Introduce fibre gradually. A sudden jump can bring wind and bloating, and your stomach already empties more slowly. Add one extra portion of vegetables or one swap to a whole-grain version, give your gut some time, then add more.

Portion size matters too. Large portions sit heavily because the stomach empties more slowly, and many people feel worse after very fatty, fried or very sugary food.1 Smaller, slower meals work better. Eat slowly and stop at the first feeling of fullness.1

Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.1 Constipation often follows that pattern, so a bout after a step up is common.

The general pattern is that effects appear in the first days after starting or increasing a dose, peak early, and settle as your body adapts.

If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Speak to your clinical team rather than pushing through severe symptoms.1 Never change your dose or skip steps by yourself.

Some symptoms can point to something more serious and need urgent attention. Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting, which may be pancreatitis. Also get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which may be a gallbladder problem; the same applies to signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.13

Constipation on its own is not an emergency, but constipation combined with severe pain and vomiting should not be treated as routine. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.1

You can also report any suspected side effect through the MHRA Yellow Card scheme.19 If you are unsure whether a new symptom is related to the medicine, describe it to your prescriber or pharmacist rather than guessing.

If you decide treatment is not right for you, tirzepatide has a half-life of about five days, and after a final dose it takes roughly four to five weeks to clear the system.1 Nothing needs doing while it clears.
