A first year costs £2,399.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,279.88 if you step up through every dose to 15mg. That is 13 pens at standard prices before discounts. 05 · Ozempic cost explained

Mounjaro vs Ozempic cost: the licensing point

Ozempic is semaglutide licensed for type 2 diabetes, not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label.¹⁸ The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg). So Mounjaro vs Ozempic cost for weight loss is really Mounjaro vs Wegovy cost, and a regulated UK pharmacy will not supply Ozempic for weight loss.¹

Wegovy's own ladder is 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.¹⁸

If you have type 2 diabetes and are already prescribed Ozempic by your GP or diabetes team, do not change or stop it based on a price comparison. Talk to the clinician who manages your diabetes. Dose ladders: what each step costs

Both medicines are priced by pen strength, so the cost depends on where you are on the ladder. Mounjaro comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ Wegovy comes in five: 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, with steps after 4 weeks on each of the first four and the 2.4mg maintenance dose starting from week 17.¹⁸

With Mounjaro, the starting pen is the 2.5mg strength, which is £129.99 as a first-order price. With Wegovy, the first pen is £99.99 as a first-order price. Mounjaro doses step up after at least 4 weeks on each dose, depending on how you respond.

Someone on 5mg pays £189.99 per standard pen; someone on 10mg pays £274.99; someone on 15mg pays £299.99. Every later order is £25 off those standard prices. The dose you end up on is a clinical decision, made with your prescriber.

07 · What the trials show

What the trials show for the money

In SURMOUNT-5, the first large head-to-head trial of tirzepatide and semaglutide for weight loss, adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy).¹⁵ Both are effective, licensed treatments. Which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone.

In SURMOUNT-5, 751 adults took tirzepatide or semaglutide 2.4mg for 72 weeks. At least 10% of body weight was lost by 81.6% on tirzepatide versus 60.5% on semaglutide. At least 15% was lost by 64.6% versus 40.1%, at least 20% by 48.4% versus 27.3%, and at least 25% by 31.6% versus 16.1%. Average loss was 22.8 kg versus 15.0 kg.¹⁵

As a worked example, for someone starting at 100 kg, 20.2% is about 20 kg and 13.7% is about 14 kg. That is straight arithmetic on the trial averages, not a forecast of your own result, which trial averages do not predict.

Each medicine also has its own pivotal trial. In STEP 1, adults on semaglutide 2.4mg (Wegovy) lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo. In that trial, 86.4% lost at least 5% of body weight (31.5% on placebo), 69.1% lost at least 10% (12.0%) and 50.5% lost at least 15% (4.9%).⁹ In SURMOUNT-1, 2,539 adults on tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. For someone starting at 100 kg, 20.9% is about 21 kg.²

STEP 1 and SURMOUNT-1 are separate trials with different designs, so their numbers should not be set against each other. Only SURMOUNT-5 supports a direct comparison.

08 · NHS and private routes

NHS vs private: what each route costs

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

On the NHS, the NHS prescription charge in England is £9.90 where it applies. The trade-off is access: you need to fall within the groups currently being offered treatment and wait your turn. NHS access to Wegovy runs through specialist weight-management services, so ask your GP.

Private treatment trades money for speed and choice. The licensed criteria for private prescribing are wider than the NHS groups. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹

A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable. If the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

How do you decide between the routes? If you meet the NHS criteria, ask your GP or local weight-management service where you stand in the rollout. If you do not meet them, or the wait is not workable, a private consultation is the regulated alternative.

09 · Side effects and safety

Side effects and safety: what to weigh beyond price

Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.¹³

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.¹ If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only; implants, coils and injections are unaffected.¹

Which medicine suits you is a decision for a prescriber who has reviewed your health, other medicines and tolerance. Tell them about any new symptom, and you can report a suspected side effect through the MHRA Yellow Card scheme.¹⁹

Never run two GLP-1 medicines together, and never combine Mounjaro with unlicensed research peptides or compounded copies, which have no safety data and no regulated supply chain. If something promises faster results outside a licensed pathway, that is the warning sign.

10 · Tablets and alternatives

Tablets and other licensed alternatives

There is no tablet form of Mounjaro: tirzepatide is injection-only in the UK. Two GLP-1 weight-loss tablets are now licensed in the UK. Wegovy tablets (oral semaglutide, made by Novo Nordisk) were approved by the MHRA in June 2026, and Foundayo (orforglipron, made by Eli Lilly) was approved in August 2026. They are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, and both are prescription-only.²⁰²¹

How you take the tablets matters. Wegovy tablets are taken on an empty stomach after fasting for at least 8 hours, swallowed whole with a sip of water (up to half a glass, about 120ml), and then you wait at least 30 minutes before eating, drinking or taking other tablets.²⁰ Foundayo can be taken any time of day, with or without food and without water restrictions, swallowed whole.²¹ Which routine suits you is a conversation for your prescriber.

Other licensed options work differently. Orlistat is a tablet that blocks some dietary fat absorption and produces smaller average losses. Saxenda (liraglutide) is another licensed injectable, but it is a daily injection, unlike the once-weekly Mounjaro and Wegovy; liraglutide is supplied at 6mg per millilitre. "Natural GLP-1" supplements such as berberine are not medicines, are not regulated for this use and have no comparable trial evidence. They are available cheaply without assessment, but there is no clinical oversight, and dosing is not standardised. Ask a pharmacist before taking any supplement alongside a prescription medicine.

11 · Switching and stopping

Switching between Wegovy and Mounjaro

There is no licensed direct conversion between semaglutide and tirzepatide. Switching is done under prescriber supervision: typically the previous medicine is stopped, a washout interval observed, and Mounjaro started at a dose your prescriber judges appropriate, often the 2.5mg starting dose and sometimes adjusted based on your history. Never run two GLP-1 medicines together.

People switch from Wegovy to Mounjaro for different reasons. Some want to try the dual-action mechanism: tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.¹ Others have found the side effects hard to tolerate, or have been advised by a prescriber that another medicine may suit them better. Your prescriber decides.

From a cost point of view, a switch is a new first order with us. The new-customer discount of £40 applies to the first order at any dose, so a switch at a higher dose still gets it. Moving from Mounjaro to Wegovy is a prescriber decision made on your history, response and tolerance. The prescriber decides when to switch and which dose to start on, Wegovy's licensed dose ladder starts at 0.25mg, and the same rule applies: never overlap the two.

What about stopping altogether? Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears. Plan any exit with your clinical team rather than stopping suddenly because of a cost worry.

12 · Buying safely

Buying safely: a very low price is a warning sign

The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ That is why a price that looks far too low should make you pause rather than click.

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation. Anyone offering to supply without a consultation or prescription is a red flag, whatever the price.¹² Our GPhC registration number is 9012878.

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.¹³

Storage is part of getting what you paid for. Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, which is useful for short trips, and must then be thrown away. Do not use a pen that has been above 30°C. Keep the pen cap on to protect it from light.¹
