The core answer: units are volume, mg is strength

The question has no single number as its answer. A unit on an insulin-type syringe is a marked slice of liquid volume. Milligrams describe how much active drug is dissolved in that liquid. To go from one to the other you need the concentration of the liquid in the syringe, which only the label of that specific vial can tell you. Two vials of the same drug can contain very different strengths, so the same 20 units can carry very different amounts of medicine.

That is why a quick online answer such as "20 units equals a set number of mg" is unreliable. It silently assumes a vial strength. If the assumption is wrong, the dose is wrong, and with an injectable medicine that matters.

Is 20 units of tirzepatide therefore 2.5mg? Not necessarily. It could be more or less, depending on concentration. 2.5mg is the licensed starting dose of Mounjaro, but that dose is delivered by the pen, set as a number in a window.1

Mounjaro does not use units or clicks at all

04 · Why units are not mg

Why units are not milligrams

Syringe units are a measure of how much liquid you draw up, not how much drug is in it. Think of a measuring jug marked in millilitres: it tells you the volume of squash, but nothing about how strong the squash is. A glass drawn from a concentrated bottle and a glass drawn from a diluted bottle look identical, yet carry very different amounts of flavour. Milligrams are the strength, units are the glass.

Insulin syringes were designed around insulin, which is sold at a standard concentration. In that one setting a unit has a consistent meaning, which is why the scale exists. Tirzepatide is a different molecule and is not insulin. When someone uses an insulin syringe for another drug, the unit markings stop meaning anything about the drug amount. They only tell you how far the plunger has moved.

To convert, you would need three things: the volume drawn up, the concentration of the liquid, and certainty that both are correct. The concentration must come from the label of the actual vial in your hand, not from a website. Only then does multiplying volume by concentration give a milligram amount. Without the concentration, any single mg answer to the question about 20 units is a guess presented as a fact.

The KwikPen was designed so that nobody has to calculate anything. The pen holds a fixed strength of medicine and delivers a dose you choose by turning a dial. The rule is plain: the Mounjaro KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window — you confirm the dose by the number in the window, never by counting clicks.1

That matters because "clicks" is a word people borrow from other injection pens. Clicks can vary in feel and are not a reliable measure. The window is. If your prescriber says your dose is a particular mg strength, you dial until that number is showing, check it again, and inject. There is no step where you convert between scales.

Each pen contains four weekly doses; the pen will not let you dial past the medicine left inside.13 This is a built-in safeguard. You cannot accidentally take more than the pen holds. It also means that if you reach the end of a pen, the dial simply stops.

If the window will not turn to a full dose, the pen does not contain enough for one and should not be used for a partial injection.1 Do not try to top up from another pen, and do not try to stretch the remainder. Contact the pharmacy that supplied the pen if you think something is wrong with it.

The pen is a multi-dose device, and it is dispensed in a strength matched to the dose your prescriber has agreed. The strengths available are the six already listed: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. If you are used to vials, this is a real change. There is no drawing up, no choosing a needle gauge to match a syringe, and no concern about air bubbles skewing a volume. For many people this is the main practical reason pens feel safer. The dose you read is the dose you get.

Every UK Mounjaro dose is stated in milligrams, and the ladder is short enough to remember. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg — higher is not automatically better, and your prescriber will guide the right pace.11

The ladder does not run in units, so there is no unit equivalent to look up for any step. The question of how many units is 10mg, for example, only makes sense if you already know the concentration of a particular vial. For a KwikPen, 10mg is what you dial, nothing more.

The slow pace is deliberate. The first weeks on the lowest strength let your body adjust, and moving up only after enough time helps tolerance. Moving faster than the licence allows is not supported, and the ladder is a minimum interval, not a target. Your prescriber can hold you at a dose for longer if that suits you better.

People who have been prescribed a different brand or a vial sometimes ask whether they can "translate" their dose when moving to a pen. The safe route is to tell the prescriber exactly what you were taking, in mg and with the label concentration, and let them decide the starting point. Do not convert by yourself from a unit chart.

07 · Why charts mislead

Why unit charts and calculators mislead

A calculator is only as reliable as its input. It cannot tell you whether the vial you have is the strength you typed. It cannot tell you whether the product is genuine, whether the contents were prepared correctly, or whether a label was copied accurately. It just multiplies. For an injectable medicine, a small input error becomes a large dose error.

Another problem is that charts blend several different products. A table headed with tirzepatide may sit beside rows for other drugs prepared differently, and a reader skimming for a quick answer may take a number from the wrong row. Charts also tend to treat a dose as a simple number to be reached, when in practice the right dose is decided by a prescriber, based on how you respond and tolerate treatment.

There is also the matter of what the chart is selling. Some exist to push vial products or compounding services, and have an interest in making the maths look easy. A page that tells you 20 units is a specific mg without asking about the vial is making a claim it cannot support.

The Mounjaro KwikPen sidesteps all of this, which is the central point of the product design. The pen is licensed, the strength is printed on it, and the dose is read from the window.1 If a chart contradicts your pen window or your prescriber, the chart loses.

The safety position on altering doses is clear. 08 · Injecting and storing

Injecting and storing the pen correctly

Using the pen well is mostly routine, and it helps protect against dosing mistakes. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1 Follow the pen's own instructions for the injection steps.

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. There is no best time of day: Mounjaro works the same morning or evening. What matters is the same day each week, and many people choose a weekend morning so early side effects fall on days off.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other. You can also change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

Storage is straightforward. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13

For travel, you can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements, and for hot climates an insulated pouch helps. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.1

None of this involves a syringe, a vial or a conversion. That is a large part of why a pen is easier to use accurately than an unlabelled vial drawn into an insulin syringe. If anything about the pen looks wrong, such as a window that will not dial or a pen that has frozen, do not improvise. Contact the pharmacy that dispensed it.

09 · What each dose delivers

What to expect at each dose

People who ask about units are usually really asking whether they are on the right dose to see results. The mg strength is what matters, and the trial evidence is stated in mg. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2

These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. The numbers are tied to the mg strength of the licensed product, which is another reason a unit figure from a chart cannot be used to predict anything. The trial used a defined medicine at a defined strength, and a vial of unknown concentration is not that.2

Pace matters too. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.1

The starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Your prescriber reviews how you are responding. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 That conversation is held in mg and in your own response, not in syringe units.

It is also worth remembering the mechanism, because it explains why dose is a matter of tolerance and response rather than a number to chase. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
