What a Mounjaro 5mg KwikPen costs

The Mounjaro pen at 5mg costs £189.99 as a standard price, for one pen that lasts four weeks. That works out at about £47.50 per week.

Two discounts sit on top of the standard price. A new customer's first order, at any dose, is £40 less with code FIRST40. After that, an automatic £25 reorder discount applies to every future order (code DOSE25).

The discount is not limited to the starting pen. If you are switching to us from another provider part-way through treatment, the first-order discount still applies to your first pen at your current dose.

How ordering works

Ordering takes three steps. First, complete the free online consultation, a short health questionnaire with no charge and no obligation. Second, a GPhC-registered prescriber reviews it. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Third, our registered pharmacy dispenses your pen and sends it by free next-day tracked delivery.

What a 5mg KwikPen is and what it contains

The Mounjaro 5mg KwikPen is a pre-filled, multi-dose pen containing tirzepatide, the active ingredient in Mounjaro, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. The pen comes in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

Each KwikPen contains four weekly doses.

Where 5mg sits on the dose ladder

Everyone starts Mounjaro on 2.5mg once a week. That starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks, the dose increases to 5mg. From there it can step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹

So 5mg is usually your second pen. Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.¹ If you tolerate 5mg well and are responding, staying there is a legitimate clinical decision, not a failure to progress.

What the pen is licensed for

Mounjaro is licensed for weight management in adults and for type 2 diabetes.¹¹ Our 5mg KwikPen uses guide explains how the dose is used in practice. Whatever the reason you are prescribed it, the pen is used once a week, on the same day each week.

A prescriber decides which dose is right for you and when to move up. Your first pen will almost always be 2.5mg, not 5mg. If you are already on 5mg with another provider, tell the prescriber during your consultation so they can assess it with your history.

05 · Year cost planning

Planning the cost of a year on Mounjaro

A first year of treatment is 52 weeks, or 13 pens. A first year costs £2,399.88 at standard prices if you stay on 5mg after your 2.5mg starting pen. It costs up to £3,279.88 if you step up through every dose to 15mg. A new customer's first order is £40 less, and every later order is £25 less than the standard price.

If you step up after 4 weeks on each of 2.5mg, 5mg, 7.5mg and 10mg, your first four pens cost £879.96 at standard prices. Your 5mg pen is the second of those four. Not everyone moves up after 4 weeks, because a prescriber may hold you at a dose if you are responding well or having side effects.

For maintenance doses between those, use the monthly price: £244.99 at 7.5mg, £274.99 at 10mg and £289.99 at 12.5mg. These are monthly prices, not year costs.

Spreading the payment

06 · What 5mg can do

What results to expect at 5mg

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg over 72 weeks, compared with 3.1% with lifestyle changes alone. Those are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.²

At 72 weeks, 85% of participants on 5mg had lost at least 5% of their body weight.² As a worked example only: for someone starting at 100 kg, 15% is about 15 kg. Trial averages do not predict your own result.

For context: the other doses in the same trial

The same trial reported average losses of 19.5% on 10mg and 20.9% on 15mg over 72 weeks. At 72 weeks, 89% of participants on 10mg and 91% on 15mg had lost at least 5%. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.² These figures show how results vary by dose in a trial. They do not mean you need a higher dose, and your prescriber will guide what is right for you.

A realistic pace

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.¹

Your prescriber reviews your response. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.¹

What the first weeks feel like

Experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹ If a new symptom worries you, speak to your prescriber, and you can also report side effects through the Yellow Card scheme.¹⁹

07 · Who can be prescribed it

Who can be prescribed Mounjaro privately

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.¹ It must be used alongside a reduced-calorie diet and increased physical activity.

A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable. The consultation is free, and a GPhC-registered prescriber reviews every one. It is a clinical review, not a scoring script. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

Why your current medicines matter

The prescriber needs to know every medicine you take. Some interactions change how a dose should be managed. For example, Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed, and doses of insulin or sulfonylureas may need adjusting because of the risk of low blood sugar.⁷ The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ Tell the prescriber about any of these during your consultation, and never change a dose of another medicine yourself.

Starting at 5mg?

Everyone starts on 2.5mg. A new customer who is not already on treatment will not be prescribed 5mg first. The prescriber will usually begin you on the starter pen, then move to 5mg after at least 4 weeks if it suits you.¹ If you are already on 5mg elsewhere, say so in the consultation so your history is assessed with that in mind.

Ongoing reviews

Prescribing does not stop after the first order. Ongoing monthly reviews, dose-titration and follow-up assessments are part of the service, along with side-effect support. 08 · NHS or private

NHS or private: which route suits you

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶

Full rollout is expected to take years. That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. The NHS guide to tirzepatide explains who it is for and how it is used.⁵

How to decide

Start with your GP. If you fall into a group the NHS is currently prioritising, NHS treatment may be open to you. If you do not, or you do not want to wait, a regulated private route is the alternative.

Private treatment means paying for the pen. For 5mg that is £189.99 per pen at the standard price; a new customer's first order is £40 less, and every later order is £25 less. NHS treatment means the normal prescription charge system applies; in England the single prescription charge is £9.90. Whether you would be charged that for Mounjaro depends on your own exemptions, so check with your GP or pharmacist.

Diabetes and the NHS

Mounjaro is also licensed for type 2 diabetes, and NICE has a separate appraisal for that use.⁸ If you have type 2 diabetes, your diabetes team is the right first conversation about NHS options. Do not stop or change diabetes medicines yourself.

Switching routes

If you begin privately and later qualify for NHS treatment, or the other way around, tell both prescribers what you have been taking, including doses and dates. Keeping a record is easier with an app such as GLPTrackr. Sharing your history lets the next prescriber assess your current dose safely and decide the right starting point for you.

09 · Using the 5mg pen

How to use the 5mg pen week to week

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Choosing a day and time

There is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off.¹

If you miss a dose

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose and take the next one on your normal day. Never take two doses within 3 days of each other.¹

Changing your injection day

You can change your weekly injection day if at least 3 days (72 hours) have passed since your last dose.¹ Write the new day down, and use a reminder or GLPTrackr so the weekly rhythm holds.

When the pen is finished

Each pen is designed and licensed for exactly four weekly doses. When the fourth is done, dispose of the pen in your sharps bin. Your prescriber will tell you whether the next pen is the same dose or a step up.¹ Reorder in good time, as we dispatch the same day when you order by 3pm, and delivery is next-day.

10 · Storage and travel

Storing and travelling with your pen

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy and the patient information leaflet before using it, rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels.¹³

Flying and holidays

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.¹

Checking a pen is genuine and still good

A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been out of the fridge for more than 30 days, or above 30°C in use.¹³

If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.¹² Our own pens arrive in temperature-controlled packaging by free next-day tracked delivery, so a pen that arrives warm, damaged or unexpected should be reported to us before use. 11 · Fakes and false savings

Fake pens and false savings

The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ A very low price for a 5mg pen is a reason for caution, not excitement.

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation, so anyone offering to supply without a consultation or prescription is not operating legitimately.⁷¹² Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked.¹ Our supply is official LillyDirect supply, direct from Eli Lilly's UK distribution.

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹ Do not use Mounjaro if you are pregnant, and stop it at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
