Mounjaro Injection on NHS Prescription: Who Qualifies (October 2026)

01 · The short answer

Can you get a Mounjaro injection on the NHS?

Yes, you can get Mounjaro on the NHS, but only if you fall within the groups being prioritised. Access is being phased in over several years rather than opened to everyone at once.⁴⁶

NICE recommends tirzepatide for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially people with a BMI of 40 or above and four or more qualifying conditions.⁴⁶ Full rollout is expected to take years.

NHS prescription or private prescription?

The right route depends on whether you meet the current NHS criteria and how long you are willing to wait. Both routes lead to the same medicine, tirzepatide, used alongside a reduced-calorie diet and increased physical activity.

04 · NHS eligibility explained

NHS eligibility: what the criteria say

The NHS criteria are narrower than the licence, because the NHS is introducing the medicine in stages. NICE recommends tirzepatide for adults with a BMI of at least 35 plus at least one weight-related condition.⁴

In practice, the primary-care rollout that began in June 2025 started with the highest-need groups, initially BMI 40 or above with four or more qualifying conditions.⁶ Over time, further groups are expected to become eligible as the rollout continues. Your GP practice is the best source for what applies where you live.

Ethnic background matters too. People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight-related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups.⁴

05 · Asking your GP

How to ask your GP about Mounjaro

The best way to start is a routine appointment where you ask directly whether you fall within the groups currently being offered tirzepatide. Your GP can check your BMI, your weight-related conditions and how local access is organised.

Before you go, it helps to gather a few things:

• Your recent weight and height, so your BMI can be calculated or confirmed.
• A list of any weight-related conditions you have been diagnosed with, such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.
• Your current medicines, including anything bought over the counter.
• Previous weight-management attempts, such as diet, activity programmes or other treatments.

Your GP may tell you that you do not yet meet the criteria being used locally, or that you are in a group invited later. That is a reflection of the phased rollout, not a judgement about whether you need support. Ask what other help is available in the meantime, and whether a specialist weight-management service operates in your area. 06 · Licensed use and safety

Licensed eligibility and who should not take it

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹¹¹ A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.

Whichever route you take, safety checks are the same. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients. It must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. A prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.¹¹³ Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials. Tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. Routine thyroid monitoring is not required for most people.¹

07 · How it works

What Mounjaro is and how it works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹

It belongs to the family of medicines that act on gut hormones, often called GLP-1 medicines. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.¹

This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).¹ Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.¹

08 · Dosing and results

Dose ladder and what results look like

Everyone starts on 2.5mg once a week, and this starting dose is about letting your body adjust rather than weight loss itself. After at least 4 weeks the dose increases to 5mg, then can step up by 2.5mg at minimum 4-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on response and tolerance. Many people settle at 5mg, 10mg or 15mg, and higher is not automatically better.¹

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages and individual results vary.

Progress is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest, with most measurable progress building from 5mg onward. Judge progress by trend, not individual weigh-ins. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.¹²

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹ Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.

09 · Planning private costs
