What Happens If You Don't Put Mounjaro in the Fridge? (October 2026)
If you don't put Mounjaro in the fridge, an in-use pen is still fine for up to 30 days not above 30°C, after which it should be discarded.1 Unopened pens belong in the fridge at 2–8°C until first use. If one has been left out, check with the pharmacy before injecting rather than guessing.13 Never use a pen that has frozen, looks cloudy or has particles.

before you start.
-
An in-use Mounjaro pen can be kept out of the fridge not above 30°C for up to 30 days, then it should be discarded.
-
Unopened pens should stay in the fridge at 2–8°C and keep until the printed expiry date.
-
If an unopened pen has been out of the fridge, ask the pharmacy before using it. Do not rely on a day limit you read online.
-
Never use a pen that has been frozen, looks cloudy or discoloured, or has particles in it.
-
Heat above 30°C, freezing and light are the real risks, so keep the cap on and keep pens away from hot places.

01 · The core answer
What happens if you don't put Mounjaro in the fridge
For a pen you have already started, very little happens in the short term. The licensed guidance says an in-use pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 That window exists so you can carry a pen to work, keep it in a bag or take it on holiday without a cool box.
For an unopened pen the position is different. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, the safe step is to check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.1
UK KwikPens carry their own wording: the in-use figure is 30 days (not above 30°C).
| Situation | What to do |
|---|---|
| In-use pen, kept not above 30°C, within 30 days | Fine to keep using. Cap on, away from light and heat.1 |
| In-use pen, beyond 30 days, or above 30°C | Discard it, even if medicine appears to remain.1 |
| Unopened pen left out briefly | Contact the pharmacy before use. Do not rely on a day figure from the internet.1 |
| Pen that has been frozen | Never use it.1 |
| Pen that is cloudy, discoloured or has particles | Do not use it. Contact the pharmacy.1 |
02 · Your next step
Which situation are you in?
Work out which of three situations applies. You forgot to refrigerate a pen you are using, you left a new pen out after delivery, or you are travelling and have no fridge at all.
If you forgot a pen you are already using, you probably have nothing to worry about. Put it back in the fridge or keep it somewhere cool and shaded, cap on, and carry on counting your days from first use. If you cannot remember when you started the pen, work it out from your injection schedule. Each KwikPen contains four weekly doses, so a pen you started on your last dose is nowhere near the limit.1
If you left a new, unopened pen out, put it in the fridge now and contact the pharmacy before you inject it. Tell them roughly how long it was out, where it was and whether it sat near a radiator, in a car or in sunlight. Our team can look at that with you; our forgot to put Mounjaro in the fridge guide covers the first few minutes after you notice.

