What an online doctor consultation involves

An online consultation is a structured health review done through a questionnaire and assessed by a prescriber, replacing a face-to-face appointment. It is a legitimate route in the UK when it is run through a registered pharmacy. You can check any pharmacy on the public register of the General Pharmaceutical Council; ours is registered under number 9012878.12

The process has four steps. First, you complete a short free health questionnaire with no charge and no obligation. Second, a GPhC-registered prescriber reviews your answers. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Why does the review matter? A prescriber has to weigh your BMI, medical history and current medicines before deciding whether a medicine is suitable. That is a clinical decision, and it should never be reduced to a form that approves everyone. 05 · Who qualifies

Who qualifies for prescription weight loss medication

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1

A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.11 Meeting the BMI figure is therefore necessary but not enough on its own. The prescriber may decide a different approach suits you better, and that is the point of a clinical review.

What should you tell the prescriber? Everything you take, including over-the-counter products, and every diagnosis you have. Some medicines need particular care alongside Mounjaro, including warfarin, insulin and sulfonylureas. Be accurate about your height and weight too, because the BMI figure sets the licensed route.

06 · Mounjaro and results

How Mounjaro works and what trials show

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This dual action distinguishes it from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

To make the percentages concrete: for someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg. That is straight arithmetic on the trial averages, not a prediction of your own result, which trial averages do not predict.

The trial ran for 72 weeks, so these figures describe a long course of treatment.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Your prescriber guides the dose you start on and how it changes during your monthly reviews.

07 · NHS or private

NHS or private: the honest picture

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 or above with four or more qualifying conditions.46 Full rollout is expected to take years.

That sequence explains a common question: "Do I qualify for Mounjaro on the NHS?" The answer depends on where you sit in the phased rollout, and your GP is the person to ask. The NHS guide to tirzepatide sets out how it is used for type 2 diabetes and obesity.5

It also explains why many people who meet the licensed criteria choose regulated private treatment rather than waiting. The licensed private criteria are wider than the early NHS groups: a BMI of 30 or above, or 27 to 30 with a weight-related condition.1

Neither route is wrong. If you may be eligible for NHS treatment soon, ask your GP. If you meet the licensed criteria and want to start now, the private route is open. You do not have to choose permanently either; the medicine and dosing are the same licensed product.

08 · Other medicines

Other prescription weight loss medicines

Mounjaro is not the only prescription option, but each medicine has its own product information and your prescriber decides which is appropriate.

Wegovy (semaglutide) is a once-weekly injection. Its pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, with the 2.4mg maintenance dose starting from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.18 Mounjaro works on two gut-hormone receptors, GIP and GLP-1, whereas semaglutide is a single-action GLP-1 medicine.1

Liraglutide (Saxenda, and products such as Erly) is supplied at a concentration of 6mg per millilitre. It is a daily injection, unlike the once-weekly Mounjaro and Wegovy.

Tablets: oral options exist, including Wegovy tablets and Foundayo (orforglipron), each with its own product information.2021 So the answer to "is there a tablet equivalent to Mounjaro?" is that tablets exist but they are different medicines, not Mounjaro in tablet form. Which medicine works best? No licensed medicine is best for everyone; your prescriber decides what suits you. SURMOUNT-5 is the first large head-to-head trial of tirzepatide and semaglutide.15 Individual results vary, and suitability matters as much as average results.

What about appetite suppressants, supplements and "natural" alternatives? Low-cost supplements are available cheaply without assessment, but they come with no clinical oversight, and dosing is not standardised. Dramatic weight-loss claims on them deserve scepticism. 09 · Safety and interactions

Safety, interactions and counterfeit warnings

Safety starts with telling your prescriber everything. Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose. Tell your prescriber about everything you take, especially warfarin or other narrow-margin medicines, insulin, or sulfonylureas, where doses may need reducing to avoid low blood sugar. No adjustment is needed for paracetamol.113

Insulin and sulfonylureas are not forbidden alongside Mounjaro; doses may simply need adjusting. That adjustment is the prescriber's decision, not something to attempt alone.

Contraception is a separate point. The medicine can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 The MHRA has published guidance on contraception with GLP-1 and GIP medicines, so discuss your method with your prescriber before starting.7 Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

Regulators have looked at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee concluded in 2024 that the evidence does not support a causal link, and the MHRA keeps this under continuous review.17 Any big change in weight, appetite and routine can affect how you feel: tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

Any new symptom, whether or not it is a recognised side effect, should be discussed with your prescriber. You can report suspected side effects through the MHRA Yellow Card scheme.19 The patient information leaflet lists the recognised side effects in full.13

Does Mounjaro show up in a routine blood test? The UK Mounjaro patient leaflet lists raised pancreatic enzymes and raised calcitonin in blood tests as common side effects (up to 1 in 10 people). If you expect to have bloods or any other tests, mention your treatment to whoever is organising them and ask your prescriber.

Is Ozempic a weight-loss medicine? Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg).

Counterfeit medicines are a real risk with weight loss injections sold online. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme.7 Buy only from a pharmacy listed on the GPhC register, and be wary of any seller offering to supply without a consultation or prescription. Storage: keep unopened pens in the fridge until first use, and they keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Treatment is a course, not a one-off purchase. Mounjaro starts at the 2.5mg pen and your prescriber steps the dose up through 5mg, 7.5mg, 10mg, 12.5mg and 15mg depending on how you respond. Not everyone needs the highest dose. Some settle at 5mg, some higher. In SURMOUNT-1, average losses were 15% on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, with individual results varying.2 A higher dose is not automatically the right dose for you; the prescriber decides at your monthly reviews.

What happens if you stop? In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

How quickly will I receive my medication?
Once a GPhC-registered prescriber approves your consultation, our registered pharmacy dispenses the medicine. Orders placed by 3pm are dispatched the same day, and delivery is free, next-day and tracked by DPD in discreet temperature-controlled packaging. Saturday delivery is £14.99. If the prescriber decides treatment is not suitable, your order is refunded automatically in full.

You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.
