects through the MHRA Yellow Card scheme.19
09 · Insurance and abroad
Travel insurance and taking weight loss jabs abroad
Check your travel insurance policy and tell your insurer about your treatment if the policy asks about medical conditions or regular medicines. If you have a condition that is being treated, a declaration made in good faith protects you better than silence, so read the small print and ask the insurer directly.
Taking weight loss jabs abroad is possible for many destinations, but each country sets its own rules on importing prescription medicines and needles. Check the official guidance for your destination, and contact the embassy or the relevant authority if the answer is unclear. Carry your medicine in its original labelled packaging at all times.
Do not try to buy a replacement abroad from an unregulated seller. Counterfeit weight loss pens are a recognised risk, and the MHRA publishes counterfeit medicine alerts.7 Beware of anyone offering to supply without a consultation or prescription. 10 · If plans change
What if something goes wrong while you travel?
Most problems have a simple answer if you keep your pens in your hand luggage. If a flight is delayed, you still have your medicine and your cooling solution. If your bag is searched, your labelled box does the explaining for you.
If you think a pen has been exposed to the wrong conditions, do not guess. Stop, read the patient information leaflet and contact the pharmacy or your prescriber. Do not use a pen if you are unsure of how it has been stored. The unopened pen should be in the fridge until first use, within the range of 2–8°C, and it keeps until the printed expiry date.1
If a pen is lost, damaged or confiscated, contact your prescriber or pharmacy for advice rather than improvising. Do not use someone else's pen, and do not stretch or adjust your doses to cover a gap. Dispensed prescription medicines cannot be returned, except for a faulty, damaged or wrong item, which you should report within a few days of delivery.
