Mounjaro Maintenance Dose After Goal Weight (October 2026)
Mounjaro has no separate maintenance product: many people settle at a maintenance dose of 5mg, 10mg or 15mg weekly, using the same KwikPen they titrated on, and higher is not automatically better. In SURMOUNT-4, people who stopped tirzepatide regained on average 14% of body weight over the following year, so stopping should be planned with your prescriber. Prices checked 25 October 2026.

A quick overview
What to know
before you start.
-
Maintenance uses the same licensed weekly doses as treatment; many people settle at 5mg, 10mg or 15mg, and higher is not automatically better.
-
Any move to a lower licensed weekly dose should be planned with your prescriber, not self-adjusted.
-
Stopping Mounjaro is followed by regain on average, so plan diet, activity and review before changing anything.
-
Reorders re-check suitability, and every later order is £25 off the standard price.

01 · The short answer
What is the Mounjaro maintenance dose after goal weight?
There is no single maintenance dose. The licensed ladder runs from 2.5mg to 15mg weekly, and the maintenance dose is simply the strength you and your prescriber judge works best once you reach your goal. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, but higher is not automatically better, and your prescriber will guide the right pace.1
That means the answer for you depends on how you respond and what you tolerate. Someone who reached goal weight on 5mg has no clinical reason to climb higher. Someone who needed 15mg to get there should not assume they can drop sharply without a plan. Both are decisions to make with a prescriber who knows your history.
You can stay on your current licensed dose. You can move to a lower licensed weekly dose if your prescriber agrees. You can plan an exit with diet and activity support.
02 · Your decision
What should you decide when you reach goal weight?
You are really facing a decision between holding, stepping down within the licence, or planning to stop. The right choice depends on your appetite, side effects, how stable your weight has been and what support you have in place. The table sets out the options in plain terms.
| Option | What it means | Licensed? | Points to raise with your prescriber |
|---|---|---|---|
| Hold your current dose | Keep injecting weekly at the strength you reached goal weight on | Yes, within the 2.5mg to 15mg ladder | Whether side effects are manageable and weight is stable |
| Step down to a lower weekly dose | Move to a lower licensed strength, still once weekly | Yes, at your prescriber's direction | How hunger and weight respond, and how closely to monitor |
| Planned exit | Stop treatment with a prescriber-led plan and habits in place | Yes, when planned | Regain risk, diet, activity and a review schedule |
| Stretching pens or fortnightly dosing | Injecting less often than weekly to save money | No | Not recommended: no trial evidence for keeping weight off |
| Microdosing | Taking smaller or spread-out amounts than the pen delivers | No | Unknown dosing accuracy and no safety data |
If cost is the driver, say so. Your prescriber can help you weigh a lower licensed weekly dose, which is better than improvising. See our guide on what maintenance costs for planning.
03 · Next step
Where do you go from here?
If you are already at or near goal weight, start with a conversation about your own numbers rather than a generic schedule. Our companion guide on Mounjaro after reaching goal weight covers the wider picture, and the page on what to do now you have reached goal weight is a practical checklist.
If you are not yet a patient, you can start with a free online consultation through our weight loss clinic. Treatment is supplied only after review by a GPhC-registered prescriber, and the consultation is free with no obligation. If you are ready to look at the pen itself, the Mounjaro pen page shows current prices and availability, with all doses in stock.
A 5mg pen has a standard price of £189.99 per month, and a new customer saves £40 on their first order with code FIRST40. An automatic £25 reorder discount applies to every future order (code DOSE25).
04 · What maintenance means
What does maintenance actually mean on Mounjaro?
Maintenance means staying on a weekly dose that keeps your weight steady once you reach your goal, rather than driving further loss. It is a role a dose plays, not a separate medicine or a separate pen. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly, and it comes as a pre-filled multi-dose pen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Every one of those strengths is a licensed weekly dose, which is why a maintenance dose is simply one of them.
Many websites present maintenance as a special stage with its own rules. The label does not. Everyone starts on 2.5mg once a week, and that starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Maintenance is where you stop moving up, or where your prescriber decides to hold or move down.
Why does the medicine need to keep working at all? Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Those effects are present while the medicine is in your system.
This is also why maintenance is not a reward for finishing the hard part. Mounjaro manages weight; it does not cure the biology behind it. Thinking of maintenance as continued management, with the dose set at the level that does the job with the fewest side effects, is the honest framing.
Maintenance versus titration
During titration, the goal is to find a dose that works and that you tolerate. Your prescriber raises the dose in steps, reviews how you are doing, and stops raising it when you reach the point where progress and tolerability balance. Maintenance begins when weight loss is no longer the aim. At that point the question flips from how much more can we lose to what is the least medicine that keeps this steady.
That flip is a judgement call. It depends on your appetite, your side effects, your daily life and your health conditions. Our page on GLP-1 maintenance dosing covers how the same idea applies across the wider medicine class.
Where to read the label yourself
The patient information leaflet and the product summary are the primary sources for licensed doses.13 If anything you read online seems to differ from them, trust the leaflet and ask your prescriber.
