What Happens If Mounjaro Is Left Out of the Fridge? (October 2026)

What happens if Mounjaro is left out of the fridge?
For a pen you have already started, very little happens if it has been out of the fridge for a short spell. The UK licence says an in-use pen can be kept out of the fridge not above 30°C for up to 30 days, which is why a weekend away or a day in a bag is not a crisis.1
The answer changes in three situations. If the pen got hot, if it froze, or if it was an unopened pen that sat out for an unknown length of time, you need to slow down and check before you inject.

Start with the pen itself. Is it a pen you have already used for at least one injection, or a sealed pen that has never been opened? That single question decides most of the answer.

Contact the pharmacy that supplied it, with the batch number and expiry date from the box to hand. 04 · Why a fridge matters
Why Mounjaro is stored in a fridge
Mounjaro is stored in the fridge because the licensed instruction is to keep pens at 2–8°C and never freeze them.1 Pens go in bags, on trips and sometimes on kitchen worktops.
Mounjaro contains tirzepatide, which is a medicine delivered in a liquid solution inside a pre-filled KwikPen.1
A day limit that appears in a forum thread, a video or a news article may come from another country's product information. Different markets can carry different wording for what looks like the same pen. For a UK pen, rely on the UK leaflet and your pharmacy, because figures circulating online often come from other countries' labels.13
The three storage rules in plain terms
First, cold is the default. Unopened pens belong in the fridge until you start them. Second, once you start a pen, you are allowed some flexibility, which is helpful because a pen is used across four weekly doses. Each KwikPen contains four weekly doses. Third, freezing is never acceptable, and neither is leaving the pen somewhere hot enough to exceed the licensed ceiling.
Light matters too. Keep the pen cap on to protect it from light.1 A pen sitting uncapped on a sunny windowsill is being exposed to two risks at once, warmth and light, even if it has not been out for very long.
What the fridge does not do
The fridge does not make a pen safe forever. A pen still has a printed expiry date, and a pen in use still has its limit once it is out. Keeping a pen cold simply keeps it within the conditions it was tested for. If you find that your fridge is unreliable, for example it sometimes freezes items at the back, move your pens to a spot that stays cold without freezing, and keep them away from the freezer compartment and the cooling vent.

An in-use Mounjaro pen can be kept out of the fridge not above 30°C for up to 30 days, and after that it should be discarded.1 This is the figure to trust for a UK KwikPen. Do not use a pen that has been out of the fridge for more than 30 days, or above 30°C in use.13
Note what the rule is not. It is not a countdown that starts when you buy the pen, and it is not an expiry-from-manufacture span. It is an in-use allowance. If your pen is in the fridge, you do not need to count anything beyond the printed expiry date and the four doses it contains.
How to keep track
The easiest method is to write a date on the carton or on a sticky label the day the pen first comes out of the fridge for storage elsewhere. If you inject from a pen kept in the fridge and only take it out for the injection, you are unlikely to approach any limit. If you keep a pen in a bag, a desk or a bedside drawer, the date matters much more.
You can also use the GLPTrackr companion app, free on iOS and Android, to log your injection days and notes. A written date on the box and an app log together make it hard to lose track, especially if you share a fridge with others or travel often.
Does the pen last four doses anyway?
A KwikPen holds four weekly doses, so a pen used at the usual pace is in use for about a month. That means the 30-day in-use allowance fits the typical life of a pen. In other words, someone who left a pen out the whole time they used it, and kept it not above 30°C, would still be within the licence by the time the final dose was injected. If you started a pen late, or paused your treatment, the picture changes, so ask your prescriber if your pen has been open for a long time.
If you miss an injection, the missed-dose rule applies rather than any storage rule. Take it as soon as you can within 4 days (96 hours) of the scheduled time; if more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1 A pen that has been parked on a shelf while a dose was missed is still bound by its in-use limit.

