In pharmacology, a peak is the highest concentration of a medicine in your blood after a dose. For an injection under the skin, the medicine is absorbed gradually, so the level rises over a period, reaches its highest point, then falls as the body clears it. With a long half-life medicine, the curve is much flatter.1

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 That long tail is why a once-weekly schedule is possible at all. It also means each injection adds to a background level that has already built up from previous weeks, rather than starting from nothing.

Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

Mounjaro and Wegovy are different molecules with different actions, so their curves should not be assumed to match.

There is also a difference between blood concentration and felt effect. The level in your blood may be highest at one point, but appetite signals, stomach emptying and nausea do not necessarily follow the same timetable. That is why two people on the same pen can describe quite different weeks.

05 · Weekly rhythm and side effects

Which day of the week do side effects feel strongest?

Many people notice that stomach-related effects such as feeling full, queasy or off food are more noticeable in the days after an injection and ease towards the end of the week. That pattern is common to describe, but it is not the same as a fixed schedule. Your own pattern may differ, and it can shift when your dose changes.

Those are general lifestyle habits rather than prescribed instructions, so ask your prescriber if you want personalised advice. If symptoms are severe, persistent, or you cannot keep fluids down, get medical help straight away rather than waiting for the week to pass.

Should you change your injection day to avoid a bad day, for example before a work event? If you want to change your injection day, you can do so as long as at least 3 days (72 hours) have passed since your last dose; talk to the prescribing team if unsure. Never take an extra injection or shorten the interval between doses to catch up or to try to boost an effect.

How long does it take to kick in, and how do you know it is working?

The signs that treatment is working are usually quieter than people expect: feeling full sooner, less thinking about food between meals, smaller portions that feel adequate, and a gradual downward trend on the scales over months rather than days.1

Headline trial data help set expectations. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

A worked example makes the scale clearer. For someone starting at 100 kg, 20.9% is about 21 kg. That is an average at the highest dose over a long period of treatment, not a forecast for the first weeks and not a promise for any one person.

Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures; a sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. The important markers in the early period are tolerability, steady habits and attending your reviews.

How do you judge progress honestly? Look at several signals together rather than the scales alone: hunger and cravings, portion size, clothes fit, energy, and how side effects are settling. Weigh yourself at a consistent time and interval rather than daily, because day-to-day fluctuation is normal.

If you feel no appetite change at all after a reasonable period on a dose, do not simply increase anything yourself. Speak to the prescribing team. Dose changes are made in the licensed steps by the prescriber, and some people respond well at lower doses while others need to move up.

07 · After the injection

What should you expect immediately after a GLP-1 injection?

For most people, very little happens right away. The injection itself is brief, and the medicine is absorbed slowly, so you will not feel an instant change. Follow the pen's instructions for how to inject.13 Needles, a sharps bin and injection guidance come with every order.

In the following hours and days, the most noticeable changes are usually in appetite and digestion. You may feel full sooner, be less interested in snacking, or have mild stomach discomfort. These effects relate to how the medicine slows stomach emptying and affects appetite regulation.1 Some people have a small mark or tenderness at the injection site, which is worth mentioning to your prescriber if it persists or looks worse.

Sensible habits after injecting include eating regular, smaller meals rather than skipping food, staying hydrated, and avoiding anything that tends to upset your stomach. Do not use a day with low appetite as a reason to eat very little for long periods; your reduced-calorie diet should still be nutritionally balanced, and your prescriber can advise.

Storage matters too. Unopened pens are kept in the fridge at 2–8°C until first use and keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. For any question about an unopened pen that has been out of the fridge, check the patient information leaflet and contact the pharmacy rather than guessing.13

When should you seek help? If you have severe or persistent abdominal pain, or signs of an allergic-type reaction such as swelling of the face or throat or difficulty breathing, get medical help straight away. If you have repeated vomiting or cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.

If you take other medicines, mention it at consultation. Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows, so ask your prescriber what contraception advice applies to you. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 People using insulin or sulfonylureas may need their doses adjusted because of the risk of low blood sugar, so those medicines should be reviewed with the prescriber, not stopped or changed alone.

08 · Dose steps and effect

Does the effect strengthen as the dose goes up?

The dose is built up in steps, and your prescriber guides the pace depending on how you respond and tolerate it. Mounjaro comes in strengths of 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

A new dose is introduced after at least 4 weeks on the previous one, depending on how you respond, and your prescriber decides whether to move up, hold or stay. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices; a new customer's first order is £40 less and every later order £25 less.

The 'peak' that matters to most people is therefore not within a single week but across these steps. A new dose can bring the strongest wave of side effects for a short while, then settle as you adjust. The effect on appetite generally becomes clearer over several injections at a given dose, not after one.

The trial data illustrate the dose relationship. In SURMOUNT-1, people lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks.2 At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Those are averages in a trial setting, and many people do well without ever reaching the top dose.

Higher is not automatically better for you. The right maintenance dose is the one that gives a worthwhile effect with side effects you can live with, as agreed with your prescriber.

09 · Stopping and clearance

How long does it take to leave your system?

After your last injection the medicine does not vanish overnight; it falls gradually. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 No drink, supplement or routine alters that timetable, and you should be wary of anyone who says otherwise.

This long tail has practical consequences. First, if you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time; if more than 4 days have passed, skip it and take the next one on your normal day — never take two doses within 3 days of each other. Also, appetite effects wear off gradually, not suddenly, so many people notice hunger returning over the following weeks rather than on a particular day.

Mounjaro also stops working slowly, over a longer period than a single week. If you plan to stop, change dose or take a break, discuss it with your prescriber first so that you have a plan for eating habits, activity and monitoring as the effect fades.

In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber.14 Your prescriber can explain what that means for your own goals.

Planning around surgery, pregnancy or other medical events is a clinical decision. Tell any clinician treating you that you use Mounjaro. Mounjaro must not be used during pregnancy and should be stopped at least one month before trying to conceive; if you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

10 · Reviews and complaints

How can you check a provider yourself?

Treat online reviews as stories, not evidence.

What about 'Ozempic breasts', 'Ozempic teeth' and celebrity stories?

If you notice a change in your body, teeth, gums or skin, or any new symptom, talk to your prescriber or GP and consider reporting it through the MHRA Yellow Card scheme.19 The leaflet is the right place to check what is a recognised side effect.13

The same applies to the question of whether someone can smell when another person uses a GLP-1 medicine. If you notice a new body or breath odour, mention it to a clinician, because there are many ordinary causes that are worth checking, including changes in eating and hydration.

People stop treatment for many personal, medical and practical reasons. A public figure's experience says nothing reliable about what will happen to you, in either direction.

What is a better approach to rapid change? If treatment leads to a quick drop in weight, changes in how you look, including facial volume or loose skin, are an effect of losing weight quickly rather than a drug effect; discuss them with your prescriber. Eating adequate protein and a varied diet, staying active, and keeping dental hygiene and regular check-ups are sensible general habits. Your prescriber or dentist can tailor advice.

When something sounds alarming online, ask three questions: who is the source, is there a recognised medical reference for it, and what would my prescriber say? If the answer to the second is no, treat it as unproven.

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

13 · NHS and private routes

Is this treatment available on the NHS or privately?

Both routes exist, and the right one depends on whether you meet the criteria and how long you are willing to wait. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