05 · Choosing 5mg, 10mg or 15mg
How do you choose between 5mg, 10mg and 15mg?
You choose with your prescriber, based on how you responded and what you tolerated, not on a rule that says bigger is better. Many people settle at 5mg, 10mg or 15mg, and higher is not automatically better. The intermediate strengths, 7.5mg and 12.5mg, are also licensed, and your prescriber may use them where they suit you.
It helps to see what the headline trial found, with a clear caveat. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
Be careful what you read into those numbers. They describe weight lost during treatment at each dose.
Questions that point towards a dose
Your prescriber is likely to ask practical things. Is your appetite controlled on the current dose, or are you fighting constant hunger? Are side effects such as nausea tolerable, or do they disrupt your week? Has your weight been steady over a sustained stretch, or is it still moving? How confident are you in your eating and activity habits? Honest answers to these matter more than any average.
If you reached goal weight on a lower strength and feel well, holding that dose is usually a sensible default. If you needed the top of the ladder, there is no rule that says you must step down, and there is no rule that says you must stay. The point of review is to decide on evidence from your own response.
Why not just keep climbing?
A higher dose is not a safer or smarter maintenance choice just because it was effective for loss. More medicine can mean more side effects. Once you are at goal weight, extra loss may not be what you want.
Equally, dropping the dose too fast, simply to save money, can leave you with returning hunger and no plan. Weigh both risks. A lower licensed dose followed by close review is a very different thing from a self-made schedule. For people who specifically ask about the top strength, our page on the 15mg maintenance dose goes into that case.
06 · Stepping down safely
Can you step down your Mounjaro dose at goal weight?
Yes, within the licence your prescriber can move you to a lower weekly strength.
Stepping down means choosing another licensed strength and still injecting once a week. It does not mean injecting less often, and it does not mean using part of a dose. The pen is built to deliver set doses, and the licence is built around them.
What a sensible step-down looks like
A careful approach has a few features. The change is agreed with your prescriber before you make it. You monitor appetite, cravings and weight trend afterwards, judging the trend rather than a single weigh-in. And you agree in advance what you will do if hunger returns or weight starts creeping up, such as returning to the previous dose.
What not to do
Altering doses outside the licence means unknown dosing accuracy and no safety data.
Tell your prescriber how you really feel
People often hide how hard hunger has become because they are embarrassed about cost or fear being told to stop. Please do not. A prescriber can only fit the dose to you if they have the real picture. Our ongoing monthly reviews and follow-up assessments exist for exactly this kind of adjustment, and repeat orders re-check suitability, with no auto-shipping, so there is always a point at which a human looks at your situation.
07 · Stretching and fortnightly dosing
Is fortnightly or stretched dosing a maintenance option?
No. Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.1
Why does the idea spread? Because it sounds logical. If the medicine lasts several days in the body, surely you could inject less often once you are at goal weight.
The risks in plain language
And because no trial tested it, nobody can tell you what happens to your weight or safety over months.
There is a further practical point. A pen contains four weekly doses, and the design assumes you use them weekly. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1
What to do instead if cost is the issue
Our page on maintenance dose cost sets this out, and payment options include Klarna and pay-monthly plans with no subscription lock-in.
If you want the full list of what we offer for this topic, our existing guide to maintenance dosing with prescription weight loss support is a good companion read.
A note on changing your injection day
One legitimate flexibility is the injection day. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That is the right tool if your routine changes, rather than stretching the interval.
08 · If you stop treatment
What happens if you stop Mounjaro at goal weight?
On average, weight comes back. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 It is the reason maintenance is a real decision and not a formality.
Mounjaro manages weight; it does not cure the biology behind it.14
How long does it take to leave your system?
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1
Planning a stop properly
Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.14 In practice that means deciding in advance what you will eat, how you will stay active, how often you will weigh yourself, and what threshold of regain would prompt you to call your prescriber. A plan written down before you stop is far more useful than good intentions afterwards.
It is worth separating two situations. Some people stop because they have chosen to, with a plan and support. Others stop because they ran out of pens, hit a cost problem or got busy. The first group is in a much stronger position.
Restarting after a gap
If you stop and later want to restart, do not assume you can resume at your old strength. Because everyone begins on 2.5mg for adjustment, the safest course is to be reviewed again before ordering. Repeat orders re-check suitability, so you will not be auto-shipped anything.
Support while you stop
Aftercare matters here. Our prescribing team offers ongoing monthly reviews and side-effect support, and the free GLPTrackr companion app can help you record weight and notes so trends are visible rather than guessed. If you are weighing up whether to stop or to continue at a lower strength, those records make the conversation with your prescriber much more productive.
09 · Plateaus and weight drift
What if weight plateaus or drifts after goal weight?
Expect some movement; plateaus are normal, and the response is to review habits before touching the dose. Weight loss on Mounjaro is not linear — plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 Once you are in maintenance, small fluctuations are part of the picture, and a single reading means little.
