30 units of tirzepatide equals whatever amount of drug sits in that volume of liquid, and the number cannot be known without the vial concentration. A unit is a marking on an insulin-type syringe. It is a fixed fraction of a millilitre. It tells you how much liquid you have drawn, not how much tirzepatide is dissolved in it.

A weaker vial gives fewer mg for the same unit count, and a stronger vial gives more. That is why a quoted "30 units" is meaningless on its own. Two people drawing the same unit count from different vials can receive very different doses.

03 · Units versus milligrams

Units versus milligrams explained

Units and milligrams measure different things, so they cannot be swapped like pounds and kilograms. A unit is a volume marking. A milligram is a weight of active ingredient. Converting between them always needs a third piece of information: how concentrated the liquid is.

Think of a cup of squash. A cup is a volume. How much sugar is in it depends on how strong the squash is made. The same cup can hold very little or a great deal. A syringe works the same way. The markings tell you how full it is, and the concentration tells you what is inside.

Units come from insulin practice. Insulin syringes are marked in units because insulin is itself dosed in units and sold at a standard concentration. Tirzepatide is not dosed in units. Its licensed dosing is in mg. Any unit figure quoted for tirzepatide is therefore someone's own conversion, and it is only as reliable as the concentration behind it.

That is why 30 units of tirzepatide seems to have several different mg answers online. Each answer silently assumes a different vial. None of them can be applied to your vial unless your label matches the assumption exactly.

04 · Why Mounjaro pens skip units

Why the Mounjaro KwikPen never uses units

The Mounjaro KwikPen is a pre-filled pen that you set by dialling the dose, so no syringe, units or arithmetic is involved. You turn the dose knob until the indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks.1

Each pen contains four weekly doses. The pen will not let you dial past the medicine left inside. If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.1 That is a built-in safeguard that a syringe and vial do not have.

This design is a strong reason to treat unit-based instructions with suspicion. A pen cannot be over-drawn. A syringe can. The pen's strength is printed on it, so what you dial is what you get.

05 · Standard doses and titration

The licensed dose ladder in mg

The licensed Mounjaro dose ladder is expressed in mg and moves in set steps, which is the language your prescriber will use. Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1

After at least 4 weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1

This is useful to remember when you meet unit-based claims. If a quoted unit instruction seems to jump about, it is likely to rest on a mistaken concentration rather than on how the ladder works.

Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

06 · Dose adjusting and microdosing

Unit-based "microdosing" is not a licensed route

Taking smaller or more spread-out doses than the pen is designed to deliver means unknown dosing accuracy and no safety data.1

The licensed ladder already starts low, at 2.5mg, for tolerability. If side effects are the worry, your prescriber can hold a dose for longer or step down within the licence.

If you are tempted to adjust dosing yourself because a unit chart suggested you could, speak to your prescriber instead. They can change the plan within the licence, and they can document the change so that your care stays consistent.

07 · Safe sourcing

Where unit-based tirzepatide instructions come from

Unit-based tirzepatide instructions usually accompany vials sold outside the licensed UK supply chain, and they carry risks a KwikPen does not. A vial that needs mixing and drawing up leaves room for errors in concentration, volume and sterility. The product itself may also not be what the label claims.

The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected side effects and falsified products.7 Treat any seller offering to supply without a consultation or prescription as a warning sign. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
