Mounjaro's is the Summary of Product Characteristics and the patient leaflet, both freely available.113 Second, a qualified prescriber decides whether it suits you, using your history and current medicines. Third, side effects are tracked after launch, and you can report your own through the Yellow Card scheme.19

Mounjaro is not suitable for everyone, and the exclusions are the most important safety information on the label. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.1

How does it work, and why does that matter for safety? Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. Because it slows stomach emptying, it can affect how quickly some oral medicines are absorbed.

Mounjaro is a once-weekly injection in a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Treatment starts at the lowest strength and steps up gradually, which is itself a safety feature: it gives your body time to adjust, and your prescriber a chance to review how you are coping before each increase.

Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Unopened pens go in the fridge at 2–8°C until first use and keep until the printed expiry date. Using the device correctly matters for safety too, so follow the pen's instructions and ask us if anything is unclear.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears. If a side effect is troubling you, that clearance period is worth knowing about, and it is a reason to speak to your prescriber rather than wait it out silently.

Serious side effects on Mounjaro are uncommon, but they need urgent attention when they happen. Knowing the warning signs in advance is one of the most practical safety steps you can take, whichever weight loss medicine you use.

Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis. Also seek immediate medical help for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which could signal gallbladder problems, and for signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar, which can feel like shakiness, sweating or confusion. Your prescriber may need to adjust those medicines, so tell them before you start rather than after.13

Dehydration deserves a special mention. Severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys, so keep fluids up and get medical help straight away if you cannot keep fluids down. Thyroid, kidneys, liver and mood

Thyroid. In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials.1 As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2, because they will weigh this in your assessment. Routine thyroid monitoring is not required for most people.1

Kidneys and liver. Mounjaro is not known to damage kidneys or liver directly, and no dose adjustment is needed for mild to moderate kidney or liver impairment.1 The practical risk is indirect: severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys. People with severe impairment need individual prescriber assessment.

Mood. Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17 That said, any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

There is also a benefit side to the ledger. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose.13 Tell your prescriber about everything you take, especially warfarin or other narrow-margin medicines, insulin, or sulfonylureas, where doses may need reducing to avoid low blood sugar. No adjustment is needed for paracetamol.13

Note the wording on insulin and sulfonylureas: the issue is that doses may need adjusting, not that these medicines can never be combined with Mounjaro. A prescriber who knows your full list can plan that adjustment safely, which is something an over-the-counter product cannot do.

Contraception is a separate point. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Since the medicine should not be used during pregnancy or while trying to conceive, reliable contraception is part of safe treatment.1

Mounjaro (tirzepatide) is a once-weekly injection that activates both GIP and GLP-1 receptors. This dual action is what distinguishes it from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1 It comes in six strengths from 2.5mg to 15mg.

Wegovy (semaglutide) is also once weekly. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17.18 The 2.4mg dose is the licensed maintenance dose for weight management. Check availability at the time of order.

Liraglutide (Saxenda, and products such as Erly) is supplied at 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. For other details on it, your prescriber can advise.

Tablets. Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) are daily tablets with their own product information.2021 No trial has compared Mounjaro directly with either tablet, so results from separate trials should not be set against each other as if they were head to head.

So which is safer, Wegovy or Mounjaro? Both are documented, licensed prescription medicines with different ingredients, and both need a prescriber to check your history. The prescriber decides which suits you, based on your health profile and tolerance.

If you want to switch between medicines, do it only through a prescriber, who will decide how to start the new medicine safely. Do not improvise your own schedule using leftover pens.

10 · Supplements and cheap pills

'Poor man's Ozempic', supplements and over-the-counter pills

Supplements marketed as natural alternatives to prescription injections are low-cost and easy to buy, but they come with no clinical oversight and far weaker evidence than the licensed medicines. The nickname 'poor man's Ozempic' is a marketing phrase, not a medical category, and we would not rely on it. Ozempic itself is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and a regulated UK pharmacy will not supply Ozempic for weight loss.

It usually refers to berberine, a supplement promoted with dramatic weight-loss claims. The key difference is structural: a prescription medicine has a licence, a leaflet, a prescriber and a reporting system; a supplement typically has none of these.

There are three red flags worth knowing:

  • Dramatic weight-loss claims or celebrity-style stories of very fast loss. These are marketing, not evidence.
  • Anyone offering to supply prescription medicine without a consultation or prescription.
  • Prices far below what regulated pharmacies charge, which can signal counterfeit or unlicensed product.
