The blood-level peak is a short-term, weekly rhythm. The peak effect on weight is a long-term pattern across the whole course. Mixing them up is the most common source of confusion, so it helps to separate them.

Should you try to time your peak?

No. Timing your life around a peak is rarely useful, and you should never change your dose or injection day to chase one. The better habit is a consistent routine: the same day each week, the dose your prescriber has set, and a note of how you feel across the week. That record helps your prescriber at reviews.

Half-life and how long Mounjaro stays in your system

The half-life explains why Mounjaro is a weekly medicine and why its level changes slowly. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

In plain terms, when the next injection arrives, some of the previous dose is still present. The weekly rhythm therefore sits on a continuous background level rather than starting from zero each time.

05 · How the effect builds

How the effect builds over weeks and months

The effect of Mounjaro builds gradually, because treatment starts low and steps up. The starting pen is 2.5mg, and the pen strengths run through 5mg, 7.5mg, 10mg and 12.5mg up to 15mg1. Your prescriber moves you up the ladder according to how you respond to and tolerate each step, after at least 4 weeks on each dose.

That staged approach means the strength of appetite suppression you feel in your first month is not the strength you will feel at a higher dose. A person on the starter dose is not seeing the medicine's full potential, and that is by design. The starter dose gives your body time to adjust.

Across the SURMOUNT-1 trial of adults taking tirzepatide alongside diet and exercise, average weight loss rose with dose: people lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

06 · Appetite across the week

Why appetite can feel different across the week

Many people notice that their hunger and fullness change across the injection week. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

If you notice that your appetite returns strongly late in the week, tell your prescriber at your next review rather than adjusting anything yourself. 07 · Peak and side effects

Does the peak bring side effects?

Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts. The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13

Side effects tend to be most relevant when you start and when you step up to a new strength, because your body is adjusting. If anything worries you, contact your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme19.

If you take oral contraception, note that Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Speak to your prescriber about this before starting.

08 · Other GLP-1 medicines

Is the peak the same for other GLP-1 medicines?

Each medicine has its own profile, so you should not assume Mounjaro's timing applies to another product. Mounjaro is once-weekly, whereas liraglutide, supplied at a concentration of 6mg per millilitre, is a daily injection and works on a different rhythm. Wegovy is a once-weekly semaglutide with its own dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg.

Planning your dose steps and cost

Understanding the build-up helps you plan, including financially. The starter pen is £169.99 at the standard price, and a new customer's first order is £40 less with code FIRST40. If you step up after 4 weeks on each of 2.5mg, 5mg, 7.5mg and 10mg, your first four pens cost £959.96 at standard prices, before those discounts. 10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
