06 · Results at 5mg

Do you lose more weight on 5mg Mounjaro, and what can you expect?

You can expect more progress on 5mg than on the 2.5mg starter, because weight loss on Mounjaro is gradual by design and most measurable progress builds from 5mg onward, month by month.1 Whether you lose more than someone on a different dose depends on the person, and higher strengths are not automatically better.

What the main trial found

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 So higher doses produced larger average losses in the trial, but 5mg was already associated with substantial results for most people who took it. Trial averages do not predict your own result.

A worked example

If someone started at 100 kg, an average of 15% would be about 15 kg. That is straight arithmetic on the trial figure, not a forecast for you, and it says nothing about how quickly any individual would reach it.

Pace matters

The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.1 If you step on the scales every morning and see numbers move up and down, that is ordinary variation and not a sign that the medicine has stopped working.

What should you expect and avoid while taking Mounjaro 5mg?

You should expect mild digestive and energy-related effects in the days after an injection, and you should avoid changing your dose or schedule on your own.

The commonest early effects

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 Because the medicine slows how quickly the stomach empties, some people notice they feel full sooner, and that can feel unfamiliar at first. These effects can reappear briefly when you step up in strength.

Many people choose a weekend morning for their injection so that early effects fall on days off. That is a practical tip, not a medical requirement, and any day works as long as you keep it consistent.

What to avoid

  • Do not alter your doses. Altering doses outside the licence means unknown dosing accuracy and no safety data.
  • Do not use a pen that has been frozen. Keep it in the fridge at 2–8°C.
  • Do not buy from sellers offering to supply without a consultation or prescription. Mounjaro is prescription-only, and the MHRA warns about counterfeit medicines.7

How do you inject Mounjaro 5mg, and what about the pen clicks?

You inject Mounjaro 5mg under the skin once a week, on the same day each week, at any time of day, with or without food.1

Where and how

Mounjaro is injected under the skin (subcutaneously). Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1 Rotating helps avoid irritation in one patch of skin. How many clicks does the pen make?

This is a common question, and the honest answer is that it is the wrong thing to count. The Mounjaro KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window. Each KwikPen contains four weekly doses.

Choosing a time of day

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember and stick to it.1

If you miss a dose

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

Changing your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That is the full rule.

09 · Storage and travel

How should you store Mounjaro 5mg, and why not use it after 30 days (not above 30°C)?

Store Mounjaro pens in the fridge at 2–8°C, never frozen, and discard an in-use pen after 30 days (not above 30°C) out of the fridge.

The storage rules

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

Why discard it after that window? Past it, sterility and potency are no longer assured, so the pen sits outside the tested window. Do not try to squeeze extra doses from it.

Unopened pens

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1 Our delivery uses discreet, temperature-controlled packaging, and you should move your pens into the fridge soon after they arrive.

Travelling with 5mg

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.1

A sensible approach is to pack your pen with the original box and label, keep it with you rather than checking it, and avoid leaving it in a parked car or beside a window. If you travel often, ask your prescriber about timing your injection day so it falls conveniently.

What counts as a damaged pen

Do not use a pen that has been frozen, a pen that looks damaged, or one that will not dial properly. If your pen arrives faulty, damaged or the wrong item, contact us within 3 days and we will put it right. Dispensed prescription medicines cannot otherwise be returned.

Why genuine supply matters

Correct storage only protects what you start with. 10 · Stopping and staying on 5mg

Can you stay on 5mg, or just stop taking it?

You can stay on 5mg for as long as your prescriber agrees it is right for you, and you can stop, but stopping should be a conversation first.

Staying at 5mg

There is no rule that you must climb the ladder. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber will guide the right pace.1 Staying on 5mg for a few months, or longer, is therefore entirely reasonable if your progress and tolerance are good. The step to the next strength is always a minimum interval, never a deadline.

Your response is reviewed along the way. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Note that this refers to the highest dose you tolerate, so if 5mg is your tolerated maximum, that is the dose that review applies to.

Stopping

If you decide to stop, tell your prescriber so they can support you and update your record. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

Stopping medicine does not remove the habits you built. Keeping up the eating pattern and activity you developed gives you the best chance of holding your progress. Ask your prescriber what support is available, because monthly reviews and follow-up assessments are part of the service.

Pausing, stepping down and restarting

Life interrupts treatment: illness, holidays, bad weeks. If you miss doses, follow the missed-dose rule rather than doubling up. If you have been off the medicine for a while, do not assume you can restart at your old strength. Speak to the prescriber, who will advise on the right point on the ladder.

What not to do

Do not try to make a pen last longer by spacing doses further apart, and do not combine leftover pens in ways the device was not designed for.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · NHS and private access

Can you get Mounjaro 5mg on the NHS, or do you go private?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

What this means for you

If you may qualify for NHS treatment, speak to your GP and ask where you sit in the phased rollout. Expect gradual progress rather than sudden change. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle. Weight loss is gradual by design, so think in months, not days, and judge progress by trend rather than individual weigh-ins. Your prescriber reviews your response, and you should report anything that worries you, including through the Yellow Card scheme.1
What is the difference between Mounjaro 2.5mg and 5mg doses?
The difference is the amount of tirzepatide in each weekly injection and what each dose is for. The 2.5mg starter dose is about letting your body adjust, not about weight loss itself. The 5mg dose is the next step, usually after at least 4 weeks, and is where progress tends to build. Both are used in exactly the same way, once a week, and your prescriber decides when you move up.1
Can I just stop taking 5mg of Mounjaro?
You can stop, but tell your prescriber first so they can support you and update your record. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system, there is no way to speed this up, and nothing needs doing while it clears. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average, so stopping should always be planned with your prescriber. Appetite effects therefore fade gradually. Keep up the eating and activity habits you have built, and ask about follow-up support from the prescribing team.1
Can I stay on 5mg Mounjaro for a few months?
Yes, if your prescriber agrees. There is no rule that you must keep climbing the ladder, because many people settle at a maintenance dose of 5mg, 10mg or 15mg and higher is not automatically better. Dose increases come at a minimum interval, never a deadline. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether to continue, adjust or stop.1
Why can't you use Mounjaro after 30 days?
Once a pen is in use, it can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded. Beyond that window the pen is outside what has been tested, so sterility and potency are no longer assured. Unopened pens stay in the fridge at 2–8°C until the printed expiry date. If an unopened pen has been out of the fridge, check with the pharmacy before using it.1
What should I avoid while taking Mounjaro?
Avoid changing your dose or schedule yourself. Do not use a pen that has been frozen, and do not buy from anyone offering to supply without a consultation or prescription. Read the patient leaflet, tell your prescriber about your other medicines, and report any worrying symptom.17
How many clicks does the Mounjaro pen make?
You should not count clicks at all. The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window, and you confirm the dose by the number in the window.
