Which is safer, Mounjaro or Wegovy?

Neither can be called safer than the other. Both are effective, licensed treatments, and the better choice depends on your health profile, tolerance and prescriber advice, not the headline number alone.¹⁵ A prescriber looks at your medical history, current medicines and previous reactions before recommending either.

Mounjaro suits people whose prescriber judges that the dual GIP and GLP-1 action fits their needs. Wegovy suits people whose prescriber prefers semaglutide, or who already tolerate it well.

03 · Deciding

Decision factors beyond side effects

Your history matters more than any league table. Tell your prescriber about pancreatitis, gallbladder problems, severe digestive disease, diabetic retinopathy, thyroid cancer history, depression and pregnancy plans.¹³ Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. 04 · Side effects in the trials

Which has worse side effects, Wegovy or Mounjaro?

No trial comparison shows that one has clearly worse side effects. In STEP 1, gastrointestinal side effects were reported by 74.2% of people on semaglutide 2.4mg, compared with 47.9% on placebo, and 7.0% stopped treatment because of side effects, compared with 3.1% on placebo.⁹

For Mounjaro, the most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.¹³ Our Mounjaro vs Ozempic side effects guide covers the related semaglutide comparison. Be cautious about any page that quotes a single “discontinuation” gap between the two without naming the trial. Different trials had different designs, so their numbers do not line up.

05 · Serious risks and warning signs

Serious side effects and permanent damage

Serious side effects are uncommon but need urgent attention. For Mounjaro, seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis). Get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems), or for swelling of the face or throat and difficulty breathing (severe allergic reaction).¹³

Is permanent damage a concern? Mounjaro is not known to damage the kidneys or liver directly. The practical risk is dehydration from severe vomiting or diarrhoea, which can strain the kidneys, so keep fluids up and get medical help straight away if you cannot keep fluids down.¹ For Wegovy’s own warnings, check its product information.¹⁸

If you take insulin or a sulfonylurea, watch for low blood sugar (shakiness, sweating, confusion); your prescriber may need to adjust those medicines.¹³ More detail is in our Mounjaro safety guide.

06 · Thyroid, mood and long term

Thyroid, mood and long-term use

In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials. Tell your prescriber if you or a close relative have had medullary thyroid cancer or MEN 2.¹

On mood, the EMA’s safety committee reviewed GLP-1 medicines and concluded in 2024 that the evidence does not support a causal link with suicidal thoughts, and the MHRA keeps this under review.¹⁷ Big changes in weight and appetite can still affect how you feel: tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts.

Heart health, belly fat and “Wegovy face”

Is Wegovy better for heart health than Mounjaro? In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures; these are trial averages, not promises.²

Does Wegovy age your face? Rapid weight loss can change facial volume on any method, including diet or surgery. Raise any new symptom with your prescriber and report suspected side effects through the MHRA Yellow Card scheme.¹⁹

08 · Switching and what to avoid

Switching, stacking and unsafe shortcuts

There is no licensed direct conversion between semaglutide and tirzepatide. Switching happens under prescriber supervision: usually the previous medicine is stopped, a washout interval is observed, and Mounjaro starts at a dose your prescriber judges appropriate, often 2.5mg.¹ Never run two GLP-1 medicines together. Do not combine Mounjaro with unlicensed “research” peptides such as retatrutide, or with compounded copies; these have no safety data and no regulated supply chain.⁷ Offers of faster results outside a licensed pathway are the warning sign.
