Can you get weight loss injections online through the NHS?

No, there is no NHS online shop for weight loss injections. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years.

The private online route and what it costs

04 · Who qualifies

Who qualifies for Mounjaro

Eligibility differs by route. On the NHS, NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition, and the rollout began with BMI 40+ and four or more qualifying conditions.⁴⁶

Privately, Mounjaro is licensed for adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used with a reduced-calorie diet and increased physical activity.¹

People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk at a lower BMI, so thresholds are usually reduced by 2.5 kg/m².⁴

05 · What to expect

What results look like

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. Individual results vary.² For someone starting at 100 kg, 20.9% is about 21 kg.

Weight loss is gradual by design. The first weeks on 2.5mg are an adjustment phase, and most measurable progress builds from 5mg onward. Think in months, not days. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether to continue, adjust or stop.¹

06 · Safety and suitability

Who should not take it

You should not take Mounjaro if you are allergic to tirzepatide or its ingredients, or during pregnancy or while trying to conceive. Your prescriber takes extra care, or may advise against treatment, with a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems.¹ Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans. Tell your prescriber if you or a close relative have had medullary thyroid cancer or MEN 2.¹

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹³ Report side effects through the Yellow Card scheme.¹⁹ Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

07 · Dosing basics

How the dose ladder works

Everyone starts on 2.5mg once a week, which is about letting your body adjust rather than weight loss itself. After at least 4 weeks the dose rises to 5mg, then steps up by 2.5mg at minimum 4-week intervals to a maximum of 15mg. Many people settle at 5mg, 10mg or 15mg; higher is not automatically better.¹

Each KwikPen holds four weekly doses, injected under the skin on the same day each week. Keep pens in the fridge at 2 to 8°C; once in use, a pen can stay not above 30°C for up to 30 days.¹
