What is the Mounjaro maintenance dose for diabetes?

The maintenance dose is the weekly strength you stay on once titration has finished, and for many people with type 2 diabetes it is 5mg, 10mg or 15mg. There is no single "diabetes dose" that applies to everyone. The UK product information describes a ladder, and your prescriber moves you along it only as far as you need and can tolerate.¹

Mounjaro (tirzepatide) is a once-weekly injection licensed in the UK for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen, called a KwikPen, in 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg strengths.¹ The dose ladder is the same shape whatever the reason for treatment, but what you are aiming for differs. With diabetes the main goal is steady blood sugar control, with weight loss as a welcome second effect.

Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.¹ After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹

Your response and your tolerance decide it, assessed by your prescriber over time. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.¹ If your blood sugar readings are where you and your clinical team want them on a lower strength, there is no clinical reason to climb further just to reach the top of the ladder.

Three practical questions guide the decision. Are your blood sugar results moving in the right direction? Are side effects manageable day to day? And does the dose fit alongside any other diabetes medicines you take? Your prescriber looks at all three, and a good review asks about each.

The ladder has six strengths, and each has a different job.

Notice what the ladder does not say. It never requires you to reach 15mg. It sets a ceiling, and the right dose for you is somewhere beneath it. Some adults stay on 5mg for as long as they take the medicine because it controls their blood sugar and they tolerate it well.

Each KwikPen holds four weekly doses, so a pen lasts roughly a month at a fixed weekly dose. Your prescriber plans pen strengths so that each titration step lines up with a new pen.¹

05 · Titration in practice

How titration works in real life

Titration is the stepwise increase from the starting dose to the dose that suits you, with at least 4 weeks on each dose. The gap exists so your body can adjust before the next increase. If you feel you are struggling at a step, your prescriber can hold you there for longer rather than push on.¹

This is why the first pen is not a failure of effect if you notice little. Experiences differ widely in the early weeks: some people notice appetite change early on, others feel little until a higher dose, and both are normal.¹ For blood sugar, your own readings and your clinical team's tests tell you more than how hungry you feel.

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.¹ Rotate the site with each dose. There is no best time of day; Mounjaro works the same morning or evening, so choose a day and time you will remember and stick to it.¹³

Life gets in the way, so the missed dose rule matters. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.¹ If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.¹

06 · Diabetes and your team

Working with your diabetes team

Your diabetes team should know about every dose change, because Mounjaro affects blood sugar and can change what your other medicines need to do. Treatment and monitoring are coordinated differently when you have diabetes, so the prescriber who supplies Mounjaro and the clinicians who monitor your condition need to be working from the same picture.¹

If you take insulin or a sulfonylurea, the doses of those medicines may need adjusting when Mounjaro is added or increased, because of the risk of low blood sugar. That does not mean the medicines cannot be used together. It means your clinical team should plan the changes rather than leave you to guess.¹¹

Bring practical information to reviews: your readings or monitor data, any episodes of feeling shaky or unwell, how appetite and eating have changed, and any side effects. Those details help your prescriber decide whether to step up, hold or step down.

NICE has published its own guidance on tirzepatide for diabetes in the NHS setting, and your NHS diabetes team will follow it for NHS-supplied treatment.⁸ If you take Mounjaro privately and also see an NHS team, make sure each knows about the other. Joined-up care is safer than two separate pictures.

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).⁴ Ask your own diabetes team what applies to you.

07 · Side effects and tolerability

Side effects and why they shape your dose

Tolerability is one of the main reasons a maintenance dose is lower than the maximum. The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.¹ Many people find they come back briefly after a dose increase, then fade again.

Many people choose a weekend morning to inject so that early side effects fall on days off. That is a lifestyle tip rather than a rule, but it works well if your week is busy.

Some people do stop treatment because of side effects. In SURMOUNT-1, 4.3% of people on 5mg, 7.1% on 10mg and 6.2% on 15mg stopped treatment because of side effects, compared with 2.6% on placebo.²

If side effects are troubling, tell your prescriber rather than quietly skipping doses. If you have severe, persistent stomach pain, pain in the upper right abdomen with fever, yellowing of the skin or eyes, or signs of a severe allergic reaction, get medical help straight away. For other new or worrying symptoms, tell your prescriber, and you can report suspected side effects through the Yellow Card scheme.¹⁹

What to expect on a maintenance dose

Progress on Mounjaro is gradual by design, and for diabetes you should judge it by trends in your readings and tests rather than a single day. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.¹ Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.

The best-known weight figures come from a weight management trial, not a diabetes one, so treat them as context. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.²

Notice the pattern: higher strengths produced higher average losses in that trial, but each strength delivered meaningful results on average.

Your prescriber reviews response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you (NICE TA1026).¹ Activity matters too: increased physical activity is part of the licence, not an optional extra, and regular moderate activity such as brisk walking plus resistance work for muscle is the evidence-backed pattern.¹

09 · Storage and travel

Storing your pens at maintenance dose

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.¹ Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements, and for hot climates use an insulated pouch.¹³

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit.¹³

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
