04 · What Mounjaro is

What Mounjaro is and how it works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.11 That is the key point to hold on to: the injection makes eating less easier, but it works alongside a reduced-calorie diet and increased physical activity, not instead of them.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1 Wegovy is also a once-weekly injection, with pens of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg. Liraglutide products such as Saxenda are different again: they are supplied at 6mg per millilitre and are daily injections rather than weekly ones.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 This matters for planning. If you stop treatment, or if you are asking a prescriber about a side effect, the medicine does not leave your body immediately, so effects can continue for some weeks.

05 · Who can have them

Who can have weight loss injections

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.1 Those criteria apply wherever you live, whether that is Basingstoke, Winchester or anywhere else in the UK.

A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.11 That is why the consultation matters. It is not a formality, and it is not a scoring script: a GPhC-registered prescriber reviews your answers. Medicines you already take are part of that decision. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. If you take insulin or a sulfonylurea, the doses may need adjusting because of the risk of low blood sugar, so tell the prescriber about every medicine. You should be honest in the questionnaire about your height, weight, health conditions and medicines. A prescriber can only judge suitability on accurate information, and the licence is specific about who the treatment is for.

Many people ask whether they are 'allowed' to try injections if they only want to lose a small amount of weight. The licence answers that: it is for adults who meet the licensed BMI criteria. It is not intended for cosmetic weight loss in people outside those criteria.

06 · NHS or private

NHS or private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially people with a BMI of 40 or above and four or more qualifying conditions.6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6

In practice, that means the first thing to do in Basingstoke is to ask your GP whether you fall into a group that can currently be prescribed Mounjaro on the NHS. The groups are being phased, so the answer can change as the rollout widens. If you do qualify, the NHS route is a perfectly sensible one, and it is not our place to talk you out of it.

If you are thinking of switching between providers, tell the new prescriber your current dose, how long you have taken it and how you have tolerated it. They will decide the right dose to continue at, rather than assuming you restart. When weighing up the decision, consider three questions. Do I meet the NHS criteria now? How long am I prepared to wait? And what would paying for treatment mean for me over a year, not just a month? Add up a first year at the dose you are likely to take, and set it against your own budget before starting. Whichever route you choose, keep your GP informed about any new treatment. It helps them look after your wider health, especially if you have conditions such as diabetes or high blood pressure that may change as you lose weight.

07 · Doses and costs

Dose ladder and what a year costs

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.11

It is also worth thinking about response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.11 So the yearly figure is a planning ceiling, not a commitment: treatment is reviewed, and you are not tied to a subscription.

08 · Realistic results

What results to expect

Weight loss on Mounjaro is gradual by design, so think in months, not days. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

To put that in perspective with a worked example: for someone starting at 100 kg, 20.9% is about 21 kg. The trial average does not predict your own result, and nothing in the figures tells you how many weeks it will take you to reach a given amount.

The trial also reported spread. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 So most people on treatment lost a meaningful amount, but outcomes covered a wide range, and the highest figures were not what everyone achieved.

The pace is not even. The first weeks on 2.5mg are an adjustment phase and losses are usually modest, and most measurable progress builds from 5mg onward, month by month.11 Experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.11

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.11 If effects are troublesome, or you are worried about anything new, speak to your prescriber. You can also report side effects through the MHRA Yellow Card scheme.19

Some practical habits help you judge progress sensibly. Weigh yourself at the same time of day, once a week at most, and look at the direction over a month rather than the number on a single morning. 09 · Injecting and storage

Injecting, missed doses, storage and travel

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.13 Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.13 Follow the pen's instructions for how to use it, and use the guidance that comes with every order.

There is no 'best' time of day: Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off.1

The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1 You can also change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 Unopened pens belong in the fridge and keep until the expiry date printed on the box.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements, and for hot climates you should use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.13

If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and the UK patient information leaflet is the reference to trust. Buying safely: counterfeits, scams and what to avoid

If a seller, social media account or website offers to supply without one, treat it as a warning sign. Low-cost supplements are available cheaply without assessment and without clinical oversight, which is a very different thing from a licensed medicine reviewed by a prescriber.

Do not take two doses within 3 days of each other. Do not use a pen that has been frozen, and do not use one that will not dial a full dose.13

Beyond that, tell your prescriber about every other medicine.
