03 · What the trial can tell you

What the main trial can and cannot tell you

The main trial figures describe weight loss while on treatment, so they help you set expectations about what you have already achieved rather than what happens next. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

That last point matters most at goal weight. Trial averages do not predict your own result, and they do not tell you how you will do after stopping. Ask your prescriber to talk through continuing versus stopping in the context of your own history. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

04 · How the medicine clears

What happens as the medicine leaves your system

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears. That means the change in appetite is usually gradual rather than sudden, and the first few weeks after your last injection are not a reliable guide to how you will feel later.

Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite and slow how quickly the stomach empties.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. As the medicine clears, that extra support fades, so the structure you have built around meals matters more.

Keep using the pen exactly as your leaflet and prescriber describe while you are still taking it. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. Keep pens in a fridge at 2–8°C and never freeze them. Use the patient information leaflet for any storage question beyond that.13

Start with protein and structure. For example, agree with yourself that if your weight drifts upward over several weeks or hunger returns strongly, you will contact your prescriber instead of waiting. 06 · Stepping down safely

Stepping down: what a sensible approach looks like

A step down should be planned with your prescriber and based on how you respond, not on a calendar you found online. Mounjaro comes in strengths from 2.5mg up to 15mg, so a lower strength is possible within the licence.1 The right pace varies a lot between people, so your prescriber guides it.

Ask your prescriber how long to stay on the new strength before judging it, what changes in appetite or weight would prompt a rethink, and whether you should go back up if things drift. These questions turn a vague experiment into a monitored plan.

Avoid improvising. Do not skip doses, stretch the interval between injections or try to split a pen to make it last. Those approaches are not part of the licensed use of the medicine and can leave you without the support you think you have.

07 · NHS and private routes

NHS and private routes after goal weight

Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).6 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you started privately and are thinking about the longer term, ask your GP how NHS services might fit your situation. The rules for who is offered treatment, and for how long, are set by the NHS and NICE, and your GP can tell you where you stand locally.

08 · When to get advice

When to talk to someone sooner

Contact your prescriber promptly if you feel unwell, if side effects continue or return, or if your relationship with food or your weight is causing you distress. If you have thoughts of harming yourself or ending your life, get help straight away: call 999 or go to A&E if you are in immediate danger, call NHS 111 and choose the mental health option, or call Samaritans free on 116 123, day or night. Reaching a goal can bring mixed feelings, including worry about regain, and those are worth raising rather than carrying alone.

You should also speak to your prescriber if you find your weight continuing to fall when you do not want it to, or if you are unsure whether your current weight is healthy for you. A review is the right place to settle that question, because it takes your height, history and health conditions into account.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