06 · Unopened pens left out
Unopened pens left out of the fridge
If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.1 The in-use allowance does not automatically transfer to a sealed pen.
What the information does say is simple. Unopened pens should be kept in the fridge, and they keep until the expiry date printed on the box.1 A pen that spent time out of the fridge has stepped outside that default, so the safe move is to ask the supplier.
What to tell the pharmacy
Give the pharmacist the details that matter. Say how long you think the pen was out, even if it is a rough guess. Say where it was, for example a hallway after a delivery, a car, a kitchen counter or a courier's van. Say whether it was in the box, and whether the room was warm. Give the batch number and expiry date from the carton. A pharmacist can then judge whether the pen is likely to be fine, and can arrange a replacement if it is not.
Deliveries and doorsteps
Our pens are sent by free next-day tracked delivery with DPD, in temperature-controlled, cold-chain packaging. When your parcel arrives, take the pens out and put them in the fridge promptly. If you were out when it arrived, and the parcel sat on a doorstep or with a neighbour, tell us rather than guessing. A pen that has had a long, warm wait is exactly the kind of doubt the pharmacy exists to resolve.
A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.13 If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.
Do not simply risk it
It is tempting to use a pen anyway to avoid missing a dose, particularly if you have only just noticed the mistake. Weigh that against what a missed injection costs. One late dose can usually be handled by the missed-dose rule, while a pen stored wrongly means the dose is of uncertain quality. Asking first is the better trade.
07 · Heat and warm places
If your Mounjaro gets warm
A warm pen is acceptable only up to a point, and that point is not above 30°C.1 The in-use allowance is written with that ceiling attached, so the 30-day window assumes the pen has stayed under it. If you think it went above, treat the pen as doubtful and contact the pharmacy.
Everyday places where this goes wrong are easy to name. A parked car on a sunny day heats up far beyond a comfortable room. A windowsill, a conservatory, a bag left in direct sun, a pocket against a hot body and a beach bag are all hotter than they feel to you. The pen sits inside and cannot tell you it is too warm. If you are unsure how hot a place got, the sensible assumption is that it was hotter than you think.
Hot climates and summer travel
For holidays, in-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch.1 The pouch slows heat getting in. It is not a refrigerator, so it does not replace the fridge at your destination. If your accommodation has a fridge, use it for any pen not in use, and avoid putting the pen directly against a cold pack where it could freeze.
What counts as a warning sign
Check the solution before each injection. It should be clear and colourless. If it looks cloudy or discoloured, or you can see particles, do not use it.13 Looking normal does not prove a pen is fine, because heat damage is not always visible. That is why the temperature and time history of the pen matters as well as how it looks.
What to do after a heat incident
Move the pen somewhere cool straight away, and put it in the fridge if it is not in use or when you are home. Note the approximate time and place. If the pen was exposed to heat above the ceiling, or you cannot rule it out, contact the pharmacy before the next injection. A prescriber or pharmacist can say whether a replacement is appropriate.
Warm weather is also a good moment to rethink your routine. If you always inject on a day when you are out and about, plan where the pen will live that day, and take it home to the fridge afterwards.
08 · Frozen pens
The opposite mistake: a pen that has frozen
A pen that has been frozen must never be used, even if it has thawed and looks normal.1 Freezing is the single storage fault with an absolute rule. The licence says to keep pens in the fridge and never freeze them, and to never use a pen that has been frozen.
People are often more worried about warmth than cold, but freezing happens more easily than you would think. A pen placed at the back of a fridge, against the cooling plate, can freeze. A pen placed in a freezer compartment by mistake will freeze. A pen pressed against an ice pack in a cool bag can freeze. A pen in hold luggage can freeze, which is why hold luggage can fall below freezing and pens should travel in hand luggage.1
How to spot a frozen pen
Sometimes it is obvious, because the solution is solid or has ice crystals. Sometimes it has thawed by the time you find it, and the only clue is where it was stored. If you suspect it froze, act on the suspicion. Do not wait for visible damage. The wider checklist applies as well: do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use.13
What to do
Discard the pen safely, using your sharps bin, which is included with every order along with needles and injection guidance. Then contact the pharmacy to arrange a replacement.
Preventing it
