When does Mounjaro peak?
Mounjaro peaks in two different ways, and mixing them up causes most of the confusion. The first is the concentration of tirzepatide in your blood, which is highest in the first few days after a weekly injection and then tapers until your next dose. The second is the cumulative effect on appetite and weight, which builds across many weeks as you move up through the strengths.
What matters for you is the pattern: a gentle rise after the injection, a slow fall, and then another injection that tops levels up again.1

02 · What to do with this
Does the peak change when you should inject?
For most people, no. Mounjaro is a weekly medicine, and the aim is a steady routine rather than timing the injection around a peak. Pick a day that suits your week, keep to it, and speak to your prescriber if you need to change it.
The more useful question is how you feel across the whole week. If appetite control seems strongest early in the week and fades towards the end, that is worth mentioning at your monthly review. It may reflect the dose you are on rather than anything you are doing wrong, and your prescriber can decide whether a dose step is appropriate.
03 · Blood-level peak
The peak after each injection
The blood-level peak is the short-term pattern that follows every weekly dose. Tirzepatide is injected under the skin, absorbed gradually, and reaches its highest concentration in the first few days. After that the level declines slowly, which is why a weekly schedule works at all.
The reason it declines slowly is the half-life. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1
Because of that long half-life, there is no cliff edge at the end of the week; the medicine does not drop to nothing when the next injection is due.

Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

04 · Long-term build-up
The peak that takes months
The weight-loss peak is not a moment; it is the result of many weeks on the right dose. Mounjaro starts at a low strength so your body can adjust, and the prescribing team steps the dose up in stages. In the trial programme, results were measured over very long periods rather than days or weeks.
The headline trial figures come from SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
Notice the duration: that is more than a year of treatment. The peak effect on weight in these trials was a matter of persistence on treatment, not a spike a few days after the first pen. If someone tells you that the medicine either works within the first few days or does not work at all, the evidence does not support that.
A worked example helps put percentages in context. For someone starting at 100 kg, 20.9% is about 21 kg. That is a trial average at the highest strength over the full trial period, not a prediction, and it says nothing about how quickly any individual will get there.
05 · Starting at 2.5mg
What to expect on 2.5mg
The starting 2.5mg pen is the introduction to treatment, and many people ask whether they can stay on it and still lose weight. Some people notice an early change in appetite on the lowest strength, but the SURMOUNT-1 results were reported for 5mg, 10mg and 15mg.
Titration exists to help your body adjust to the medicine. Whether you continue to a higher dose depends on how you are responding and tolerating treatment, and that is a conversation for your monthly review rather than something to decide alone.1
If you are still hungry on 2.5mg, that is common information to share with your prescriber rather than a sign that something has failed. Early on, your body is still adjusting, and the dose is intentionally low. It is not a reason to change how or when you inject, and it is certainly not a reason to use extra doses. Only use the pen exactly as prescribed.
Comparing 2.5mg with 5mg is also a question for the prescriber. The licensed strengths exist because people differ in what they need, and the trial figures show that results across 5mg and above were reported separately. Your plan should be built on your own tolerance and response.
06 · Side-effect timing
When side effects tend to be noticed
Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.
The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Digestive symptoms are the group people ask about most, and they are worth watching especially after starting or increasing a dose.13 If you notice a pattern, such as feeling worse for a particular stretch after the injection, make a note of it and tell your prescriber. If you have severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or swelling of the face or throat with difficulty breathing, get medical help straight away. For other new or worrying symptoms, contact your prescriber or pharmacist. You can also report suspected side effects through the MHRA Yellow Card scheme, which helps monitor the safety of medicines.19
07 · Wearing off and stopping
Does it wear off, and what if you stop?
Mounjaro does not abruptly wear off a few days after an injection. Because of the long half-life, levels fall gradually, and the medicine stays in your system across the full weekly interval and beyond. Some people feel appetite control fade towards the end of the week, which is a reason to talk to your prescriber about the dose, not a sign that the injection has stopped working.
Stopping is a separate question. After a final dose, the medicine takes roughly four to five weeks to clear. That is a matter of the half-life, and nothing needs doing while it clears and nothing speeds it up.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
If you are thinking about coming off Mounjaro after only a short time, speak to the prescribing team first. You might be reacting to early side effects, which can often be managed, or you might simply be impatient for results, which is understandable given that the main benefit builds over months. Either way, a conversation is better than an abrupt decision.

08 · Supporting your results
What actually helps, and what does not
No trick moves the peak earlier or makes it bigger. The trials that produced the headline figures combined the medicine with a reduced-calorie diet and increased physical activity, and the results depended on staying on treatment for a long time.2
Sensible habits make treatment easier to live with. Smaller, simpler meals are often more comfortable when the stomach empties more slowly. Eating regularly, staying hydrated and being honest at your monthly review about how you feel all help your prescriber make good decisions. None of this is a way to accelerate the medicine; it is a way to support it.
It is worth being sceptical of anything offering dramatic weight-loss claims or a shortcut. That includes dose stretching, extra injections or unlicensed combinations. These are unlicensed and unsafe. Equally, low-cost supplements sold as alternatives come with no clinical oversight and are not a substitute for a prescribed medicine.

09 · Access and eligibility
Who can get Mounjaro
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
