What to do instead of trying to flush it out
The useful question is usually not how to remove Mounjaro, but what problem you are trying to solve. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That clearance is gradual and cannot be hurried by anything you drink, take or do.
So the decision depends on your reason. If you have decided to stop, you simply stop injecting; you do not need to do anything else. What it costs to continue or restart, and the NHS picture
If you stop and later restart privately, the Mounjaro pen costs £169.99 per month at the 2.5mg starting dose, £218.99 at 5mg, £279.99 at 7.5mg and £290.99 at 10mg. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. On the NHS, Mounjaro is available, but access is being phased in over several years. 05 · Half-life explained
How long Mounjaro stays in your system
Mounjaro stays in your system for roughly 4–5 weeks after your final dose. The active ingredient, tirzepatide, has a half-life of about five days.113
A half-life is the time the body takes to reduce the amount of a medicine by half. It is a rate, not an expiry date. Because tirzepatide is injected once a week and clears slowly, the medicine is still present between doses and for weeks after the last one. This is by design: it is what allows one injection every seven days rather than daily dosing.
Is it true that Mounjaro wears off after 5 days? No. Five days is the half-life, not the point at which the effect disappears. After your last injection the medicine keeps clearing for roughly 4–5 weeks. You may notice appetite creeping back before it has fully cleared, but that is a separate matter from how long the drug takes to leave your body.
Why does this matter in practice? First, if you are worried about a side effect, stopping will not make it vanish overnight; contact your prescriber instead of waiting. Second, if you are planning a procedure, a pregnancy or a change of medicines, tell the clinician involved that you take or have recently taken tirzepatide so they can advise on timing. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Third, if you restart after a break, the medicine may not have fully cleared, which is one reason restarting needs a prescriber's guidance.
Nothing needs doing while it clears. You do not need a detox, a special diet, extra water or a particular exercise routine. If you have a specific worry about how long it will stay, your prescribing team can discuss it with your medical history in mind.
06 · Detox and flushing myths
Can Mounjaro be flushed out of your system?
No, Mounjaro cannot be flushed out of your system. There is no way to speed up clearance of tirzepatide, and nothing needs doing while it clears.1 That rules out the popular suggestions you will see online.
Drinking extra water is harmless in normal amounts, but it does not change how quickly tirzepatide is cleared. Exercise is good for you and works alongside treatment, but it will not shorten the 4–5 week window. Fasting is a poor idea if you already feel unwell or nauseous on treatment, and it will not hasten clearance either. Detox teas, cleanses and so-called natural GLP-1 products are sold with dramatic claims, but none can alter how the body handles this medicine. Low-cost supplements are available without any assessment or clinical oversight, and their dosing is not standardised, so they add uncertainty rather than safety.
Be especially wary of anyone offering a product that claims to "reverse" or "neutralise" Mounjaro. There is no antidote sold over the counter. Sellers offering to supply medicines without a consultation or prescription are a separate danger: the MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected problems.7 If you have bought something online and feel unwell, seek medical advice and report it.
It is also worth separating two different goals. If you want the drug out because of side effects, the real fix is a conversation with your prescriber, who can adjust the plan. If you want it out because you have changed your mind about treatment, you can simply stop; the medicine will clear on its own. In neither case does a flush help.

07 · Getting into your system
How long does Mounjaro take to get into your system?
Mounjaro is started gradually, with the dose stepped up over weeks rather than all at once.
Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at 5mg, 10mg or 15mg; higher is not automatically better.
In the early weeks, experiences differ widely. Some people notice an appetite change and early loss in week one, while others feel little until a higher dose; both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13
What matters most is consistency rather than the time of day. You inject under the skin once a week, on the same day each week, at any time of day, with or without food. There is no best time of day; many people pick a weekend morning so early side effects fall on days off.1
Weight loss is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Think in months, not days, and judge progress by trend.1 Three months in, most people are still on the lower rungs of the dose ladder. Slower early progress does not predict failure.

08 · Managing side effects
How to get rid of Mounjaro side effects
The most reliable way to deal with Mounjaro side effects is to speak to your prescriber, because the medicine cannot be removed faster and the dose can be adjusted. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 If yours are not settling, do not wait it out silently.
Because tirzepatide takes roughly 4–5 weeks to clear after a final dose, a side effect may not disappear the moment you stop. That is expected rather than a sign something has gone wrong, but it is a good reason to be in touch with your prescriber instead of simply abandoning treatment and hoping.

Where symptoms are new, unusual or worrying, tell your prescriber. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

09 · Stopping and restarting
What happens when you stop or restart Mounjaro
When you stop Mounjaro, the medicine clears over roughly 4–5 weeks, and you do not need to do anything while it does.1 What you may notice in that time is a matter for your own observation and your prescriber's advice. If you feel unwell after stopping, contact your prescriber or GP.
Is it safe to restart after a 3-week break? Your prescriber decides where to restart after a gap. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1
A three-week break is well beyond a single missed dose, so do not simply carry on as before. Contact your prescriber, tell them how long you have been off and which dose you were on, and let them decide where to restart. Restarting at your previous high dose without advice is not something we would suggest.
If you need to change your injection day, you can, as long as at least 3 days (72 hours) have passed since your last dose.1 This is useful if your schedule changes and avoids missing doses altogether.

10 · Weight after stopping
What happens to your weight and appetite after stopping
After stopping Mounjaro, the medicine clears over roughly 4–5 weeks.1 In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
To understand why, it helps to recall how the medicine works. Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1
For context on what treatment achieves while you are on it: in the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2 For someone starting at 100 kg, 20.9% is about 21 kg. Those figures describe treatment, not what happens after stopping.
In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes rather than promises.2
If your reason for wanting Mounjaro out is that progress has stalled, note that plateaus of a few weeks are normal as the body adapts to a lower weight. Within the licence, review food intake honestly, protect protein and activity, check sleep and alcohol, and ask your prescriber whether a dose step-up is appropriate if you are not yet at maximum. A plateau is not a reason to double doses or add unlicensed products.1
If you lose less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you. Any decision to stop is best made with them, ideally with a plan for diet and activity afterwards. Treatment is used alongside a reduced-calorie diet and increased physical activity, not instead of them.
11 · Storage and leftover pens
Storing pens and what to do with leftovers
If you stop Mounjaro with a pen part-used, the key rule is that an in-use pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.13 Do not use a pen that has been above 30°C or has been frozen.
Keep pens in a fridge at 2–8°C and never freeze them. Keep the pen cap on to protect it from light, and never use a pen that has been frozen. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.13
Each KwikPen contains four weekly doses. If you pause treatment, an in-use pen cannot simply be saved for months; its 30-day out-of-fridge allowance still applies once you have started it.
Travelling is straightforward. You can fly with Mounjaro: carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.13

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