Choose a shelf in the main fridge compartment rather than the back wall, and keep pens away from the freezer box. Keep the pen in its carton, which also protects it from light. In a cool bag, separate the pen from any ice pack with a layer of cloth or card. In winter, avoid leaving a pen in a car overnight, where it can get colder than a fridge.
If you are travelling by air, the advice is to carry pens in hand luggage in a cool bag with the prescription label or a copy of your prescription, and to check destination rules for travelling with prescription injectables before you fly.1
A frozen pen is a replaced pen. There is no clever workaround, and no amount of waiting makes it usable again.
09 · Is it still effective?
Is Mounjaro less effective if not refrigerated?
A pen kept within the licensed conditions is expected to work as intended, and a pen outside them is the one we cannot vouch for. The practical position is binary: a pen within the licensed limits is fine to use, and one outside them should be discarded.
That is why the limits are written as firm cut-offs. An in-use pen is fine for up to 30 days not above 30°C, and then it should be discarded.1 Past the limit, the pen is outside the tested window, and sterility and potency are no longer assured.
What you might notice
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 A change in how you feel is not a reliable test of whether a pen is good.
This is why the dates, the temperature and the appearance of the solution matter more than how you feel after injecting. Do not use your own response as a quality check.
Judging progress fairly
Because results build over months, and trials measured them at 72 weeks, a single doubtful dose is not going to define your outcome. The trial average in SURMOUNT-1 was 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg, compared with 3.1% with lifestyle changes alone, and individual results vary.2 Those figures come from pens stored and used properly, alongside a reduced-calorie diet and increased physical activity. If a dose was missed because a pen was discarded, the prescriber can advise on getting back on track.
If you feel unwell after a doubtful dose
If you used a pen and then have new or worrying symptoms, speak to your prescriber or pharmacist and tell them what happened with the pen. You can also report suspected side effects through the MHRA Yellow Card scheme.19 If you think a pen may be counterfeit or incorrectly supplied, the MHRA also publishes counterfeit medicine alerts.7
Do not try to compensate for a doubtful pen by injecting extra or taking doses closer together. Never take two doses within 3 days of each other.1
10 · Checks and prevention
How to tell if a pen has gone bad, and how to prevent it
You cannot always tell by looking, but a few checks catch the obvious cases. Before each injection, check the label, the expiry date, the clarity of the solution and the history of the pen. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use.13
A simple pre-injection routine
First, look at the carton: is the expiry date still ahead, and have you noted when the pen first left the fridge? Second, look at the solution through the pen window: it should be clear and colourless. Third, think about where the pen has been: any hot car, windowsill, freezer compartment or hold luggage? Fourth, check the dose window.
Habits that prevent the problem
Put pens away as soon as a delivery arrives. Keep them in the carton. Choose a fixed fridge shelf. Pick an injection day and time you will remember: there is no best time of day, and Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1 Link the injection to a routine, so the pen comes out of the fridge, is used and goes straight back.
Plan ahead for travel: use hand luggage, a cool bag, a copy of the prescription and an insulated pouch for hot climates. If you are away from home on injection day, prepare the pen there rather than carrying it around all day.
When a replacement is the right call
If any check fails, replace the pen rather than argue with yourself. Treatment is long term and gradual, and your prescriber reviews response over time, so one replaced pen does not derail it. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Good storage helps you reach that review with a fair picture.
Treatment on the NHS is also being phased in over several years, and NHS pens follow the same storage rules, so the habits here apply wherever your supply comes from.

What to ask the pharmacy or prescriber
Contact the supplier of your pen as soon as you have doubt, and be ready with specific details. Pharmacies deal with storage questions all the time, and a clear description speeds up the answer.
The information that helps most
Have the box in front of you. Read out the batch number and expiry date. Say whether the pen is unopened or already in use, and how many injections you have taken from it. Describe where the pen has been since it left the fridge, and give your best estimate of how long. Say whether the solution looks clear and colourless. Finally, say whether it could have frozen, or been somewhere hot.

Leaving a pen out is one of the most common storage slips, and the licence is built with some tolerance for it. For an in-use pen below the temperature ceiling and inside the time window, carry on. For anything else, check first.
