01 · The core answer

How many mg is 10 units of tirzepatide?

There is no single answer, and for a UK Mounjaro pen the question does not really apply. Mounjaro is the brand name for tirzepatide, a once-weekly injectable made by Eli Lilly, supplied as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Your prescription is written in mg, and the pen is built to deliver that dose.

A "unit" belongs to a different system. On an insulin syringe, units mark a volume of liquid. How many mg sit in that volume depends on how strong the liquid is, and that differs from product to product. Without the concentration of the specific product in your hand, nobody can honestly turn units into mg. That is why a fixed answer such as "10 units equals a certain number of mg" is unreliable wherever you see it.

The same goes for "10 clicks". The KwikPen is not a click-counting device. The dose is confirmed by the number showing in the window.1

03 · Units versus mg

Why units and mg are not the same thing

Milligrams (mg) measure the weight of medicine. Units, when printed on an insulin syringe, measure how far the plunger is drawn, which is a volume. The two are linked only through concentration: how much medicine is dissolved in each unit of liquid.

That is why the question "is 10 units 1 mg?" has no universal answer. It could be true for one concentration and badly wrong for another. The same applies to "is 2.5mg equal to 25 units?" and "is 10 units the same as 1 mL?".

The Mounjaro KwikPen sidesteps this entirely. The prescribed strength is the dose, and the pen delivers it. Your prescriber works in mg, the label is in mg, and the patient leaflet describes the dose in mg.13 Nothing in that chain involves units.

Insulin is where the confusion often begins. People who have seen insulin dosing, or who follow social-media posts about syringes, carry the idea of units over to tirzepatide. For Mounjaro it is a category error. If you also take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines. Never rely on a unit-based instruction written for another product.

04 · How the pen dial works

How the KwikPen dial and clicks work

The Mounjaro KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window. You confirm the dose by the number in the window, never by counting clicks.1 That is the direct answer to anyone asking how many mg 10 clicks of Mounjaro is: clicks are not a dose measurement.

The pen also has built-in protection. Each pen contains four weekly doses; the pen will not let you dial past the medicine left inside. If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.1

In practice this means you do not need to calculate anything. Take the pen out, attach a new needle as the instructions show, dial to the strength on your label, confirm the number in the window, and inject as the patient information leaflet describes. If the number you see does not match your prescription, stop and ask.

05 · Why not use a syringe

Why drawing up a pen with a syringe is unsafe

Using a syringe to pull medicine out of a KwikPen, or to measure a number of units, is not how the product is designed, validated or licensed. The pen is a sealed delivery system built to give a set dose in mg. Taking it apart or bypassing it means unknown dosing accuracy, with no safety data behind it.

The same position applies to "microdosing", meaning smaller or more spread-out doses than the pen is designed to deliver. It is not licensed, not supported by the pen's design and not endorsed by any UK regulator. The licensed dose ladder already starts low (2.5mg) for tolerability, and your prescriber can hold a dose longer or step down within the licence if side effects are the concern.1 Altering doses outside the licence means unknown dosing accuracy and no safety data.

There is also a supply-safety angle. Anyone offering tirzepatide as a vial to be measured in units, without a consultation or prescription, is outside the regulated route. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected problems.7 The MHRA has seized thousands of fake weight-loss pens in the UK, and fake pens have caused hospitalisations. 06 · The licensed dose ladder

If your real question is "what dose am I on, and what comes next?", the answer is in mg. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.111

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1

If you are prescribed 10mg, you dial the pen to 10mg in the window. That is a dose in milligrams, and it does not translate into a unit count on a syringe.

07 · Injecting and missed doses

Injecting correctly and handling a missed dose

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 There is no best time of day: Mounjaro works the same morning or evening. What matters is the same day each week.

None of these rules uses units. If a guide mixes them in, it is describing a different product or a non-standard way of using it.

08 · Storage and travel

Storing and travelling with your pen

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit; figures circulating online often come from other countries' labels.13

That last point is a good example of why unit-and-day folklore causes trouble. Numbers copied from another market's label, or from a product that is not the UK pen, can be wrong for yours.

09 · Realistic expectations

What dose changes can and cannot do

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days.1

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Those results came from the licensed doses given as designed. They say nothing about measuring doses in units or splitting them. Your prescriber reviews your response, and if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.1

Never assume a conversion you have read online. For Mounjaro, rely on the mg dose on your prescription and the number in the pen window.
What is 10 units on an insulin syringe?
It is a volume marking: a small measured amount of liquid, not a dose in mg. How much medicine that holds depends on the concentration of what is in the syringe. Insulin syringes are designed for insulin, and using one to measure Mounjaro from a KwikPen is not how the pen is designed or licensed.
Is 10 units the same as 1 mL?
No. A unit on an insulin syringe is only a fraction of a millilitre, so a millilitre corresponds to many units. Even so, converting volume to mg still needs the concentration of the specific product. This is another reason not to translate Mounjaro doses into syringe units yourself.
