Yes. You can fly with Mounjaro, and the rule that matters most is to carry the pens in hand luggage rather than checked baggage, because hold luggage can fall below freezing.13 A pen that has frozen must not be used, so the hold is the one place your pens should never go.1
Ideally the pens travel in a cool bag, together with the prescription label or a copy of your prescription.
02 · Before you go
What to decide before you fly
Two decisions matter before departure: which pens to bring, and whether your injection day falls during travel. The table sets out the common situations.

03 · Packing your pens
Packing Mounjaro in hand luggage
Pack pens in your cabin bag, in their original box, with the pharmacy label visible. The original packaging helps anyone checking your bag see at a glance that this is a prescribed medicine, and it protects the pen from knocks and light.1 Keep the pen cap on to protect the medicine from light.1
A cool bag is the sensible default. Carry the pens ideally in a cool bag with the prescription label or a copy of your prescription.13 Keep the pen away from direct contact with ice packs so that it cannot freeze, because a frozen pen must never be used.1

Do not put a spare pen in checked baggage "just in case". Hold luggage can fall below freezing, and you cannot check on it mid-flight.13 If your cabin bag is gate-checked at a full flight, take the pens out and carry them with you in a small bag before handing it over.
04 · Security and paperwork

Carry your prescription label or a copy of your prescription with the pens, and be ready to tell security staff you are carrying a prescribed injectable medicine.13 Rules on screening, cool packs and medical items differ by airport and airline, and they change. Check your airline's and airport's published guidance before you travel rather than relying on a general rule.
Keeping everything in one clear pouch, easy to pull out of your bag, also makes screening quicker for you and the staff.
Finally, check destination rules for travelling with prescription injectables before you fly.13 Some countries have specific requirements for importing prescribed medicines. The relevant embassy, consulate or national health authority website is the most reliable place to look.
05 · Keeping pens cool
Keeping Mounjaro cool while travelling
Unopened pens are stored in the fridge at 2–8°C and must never be frozen.1 Once a pen is in use, it can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded.1
That in-use allowance is why most holidays need no special arrangements. The pen you are currently injecting from can sit in your bag, in a hotel room or in a cool pouch without a fridge. For hot climates, use an insulated pouch.13 Do not leave pens in a hot car, on a sunny windowsill or on a beach towel.
For unopened spare pens, keep them in the fridge until you leave, then carry them cool. At your destination, use the hotel or rental fridge if there is one. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and the UK product information is what applies to your pen.13

06 · Injection day and time zones
Injection day, time zones and missed doses
Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 Because the time of day does not matter, crossing time zones is rarely a problem: the aim is simply to keep the same day each week.
If your usual day falls awkwardly on a travel day, you do have flexibility. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Plan any change in advance and keep a note of when you last injected. If you do miss a dose while travelling, the rule is clear. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1
Do not try to "stretch" doses to make a pen last through a trip, or split doses: altering doses outside the licence means unknown dosing accuracy and no safety data.
07 · Insurance and destination rules
Travel insurance and destination rules
Tell your travel insurer that you take Mounjaro, and check how they treat the condition it is prescribed for. Policies differ widely in what they ask you to declare, so the only reliable answer comes from your own insurer in writing.
It is better to declare too much than too little, because a claim can be affected by something left out. If you are travelling for a long period, ask the insurer whether they need to know about ongoing prescription treatment and any associated medical history.
For the destination itself, check the rules for travelling with prescription injectables before you fly.13 Look at the official site for the country you are visiting and at any countries you transit through, since some require paperwork even for a short stopover.
If you are covered by an airline's special assistance service, it can also be worth telling the airline in advance that you carry medical items in the cabin.
08 · Planning supply

Work out how many weekly doses the trip covers and make sure you have them before you leave.

09 · Feeling unwell away from home
Side effects while travelling
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If you are new to treatment, think about whether your injection day sits just before a long journey, and shift it if that makes things easier, as long as at least 3 days (72 hours) have passed since your last dose. Many people prefer a weekend morning so early side effects fall on days off.
If you notice a new symptom while away, tell your prescriber. You can also report any suspected side effect through the MHRA Yellow Card scheme.19

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
