What to do before you fly
Most of the preparation takes a few minutes. Work through this list the week before you travel:
- Pack the pen in its original box so the pharmacy label stays attached to it.
- Carry the prescription label or a copy of your prescription, ideally alongside the pen.1
- Use a cool bag to protect the pen from heat and from freezing.
- Check your destination's rules for travelling with prescription injectables before you leave.1
- Pack your needles and sharps bin plan, because you will need a safe way to dispose of used needles abroad.
- Plan your injection day. Mounjaro is once weekly, so decide whether your dose falls before, during or after the trip.

03 · Why not the hold
Why Mounjaro should not go in the hold
Mounjaro pens must be kept in a fridge at 2–8°C and must never be frozen.1 Aircraft holds are not a reliable environment for that. Hold luggage can fall below freezing, and a pen which has been frozen must not be used.1
That makes the cabin the only sensible option. Under your seat or in the overhead locker, the pen stays in conditions you can monitor.
The same logic applies to parcels and luggage forwarding services. If a pen is going anywhere unattended, you cannot know what temperature it will meet. Keep it with you.
04 · Packing and cooling
Packing and keeping pens cool
A cool bag with the pen inside its original box is the simplest set-up. Keep the pen cap on, because the cap protects the pen from light.1
Choose a bag that is insulated and soft enough to fit under a seat. A small medical-style pouch is a modest accessory purchase and does the job for most trips. For hot climates, use an insulated pouch rather than leaving the pen loose in a bag.1

Do not leave the bag in a parked car, on a sunny windowsill or in a beach bag.
05 · Security and paperwork
Airport security, labels and declaring medicines
Carry the prescription label or a copy of your prescription with the pen so staff can see the medicine is yours.1 Keeping the pen in its original box with the pharmacy label visible makes any conversation at security short.
Do you have to declare it? Rules on liquids and medicines at security are set by each airport, airline and country, and they change. Check the current guidance from your departure airport, your airline and your destination before you travel, and tell the security officer at the start that you are carrying an injectable medicine so they can follow their own process.
Expect that a pen may be examined or that you may be asked a few questions. Staying calm, having the label to hand and keeping the medicine separate from the rest of your toiletries makes this easy.
Medicines that are restricted differ from country to country. Mounjaro is a prescription-only injectable, so check the destination's rules for prescription injectables in advance.1 If you are unsure, your embassy or the destination's official health or customs pages are the right place to look.
06 · Holidays and heat
How long a pen lasts on holiday
Once you have started using a pen, it can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 That is why most holidays need no special arrangements: a pen already in use will see you through a typical trip.
Unopened pens are different. They should stay in the fridge and keep until the expiry date printed on the box.1 If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a day limit you remember. Figures circulating online often come from other countries' labels, and the UK pen is licensed differently.1

07 · Check before injecting
Checking your pen after the journey
Look at the pen before every injection, especially after travel. A genuine UK pen contains a solution that is clear and colourless.13 Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use.13
If a pen looks wrong, do not inject and do not guess. Contact the pharmacy that supplied it. If supply or packaging ever seems wrong, you can also check the pharmacy on the GPhC register; ours is GPhC 9012878.12
This matters more when you are abroad, because replacing a damaged pen is harder. It is another reason to carry the pen yourself and keep it protected.
08 · Travel and side effects
Travel, timing and how you feel
Plan your injection day around your travel rather than the other way round. Mounjaro is a once-weekly injection, so keep to the same day each week; if you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, otherwise skip it and take the next on your normal day.
Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines, so talk to them before travelling.

Check your travel insurance policy wording and tell your insurer about your health conditions and medicines if the policy asks for it. Read the policy or phone them before you buy.
