In the SURMOUNT-1 trial of 2,539 adults, results were measured at 72 weeks, so six months sits comfortably inside the period studied.2

There is no fixed six-month cut-off. Your prescriber reviews how you are responding, how you are tolerating each dose and whether continuing or stopping is right for you. Treatment is used alongside a reduced-calorie diet and increased physical activity, not instead of them.1

What results to expect by six months

Trial averages show weight loss builds gradually, so six months is part-way through the course, not the end of it. In SURMOUNT-1, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, against 3.1% with lifestyle changes alone.2 Those are 72-week averages, not six-month figures, and individual results vary.

To put it in numbers: for someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg over the full 72 weeks. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2

At this stage the pace matters more than any single result. Weight loss on Mounjaro is gradual by design, so think in months, not days, and judge progress by trend rather than individual weigh-ins. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

05 · Reviews and the 5% check

How your prescriber reviews progress

If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.

That is a conversation, not an automatic cut-off. It looks at your dose, side effects, eating pattern and goals. A prescriber may also consider what else has changed, such as sleep, alcohol or new medicines.

Beyond the scales, trial participants also saw average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises. They are why prescribers talk about weight management as health management.

06 · Plateaus along the way

What if progress stalls before six months?

A plateau of a few weeks is normal and does not mean the medicine has stopped working. Weight loss on Mounjaro is not linear, and the body adapts to a lower weight.

Within the licence, the sensible responses are:

  • Review food intake honestly, as portions drift back up.
  • Protect protein and keep active.
  • Check the basics: sleep and alcohol.
  • Discuss with your prescriber whether a dose step-up is appropriate, if you are not yet at maximum.

A plateau is not a reason to double doses or add unlicensed products. If it is the first weeks that worry you, appetite change and early loss vary widely: some people notice them in week one, others feel little until a higher dose. Both are normal. Early loss includes water weight, and mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

07 · Pens: can one last longer?

Can you use one Mounjaro pen for longer than 4 weeks?

No. A pen is a four-week supply, one weekly dose at a time, and you should not try to stretch it to eight weeks or two months. A pen in use can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or frozen.13

Unopened pens belong in the fridge until first use and keep until the printed expiry date.13 If you are unsure about a pen's condition, check the leaflet and contact the pharmacy rather than guessing. Changing how a pen is used, such as extra doses or stretching, is outside the licence, and we do not advise it.

Six months of treatment therefore means a series of pens, each reviewed and re-ordered in turn, with the dose set by your prescriber.

08 · After six months

What happens if you stop at six months?

You can stop at six months, but talk to your prescriber first. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

Stopping should always be planned with your prescriber, with diet and activity habits ready. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued lost a further 5.5% on average. 09 · Eligibility and the NHS

Who can take it, and NHS access

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples are high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.1 A prescriber will assess your BMI, medical history and current medicines first.

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
