A useful way to think about it is in stages. First comes the titration, where the dose steps up gradually. Then comes the period on the dose that suits you. After that comes a review of whether to continue, change or stop. Each stage is a decision point, not a deadline.

If you are asking because of cost, side effects or a planned life event, say so at your review. A good prescriber will plan around it rather than simply telling you to carry on.

04 · What the trials measured

What the trials measured, and over how long

The main trial of Mounjaro in weight management is SURMOUNT-1, a trial of 2,539 adults. People taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

The point for your question is the timescale. Those results were measured at 72 weeks, which is well over a year. Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.1

05 · The early months

The early months: what to expect

The early months are mostly about adjustment, so modest numbers on the scales are normal. The 2.5mg starting weeks are about adjustment, and experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.

By the end of the early months, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures. A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.1

Slower early progress does not predict failure, because the dose you respond to may still be ahead of you. This is the main reason a very short course often feels disappointing. You may stop just before the doses that do most of the work. Weight loss is gradual by design, and most measurable progress builds from 5mg onward, month by month.1

06 · The titration phase

The dose ladder

Mounjaro comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Each pen contains four weekly doses. The dose is stepped up gradually rather than started high, which is why the first part of any course is slower.

The total time depends on where you settle.

Your prescriber may hold you at a dose for longer if side effects are troublesome, or if you are responding well and do not need to go higher. Neither is a failure. It simply stretches the timeline in a sensible direction.

One thing to avoid is changing your own schedule. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use and must then be thrown away; your prescriber will plan doses around the licensed schedule.13

07 · Plateaus and reviews

Plateaus, reviews and when to reassess

A plateau is not a signal to quit. Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.1 Within the licence, the responses are sensible and practical:

  • Review food intake honestly, because portions drift back up.
  • Protect protein and activity.
  • Check the basics, such as sleep and alcohol.
  • Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.

A plateau is not a reason to double doses or add unlicensed products.1

When you take your final dose, the medicine leaves your body slowly. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

If you stop and later want to restart, tell your prescriber how long the gap was. Restarting is a prescriber decision, not a do-it-yourself one.

If you are stopping because of a side effect, report it through the Yellow Card scheme as well as telling your prescriber.7 In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.

09 · Beyond the scales

Health benefits that shape the decision

Weight is only part of the picture. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.1 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

That framing changes the question. Instead of asking only how many months it takes to reach a number, ask what you and your prescriber want the treatment to achieve for your health, and how you will know it has. For some people the answer is a defined target. For others it is ongoing management, much like other long-term treatments.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

NHS and private routes

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Serious side effects are uncommon: seek immediate medical help for severe, persistent stomach pain that may spread to your back, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or signs of a severe allergic reaction. Report any suspected side effect through the Yellow Card scheme.
What should I avoid while taking Mounjaro?
Avoid changing your dose or schedule yourself, doubling up, or adding unlicensed products. Review alcohol and sleep if progress stalls, and tell your prescriber about every other medicine you take. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1
Is Mounjaro a lifelong medicine?
Not necessarily. For weight management the licence sets no fixed end date, so the length is decided by your response, tolerability, health and goals, reviewed regularly with your prescriber. Some people continue for a long time, others stop after reaching their goals. Make that decision with your prescriber, not on your own.