Within the licence, the responses are as follows. Review food intake honestly, because portions drift back up. Protect protein and activity. Check the basics such as sleep and alcohol. And discuss with your prescriber whether a dose change is appropriate.1 A plateau is not a reason to double doses or add unlicensed products.1
Telling noise from a trend
Body weight moves with water, salt, hormones, travel and illness. Judge progress by trend rather than individual weigh-ins. Weighing at the same time on the same day each week, and looking at several readings together, gives you a far clearer view than reacting to a daily number. Early loss in particular can include water weight, which is one reason single readings mislead.
If your weight is creeping up steadily over a sustained period, that is a signal worth acting on early. Small corrections are easier than large ones. Contact your prescriber, describe what has changed in your appetite and routine, and ask whether holding, adjusting or returning to a previous strength makes sense.
Common reasons for drift
Portions grow without anyone deciding they should. Snacking returns when the novelty of reduced appetite wears off. Sleep slips, alcohol rises and activity falls during a busy stretch. Illness or travel can interrupt injections. Each of these is fixable, and most are more about routine than about the dose itself. That is why we suggest looking at habits first and treating the dose as one lever among several.
Missed doses during maintenance
Missed doses can cause an unplanned dip in levels. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 Having this rule clear in your head means a missed dose does not become a reason to panic or double up.
Beyond the scales
It is worth remembering what maintenance is protecting. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average outcomes, not promises, but they are why prescribers treat weight management as health management rather than appearance. Keeping the gains steady is part of keeping those benefits.
10 · Day-to-day practicalities
How do injecting, storage and travel work in maintenance?
Nothing about the routine changes just because you are at goal weight: you inject once a week, on the same day, and store the pens properly. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day. It works the same morning or evening, and what matters is the same day each week. Pick a day and time you will remember and stick to it.
Injection sites and technique
Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses.1 Needles, a sharps bin and injection guidance come with every order, and the pen's own instructions tell you how to deliver the dose, so follow them rather than relying on habit.
Storage
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 Unopened pens belong in the fridge until first use and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.
Travel
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1
Why routine matters more in maintenance
During titration there is a built-in rhythm of dose increases and reviews. In maintenance that structure fades, and it becomes easy to let small things slide: a missed dose here, a late injection there, a pen left out of the fridge. Setting a recurring reminder, keeping a spare pen date noted, and recording your weight and dose in the free GLPTrackr app all help you stay consistent without much effort.
Reading the leaflet
The patient information leaflet is the authoritative source for side effects and warnings, and it is worth rereading when you move between strengths.13 If you notice a new symptom, speak to your prescriber, and you can report suspected side effects through the Yellow Card scheme.19 Reporting helps regulators monitor safety, and it is open to patients as well as professionals.
Supply and genuine pens
Maintenance often means ordering regularly for a long time, so supply matters. We supply Mounjaro through official LillyDirect supply, direct from Eli Lilly's UK distribution, as genuine KwikPens, with free next-day tracked delivery in temperature-controlled packaging. Our registration is GPhC 9012878, and you can check it on the public register.12 Be wary of anyone offering to supply without a consultation or prescription.7
11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
12 · Costs and NHS access
What does maintenance cost, and what about the NHS?
Maintenance costs the standard price of whichever pen strength you settle on, with every later order £25 off the standard price. Prices checked 25 October 2026: a 5mg pen has a standard price of £189.99 per month, 7.5mg is £244.99, 10mg is £274.99, 12.5mg is £289.99 and 15mg is £299.99.
Year planning
At standard prices, a first year costs £2,399.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,279.88 if you step up through every dose to 15mg. These are before discounts: a new customer's first order is £40 less with code FIRST40, and every later order is £25 less (code DOSE25). If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £879.96 at standard prices. For other maintenance strengths, plan around that strength's monthly price rather than a year figure.
If you are switching to us from another provider at a higher dose, the new-customer discount still applies to your first order at any dose. For people comparing providers, across the UK pharmacies we track (checked 25 October 2026), pens range from £143.96 to £319.99 per month depending on dose and provider. We show our own prices live so you can compare like with like.
Paying your way
Payment is by Visa or Mastercard, or Klarna, with pay-monthly options and no subscriptions. The consultation is free, and the price shown is the price paid, with no hidden fees. Delivery is free next-day tracked, with same-day dispatch when ordered by 3pm. You can see current pricing and availability on the Mounjaro pen page, and a stock check from our side is available on the guide to maintenance dose availability.
The NHS picture
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6 If you are on the NHS pathway, your maintenance plan will be set by your NHS clinical team, so follow their advice.
If you take medicine for diabetes, such as insulin, speak to your prescriber about dose changes. You can read about diabetes-specific planning in our guide to maintenance dosing with diabetes.
How ordering works
Ordering has a few simple steps. First, complete a free online consultation, a short health questionnaire with no charge and no obligation. Second, a GPhC-registered prescriber reviews it, and if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Third, our registered pharmacy dispenses and dispatches, and your pen arrives by free next-day tracked delivery in discreet, temperature-controlled packaging. Saturday delivery is £14.99. Support is available Monday to Friday, 9am to 5pm, by email, account chat or phone on 0333 207 0413.