03 · In-use pens
An in-use pen out of the fridge
A pen you have started can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be thrown away.1 The limit is a combination of two things: temperature and time. Staying not above 30°C matters as much as staying inside the 30 days.
Most UK homes sit comfortably below that temperature for most of the year. The risk tends to appear in specific places: a parked car in summer, a windowsill, a bag left in the sun, a bathroom shelf next to a hot radiator or a kitchen counter beside the oven. Treat any of those as a place the pen should not stay.
Keep the cap on between injections. The pen needs protecting from light, and the cap is what does that job.1 A pen sitting uncapped on a table is a worse storage situation than a capped pen in a drawer.
Count the days from your first injection with that pen, not from the date it arrived. Write the start date on the box or set a reminder on your phone. Our free weight loss clinic support team can talk you through a routine if you worry about forgetting.
Once the 30 days are over, discard the pen. Do this even if some solution looks left in the window, because after that point the manufacturer's tested window has passed and potency and sterility are no longer assured. Your sharps bin is the right place for it.
04 · Unopened pens
Unopened pens: why the fridge still matters
Unopened pens should be kept in the fridge at 2–8°C and never frozen, and they keep until the expiry date printed on the box.1 The fridge is for the pens you are not yet using. That is the version of the rule most people actually need.
So what if an unopened pen sits on the side for an afternoon? The patient information leaflet and the pharmacy are the right places to check.13
Practical steps if it happens:
- Put the pen in the fridge straight away, away from the freezer compartment and the back wall where items can freeze.
- Note where it was left, roughly for how long and what the room temperature was like.
- Contact the pharmacy that supplied it and ask before you inject.
- Do not inject it in the meantime to see whether it feels fine. A pen cannot be judged by feel.
If you have several pens at home, use them in the order they were supplied, and keep the spare ones in the fridge door or a shelf, not the freezer drawer.
05 · Heat and freezing
Heat, freezing and light: the real risks
The storage guidance names freezing as a hard stop: never freeze Mounjaro pens, and never use a pen that has been frozen.1 A frozen pen cannot be rescued by thawing it. If you suspect it has frozen, even briefly, do not use it and speak to the pharmacy.
Freezing is a bigger issue than many people expect, because it happens in the fridge itself. A pen pushed to the back wall, next to the cooling element or beside a frozen item can end up colder than intended. Keep pens where the fridge is steadiest, usually a middle shelf or the door shelf if your fridge is dependable.
Heat is the other side. The in-use allowance is not above 30°C, so a hot car, direct sun or a radiator puts a pen outside what it was tested for.1 A pen that has overheated cannot be checked by looking at it, so do not use a pen that has been above 30°C, and contact the pharmacy.
Light matters too. Keep the cap on to protect the pen from light.1 A pen in a transparent bag on a sunny table is a worse choice than one in an opaque pouch.
The pattern is simple. Cold but not frozen is right, cool and shaded is acceptable for a pen in use, and hot, frozen or bright is wrong.
06 · Checking a pen
How to tell if Mounjaro has gone bad
You cannot tell with certainty by looking, but certain signs mean you should not use the pen. A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.
Do not use a pen that:
- has been frozen
- looks cloudy or discoloured
- has particles in it
- is past its expiry
- has been out of the fridge for more than 30 days, or above 30°C in use
If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.12
A clear pen is not proof that a pen is fine. It only rules out the visible problems. If a pen has been somewhere hot or you do not know how long it was out, the visual check is not enough, and the pharmacy conversation is the right route.
Fake or poor-quality pens are a separate concern. The MHRA publishes counterfeit alerts, and buying only from a registered pharmacy keeps you out of that problem.7 Offering to supply Mounjaro without a consultation or prescription is a warning sign in itself.
07 · Travel and no fridge
Travelling, or no fridge to hand
You can fly with Mounjaro, and most holidays need no special arrangements. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.1 In-use pens tolerate up to 30 days not above 30°C, and for hot climates an insulated pouch is the sensible choice.
That answers a question people often ask: how to refrigerate medicine without a fridge. For a pen in use, you often do not need to. A cool, shaded spot such as a bag in a hotel room, away from windows and heaters, is enough while you stay under the temperature and time limits. For hot places, an insulated pouch is a small accessory purchase and worth having.
For unopened pens on a long trip, plan to keep them in a fridge, for instance in a hotel room, and ask your accommodation in advance. Do not put pens in ice directly, because direct contact risks freezing.
Check destination rules for travelling with prescription injectables before you fly. Pack needles and a sharps solution too. Our do you have to put Mounjaro in the fridge page covers the same ground from the everyday angle.
08 · Efficacy concerns
Will a pen left out still work?
Inside those limits, you can expect the pen to behave as intended. Outside them, we cannot vouch for it, because sterility and potency are no longer assured.1
The reason this matters is that results depend on a steady weekly dose. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 A pen you cannot trust undermines the dose you think you are taking.
Our guide to what happens if you don't refrigerate Mounjaro goes through the reasoning in more detail.
Never respond to doubt by taking an extra dose, a larger dose or a repeat injection. Altering doses outside the licence means unknown dosing accuracy and no safety data.1
09 · Side effects and reporting
If you injected a pen that was left out
If you have already injected from a pen that was out of the fridge, do not panic. Check how long and how warm it was. If it was an in-use pen inside the limits, you have done nothing wrong.
If it was outside the limits or you are unsure, contact the pharmacy and your prescriber, describe what happened and follow their advice about the next dose. Do not inject an additional dose to make up for it. The usual missed-dose and spacing rules still apply.1
Any new or worrying symptom should be discussed with the prescriber. You can also report suspected problems with a medicine or a pen through the MHRA Yellow Card scheme, which the patient information leaflet describes.19 Reports help regulators spot quality problems that individual users cannot see.
Keep the box, the batch number and the expiry date handy when you call. The more exactly you can describe what happened, the more useful the answer will be.
10 · Building a routine
A simple routine so it does not happen again
Most storage mistakes come from the same moments: unpacking a delivery, packing for a trip and the minute after an injection. A routine takes those moments out of memory.
- When a delivery arrives, move spare pens into the fridge before anything else, and keep only the pen you are using out.
- After each injection, put the cap back on and return the pen to its usual place straight away.
- Write the start date on the box so you know where you are against the 30 days.
- Keep a pouch ready for trips, and keep pens out of cars.
- Check the fridge is not freezing items at the back, because a frozen pen must not be used.1
Pick your injection day and time and link it to something fixed. Many people choose a weekend morning. There is no best time of day, as Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week.1
Orders reach you in temperature-controlled packaging by free next-day tracked delivery, and same-day dispatch applies when you order by 3pm. Open the parcel promptly and put the pens in the fridge. If a parcel has clearly been left somewhere hot or arrives damaged, contact us before using it.
